    The Analysis of Fractures of Forearms, Wrists and Hands due to Traffic Accidents

    INTRODUCTION: Limb injuries have an important place in traffic accidents. Although it has been identified among the most frequently injured areas in hand, wrist and forearm in traffic accidents; often neglected due to more severe injuries. In our study, it was aimed to evaluate the cases with hand, wrist and forearm fractures after traffic accidents, to define the age and gender distribution of the injured people, to compare the location and types of fractures in and out of the vehicle. METHODS: In this study, the medical files of patients who applied to the Emergency Department of the Medical Faculty and the Forensic Medicine Outpatient Clinic after a traffic accident in the 7-year period between 01.01.2013 and 31.12.2019 were included in the study. RESULTS: In our study, ulna-radius diaphysis fractures, which usually occur as a result of a direct blow to the forearm, were seen in more than 70% of all cases in the presented series with forearm, wrist and hand bone fractures due to traffic accidents. Direct mechanisms were effective in 65.9% of the fractures, and direct fractures were prominent especially in pedestrians and passengers. DISCUSSION AND CONCLUSION: In our study, the founding of radius-ulna fractures, which usually occur as a result of a direct blow to the forearm in more than 70% of the cases in the presented series, the detection of effect of direct mechanisms in 65.9% of the fractures, and the seeing of in the foreground of direct fractures in pedestrians and passengers have been accepted as important indicators revealing that primary injuries are at the forefront rather than secondary injuries in traffic accidents

    Carpal Tunnel Release Surgery and Venous Hypertension in Early Hemodialysis Patients without Amyloid Deposits

    Aim. Carpal tunnel syndrome (CTS) is one of the frequent problems of the patients who underwent hemodialysis (HD). The role of venous hypertension due to arteriovenous fistula (AVF) has not been clarified completely; therefore, we aimed to investigate the role of venous hypertension due to AVF in hemodialysis patients who had CTS. Patients and Methods. We included 12 patients who had been receiving HD treatment for less than 8 years and the newly diagnosed CTS patients with the same arm of AVF. All patients were diagnosed clinically and the results were confirmed by both nerve conduction studies and electromyography. Open carpal tunnel release surgery was performed on all of them. Venous pressure was measured in all patients before and after two weeks of surgery. Results. There were significant differences before and after the surgery with regard to pressures (). After the surgery, all carpal ligament specimens of the patients were not stained with Congo red for the presence of amyloid deposition. Conclusion. Increased venous pressure on the same arm with AVF could be responsible for CTS in hemodialysis patients.     Bu çalısmada, ortaöğretimde tarih derslerinde, zaman kavramının öğretiminde öğretmenlerimizin karsılastığı sorunlar ve öğretmenlerimizin konu ile ilgili görüsleri ele alınmıstır. Tez; giris, tarih ve zaman, tarihsel zaman kavramının öğretimi, yöntem, bulgular ve yorumlar, sonuç ve öneriler olmak üzere altı bölümden meydana gelmistir. Çalısma, nicel ve nitel yöntemlerin kullanıldığı betimsel bir alan arastırmasıdır. Arastırma Aksaray il merkezindeki ortaöğretim kurumlarında görevli tarih öğretmenleri arasında yapılmıstır. Tarih öğretmenlerinin zaman kavramı öğretiminde karsılastıkları / karsılasabilecekleri sorunları yansıtan ifade ve durumları içeren anket uygulanmıs, ayrıca öğretmenlerimizin konuya iliskin baska görüs ve deneyimlerini almak için görüsme yapılmıstır. Sonuçlar tablolar halinde analiz edilmis ve yorumlanmıstır. Arastırmanın verileri yorumlandığında; hangi tür lisede olursa olsun, öğretmenlerimizin zaman kavramının öğretimi konusunda çesitli sorunlarla karsılastıklarını, öğrencilerin ilköğretimde öğrenilmesi gereken zaman kavramını tam anlamıyla kavramadıklarını, miladi-hicri, hicri-miladi kavramlarına bağlı islem yapamadıklarını, tarih ders kitaplarındaki konularda yer alan zaman ifadeleriyle kastedilenleri tam anlayamadıklarını ortaya çıkarmıstır. Arastırmanın sonucunda; öğretmenlerimizin zaman kavramı öğretiminde, öğrenciden, öğretmenden, ders müfredatından, araç-gereçlerden, ders saatlerinin az olmasından, kullanılan öğretim stratejilerinden, yöntem ve tekniklerinden kaynaklanan problemlerle karsılastıkları ortaya çıkmıstır. The thesis consist of six chapters, namely introduction, history and time, teaching historical time concept, methodology, findings and their interpretation, conclusion and suggestions. The study is a descriptive field study, where qualitative and quantative methods are used . The investigation was carried out in the secondary schools of history teachers in the iner city of Aksaray. Questionaire, reflecting problems that includes expressions and situations confronted or to be confronted by history teachers in teaching time concept, was carried out. Also teachers were interviewed to take their other experiments and opinions related to subject. Findings were analysed and interpreted . Interpreting the data obtained from the investigations, one can conclude that whichever secondary education it has been, teachers who are confronted with various problems about teaching time concept that must be tought wien they were attending primary school, and students couldn't deal with transfering pertaining of the Christian era to pertaining of to Hegira, and they couldn't understand exactly time expression that take part in history lesson. It can be concluded from the study that teachers confronted with problems in teaching time concept resulting from students, teachers, school curriculum, materials, limited lesson hours, educations strategies, methods and techniques

    Does retinitis pigmentosa relate with polycystic kidney disease?

    Autosomal dominant polycystic kidney disease (ADPKD) is one of the most common genetic disorders. A 21-year-old woman presented with temporary visual loss and gross hematuria. Fundoscopy showed retinitis pigmentosa, which was confirmed by electroretinogram. Her serum creatinine concentration was 1.6 mg/dl, and her renal ultrasonography revealed bilateral polycystic kidneys; she was unaware of having this condition. In this patient, there was probably an inherited ciliary defect, which may explain the association of ADPKD and retinitis pigmentosa
