End-stage chronic renal failure in patients treated by antireflux operation

全例逆流性腎症によると考えられる蛋白尿を術前から有していた.1例ではPaquin法, 2例ではPolitano-Leadbetter法が施行されていた.腎機能の急速な悪化は, 全例で思春期以降で, 2症例では初診時すでにsmall kidneyが認められていたWe report 3 patients progressed to end-stage chronic renal failure after antireflux surgery. Preoperatively, the 3 patients had proteinuria, probably due to reflux nephropathy. Surgical procedures used were Paquin's method in 1 patient and Politano-Leadbetter's method in 2. Rapid deterioration of renal function was observed from puberty in all patients. In addition, renal hypoplasia was suspected in 2 patients. We concluded that both reflux nephropathy and renal hypoplasia played important roles in the progression of renal insufficiency

作業療法学生の地域領域実習による経験─身体領域用のチェックリストを活用した地域領域実習における分析─

《目的》経験状況を把握するチェックリストから現在の地域領域実習における経験状況を明らかにし，地域領域実習用のチェックリストを検討することである．《方法》2019年度に地域領域実習を履修した4年生67名が活用したチェックリスト127項目について，見学，模倣前期，模倣後期，実施の4ステップそれぞれの経験の有無を調査し，さらに，各項目の経験，未経験の人数から経験率，未経験率を算出した．《結果》80％以上の学生が経験できた項目は25項目，それ以外の50％以上の学生が経験できていた項目は36項目あった．また，80％以上の学生が未経験だった項目は23項目あり，それ以外の50％以上の学生が未経験だった項目は43項目あった．《結論》半数以上の項目は学生の経験率が50％以下であり，地域領域実習に適していないチェックリストの項目が複数存在する可能性が考えられた．そのため，本研究対象の地域領域実習で経験率の高い項目を中心に，【オリエンテーション】4項目，【作業療法士の求められる適性】14項目，【情報収集】4項目，【評価の解釈】2項目，【リスク管理】8項目，【環境因子】6項目，【個人因子】5項目，【活動と参加】6項目，【身体機能・身体構造】3項目の計52項目からなる地域領域実習チェックリストを作成した．Objective: The aim of this study is to clarify the current situation of clinical community internship experience using a checklist, to grasp the situation of the experience, and to examine the checklist of clinical community internship.Method: We used 127 checklist items used by 67 4 th graders who received clinical community internships in 2019 to investigate whether they have any experience in each of the four steps of study by observation, early imitation, late imitation, and implementation. The experience and inexperience rates were calculated from the number of inexperienced people.Results: There were 25 items that more than 80% of students can experience and 36 items that more than 50% of students can experience. In addition, 23 items were inexperienced by more than 80% of students, and 43 items were inexperienced by more than 50% of students.Conclusions: More than half of the items have less than 50% student experience and may have multiple checklist items that are not suitable for clinical community internships. Therefore, focusing on items with a high experience rate in clinical community internship for this study, we obtained 4 items for orientation, 14 items for aptitude required for occupational therapists, 4 items for information collection, 2 items for interpretation of evaluation, 8 items for risk management, 6 items for environmental factors, 5 items for individual factors, 6 items for, activities and participation, 3 items for, physical function / body structure. A clinical community internship checklist consisting of 52 items has been created

We have measured the mobility of positive ions in several gas mixtures frequently used in proportional and drift chambers, namely Ar-(OCH3 )2CH2, Ar-(OCH3)2CH 2-IsoC4H10, Ar-CH4, and Ar-CO 2. In all gas mixtures considered, Blanc's law of positive ion mobility has been verified, except for a very small methylal concentration in argon-isobutane-methylal. The law permits the calculation of ion mobility in any mixture of the gases considered
