Toe\u27s power evaluation in the nursing physical assessment (The 3rd report) : Research review of the toe\u27s function for the fall prevention
From a Japanese Medical Central Magazine Web version, we searched "physical assessment and lower limbs function" or "physical assessment and toes function" in a keyword. We found 38 literatures in total, and examined the outline of all articles. In the literature about lower limbs function, they investigated the patients with specific disease or identical operation. These reports are most common with 15 articles, and other literatures which intended for elderly people 65 years or older were 13 articles, and healthy subjects were four. In six articles about toes function, the subjects were normal adults, and there was no article which was approached for elderly people or specific disease. Among 38 literatures in total, we found 29 articles that assumed fall prevention (76.3%), but there was no article in view point of the nursing. Moreover, there was no article in the literature for the lower limbs function. However, in the literature about the toes function, we found three articles which have been paid attentions to standing position or walking balance. In the nursing physical assessment, the attention to lower limbs function or toes function was extremely rare, but it was thought to be necessary for the fall prevention.医学中央雑誌Web版から,「フィジカルアセスメント(理学的検査)・下肢機能」または「フィジカルアセスメント(理学的検査)・足趾機能」をキーワードで検索し,合計38件の文献について,論文の概略を検討した.下肢機能について報告された32件の文献では,特定の疾患を有する患者または同一の手術を受けた患者を対象に調査した文献が15件と最も多く,65歳以上の高齢者を研究の対象としている文献は13件,健常者を対象としている文献が4件であった.また足趾機能に関する6件の論文ではすべてが健常成人に対するものであり,高齢者や疾患を有する患者に対してアプローチした文献はなかった.合計38件の文献のうち,転倒予防を視点とした論文は29件(76.3%)であったが,看護の立場から検討した論文はなく,また下肢機能に関する文献の中で,足趾にまで言及している論文は1件のみであった.しかし足趾機能のみに関する文献の中で,立位歩行バランスに着目している文献を3件認めた.看護フィジカルアセスメントの中で,下肢機能あるいは足趾機能への注目度は極めて低いが,転倒予防の立場で考えるとこれらの機能に対するアセスメントも必要ではないかと考えられた
Construction of the Walking Motion Model for Animated Character\u27s Motion
CG による人物動作生成を簡易化する為には, 少ないルールセットで人物動作を記述し, 簡易的なインターフェースで動作を制御する事が可能なモデルが必要である. そこで, モーションキャプチャで収録した歩行動作データに対し, 可動部位の連動性に着目して分析を行った結果, 歩行動作は幾つかのレベルの独立な動作群として構成されている事が確認された. さらに, 「速さ」「大きさ」「姿勢」のような物理的な特徴を表す特徴量成分の抽出を行った. そして, 歩行動作において独立している動作群に対し, 抽出した特徴量成分を様々に付加する事で, 多様な歩行動作を制御可能な歩行動作制御モデルを構築した. 最後に, 本モデルの評価実験を行い, モデルが多様な歩行動作を制御可能である事を確認した.修士論
Toe\u27s power evaluation in the nursing physical assessment (The 2nd report) : Examination in the locomotive syndrome with fall fracture
Based on the medical record of 18 patients who underwent rehabilitation after the fall fracture associated with the locomotive syndrome, we collected the data of lower limbs function (lower limbs muscular strength, toes pinch power, and toes ten-second test) and records by the examination with center of gravity unrest meter. As a whole, the individual lower limbs muscular strength was almost normal, but the bilateral toes pinch power was evaluated at 1.3±0.6kg (Yamashita\u27s classification group-1: The decrease of lower limbs muscular strength and the foot part function), which showed a low-level obviously.However, the toes ten-second test was 14.8±4.2 times of right, 14.2±4.3 times of left, which were similar with a normal value. The total trace length on the examination of the center of gravity unrest meter was strongly correlated with patient\u27s age (r=0.905, p=0.001) in the evaluated nine patients, but did not correlate with the results of the test for toes pinch power, nor toes ten seconds.運動器症候群に関連する転倒骨折後の患者に対してリハビリテーションを施行した18例の患者の診療記録をもとに,下肢機能(下肢筋力,足趾挟力,足趾10秒テスト)と重心動揺計検査による記録を収集した.全体として個々の下肢筋力は概ね正常であったが,足趾挟力は両側とも平均が1.3±0.6kg(山下の分類によるグループ1:下肢筋力および足部機能の低下あり)となり,明らかに低値を示した.しかし足趾10秒テストは右14.8±4.2回,左14.2±4.3回であり,標準値とほぼ同じであった. 9例に実施された重心動揺計の検査では総軌跡長は患者背景における年齢と強い相関関係( r=0.905,p=0.001)が認められたが,足趾挟力,足趾10秒テストの結果とは相関しなかった.以上のことから,運動器症候群に関連する転倒患者には個々の筋力だけでは評価しきれない足趾の巧緻運動障害が潜んでいる可能性が示唆され,運動器に対する看護のフィジカルアセスメントにおいて足趾挟力を測定する意義が認められた

Subject or Citizen?: African Citizenship between Resilience and Resistance
This paper examines the subjectivity of African citizenship in the term of resilience, which we need to argue under the postcolonial conditions in Africa. At first, the concept of ”resilience” will be considered critically through the author’s experience in the aid work in East African regions. Secondly, the dilemma of the African citizenship is proposed by looking at Mamdani’s authentic text of “Citizen and Subject”. The colonial legacy of “Divide and Rule” has brought the condition of so-called “the bifurcated states” in African countries and still been suffered by the result of it, which are clientelism, conflicts, and, so-called the collapsed state’s condition. In that sense, African subjectivity can be both categorized as one of resilience and resistance. I would like to point out the idea of resilience, on one hand, contains the ideal notion of modern citizenship for the African Nation-States, on the other, it also introduces us to the subject of resistance under the current political environment in Africa. Although the most of cases are picked up by reviewing recent ethnographic notes in Uganda, partly I will describe the case in my research field at a slum area in Kampala,which will show us the highly flexible and plastic sense of the self, which would be the basis of the African citizenship
