    A multimechanistic antibody targeting receptor-binding sites potently cross-protects against influenza B viruses

    流感病毒HA是研制流感药物和流感疫苗的重要靶标,但HA具有高度变异性,如何在高变异HA中找到不变之处,即高度保守表位,是研制流感特效药物和广谱疫苗的关键。近年来国外报道的流感HA广谱中和单抗的识别位点均在较为保守的HA茎部区,而针对流感病毒与细胞受体结合部位的HA头部区尤其是RBS区,一直未能发现广谱中和抗体。夏宁邵教授团队通过探索多种免疫策略和筛选策略,成功筛选出一株广谱中和单抗12G6,识别一个位于HA头部RBS上的全新保守性表位。体外实验显示12G6人源化改造的C12G6抗体能高效中和1940-2016年间世界各地历年流行的代表三个遗传变异亚系的18个乙型流感病毒代表株对细胞的感染,并能保护小鼠致死性感染,治疗效果显著优于已报道的代表性抗体以及抗流感药物;C12G6与“达菲”联合用药具有明显的协同效果。此外,雪貂感染模型的预防和治疗效果进一步证实了C12G6作为抗体药物的治疗潜能。研究还显示该表位是病毒感染复制的关键表位,该位点的突变会造成病毒毒力显著下降。最后,研究揭示了C12G6通过五种不同的抗病毒作用机制发挥作用,提示其高效的抗病毒活性得益于多机制协同效应,这也是目前国内外第一次发现一个流感抗体能通过如此全面的抗病毒机制发挥作用。 该发现为研制能抵抗各种变异株的乙型流感特效治疗药物和通用疫苗带来新希望。 该研究工作依托分子疫苗学和分子诊断学国家重点实验室(厦门大学)、国家传染病诊断试剂与疫苗工程技术研究中心、厦门大学养生堂生物药物联合实验室完成。陈毅歆副教授、夏宁邵教授为该研究论文的共同通讯作者。在读博士研究生沈晨光、陈俊煜、李睿、王国松和硕士研究生张梦娅等为共同第一作者。【Abstract】Influenza B virus causes considerable disease burden worldwide annually, highlighting the limitations of current influenza vaccines and antiviral drugs. In recent years, broadly neutralizing antibodies (bnAbs) against hemagglutinin (HA) have emerged as a new approach for combating influenza. We describe the generation and characterization of a chimeric monoclonal antibody, C12G6, that cross-neutralizes representative viruses spanning the 76 years of influenza B antigenic evolution since 1940, including viruses belonging to the Yamagata, Victoria, and earlier lineages. Notably, C12G6 exhibits broad cross-lineage hemagglutination inhibition activity against influenza B viruses and has higher potency and breadth of neutralization when compared to four previously reported influenza B bnAbs. In vivo, C12G6 confers stronger cross-protection against Yamagata and Victoria lineages of influenza B viruses in mice and ferrets than other bnAbs or the anti-influenza drug oseltamivir and has an additive antiviral effect when administered in combination with oseltamivir. Epitope mapping indicated that C12G6 targets a conserved epitope that overlaps with the receptor binding site in the HA region of influenza B virus, indicating why it neutralizes virus so potently. Mechanistic analyses revealed that C12G6 inhibits influenza B viruses via multiple mechanisms, including preventing viral entry, egress, and HA-mediated membrane fusion and triggering antibody-dependent cell-mediated cytotoxicity and complement-dependent cytotoxicity responses.     This paper summarized the treatment effect of thunder-fire moxibustion combined with acupoint massage on the rehabilitation of facial nerve function in a patient with facial paralysis of wind-cold pattern. Based on the syndrome differentiation, a combination of traditional Chinese Medicine external techniques including thunder-fire moxibustion and acupoint massage was carried out to relieve the clinical symptoms and improve the recovery of facial nerve function of the patient with facial paralysis of wind-cold pattern. (本文总结1例采用雷火灸配合手法穴位按摩改善风寒证面瘫患者面神经功能的护理经验。在常规护理的基础上, 根据患者辨证采取雷火灸配合手法穴位按摩, 通过中医外治法精准作用于病灶, 增强经络的感知传导, 改善面神经活动能力, 进而促进患者面瘫的康复。

    T2* mapping评估克罗恩病炎症活动性的应用研究

    目的】探讨T2 * mapping对评估克罗恩病(CD)炎症活动性的应用价值。【方法】98例CD患者行MR肠道造影及 T2*WI扫描。在T2 * mapping上测量病变肠壁的T2 * 值,以MR活动性指数(MaRIA)为CD炎症活动性的参考标准,分析T2 * 值 与CD活动性的关系。【结果】160个肠段中,缓解期CD(MaRIA < 7)26段,轻度活动期CD(7 ≤ MaRIA < 11)23段,中-重度活 动期(MaRIA≥11)111段。三组CD肠壁的T2*值差异有统计学意义(均P < 0.05)。160个肠段的T2*值与MaRIA具有较好的 相关性(r = 0.743,P < 0.05)。ROC曲线分析显示T2 * 值在鉴别缓解期和活动期CD(AUC = 0.877)以及鉴别缓解期-轻度活 动期和中-重度活动期CD(AUC = 0.848)均具有较高的诊断准确性,其中T2 * 值选择阈值为20 ms时诊断轻度和中-重度活动 期CD的敏感性为74.5%,特异性为84%。【结论】T2 * mapping的定量指标T2 * 值与CD活动性有较好的相关性,可准确诊断CD 的炎症活动性

    Emerging infectious disease is characterized by sudden and rapid spread, and failure to curb its development in the first place would pose a serious threat to social stability and human health. The hierarchical medical system can realize rapid triage, better separate infectious patients, suspected patients and non-infectious patients, and optimize diagnosis, treatment and nursing work. Taking COVID-19 epidemic as an example, this paper will discuss the present situation of the hierarchical medical system in control and nursing management of sudden infectious diseases, summarize the problems existing in current graded diagnosis and treatment system of China, and propose proposals to promote the development and improvement of the hierarchical medical system. (突发性传染病具有发生突然、传播速度快等特点, 若不能在第一时间采取措施遏制其发展, 将会对社会稳定和人类健康构成严重威胁。分级诊疗制度可以实现快速分诊, 将传染性患者、疑似患者、非传染性患者分开, 优化诊疗、护理工作流程。本研究以新型冠状病毒肺炎疫情为例, 总结分级诊疗制度在突发性传染病护理工作中研究进展, 分析我国目前分级诊疗制度存在的问题, 并提出促进分级诊疗制度发展和完善的刍议。

    应用丝网印刷和共烧结制备LaNi0.6Fe0.4O3-&delta;(LNF)-Gd0.2Ce0.8O2(GDC)梯度复合阴极/Gd0.2Ce0.8O2/Sc0.1Zr0.9O1.95(ScSZ)/Gd0.2Ce0.8O2/LaNi0.6Fe0.4O3-&delta;(LNF)-Gd0.2Ce0.8O2(GDC),组成梯度复合阴极对称电池. 实验表明,在750 oC工作温度下单层70%LNF-30%GDC(文中均指质量百分比)复合阴极的极化电阻为0.581 &Omega;&middot;cm2,而三层60%LNF-40%GDC/70%LNF-30%GDC/100%LNF复合阴极的极化电阻最小(0.452 &Omega;&middot;cm2). 由于阴极组成在ScSZ电解质和LNF阴极之间呈梯度变化,因此获得了最佳的阴极/电解质界面,大大加快了三相界面或气体/阴极/电解质三相接触点反应区的扩散,其电荷传递电阻Rct和浓差极化电阻Rd均减小,因而具有最低的阴极极化电阻值.A LNF-GDC composite cathode with a gradual change in the composition between ScSZ electrolyte and LNF cathode was fabricated to reduce the cathode polarization resistance (Rp). The gradual change in composition between ScSZ electrolyte and LNF cathode shows the decreases in the charge transfer resistance (Rct) and gas phase diffusion resistance (Rd). The results revealed that the Rp value, measuring 0.452 &Omega;&middot;cm2 at 750 &deg;C, was the lowest for LNF-GDC composite cathodes with three layers and gradient changes in composition between ScSZ and LNF (Cathode C),, whereas the Rp value of 70%LNF-30%GDC composite cathodes with one layer (Cathode A) was 0.581 &Omega;&middot;cm2. The reduction in Rp for the LNF-GDC composite cathodes with three layers and gradient changes in composition between ScSZ and LNF may be related to the fact that the microstructure of the cathode/electrolyte interfaces is significantly improved, resulting in the increase in the area of triple phase boundaries (TPBs), which enhanced the surface exchange of oxygen.
