鸡尾酒抗体D2-40/CD34-CK对淋巴结检出数量不足结直肠癌标本的临床病理评价

目的探讨鸡尾酒抗体D2-40/CD34-CK双重免疫组化染色在淋巴结检出数量不足的结直肠癌(colorectal cancer,CRC)标本脉管侵犯评估中的应用及意义。方法收集133例淋巴结检出数量<12枚的CRC标本,分别行HE染色及鸡尾酒抗体双重免疫组化染色,比较两种方法在脉管侵犯筛查效果中的差异,并分析应用鸡尾酒抗体免疫组化染色证实的脉管侵犯与临床病理特征及患者总生存期(overall survival,OS)的关系。结果 (1)鸡尾酒抗体D2-40/CD34-CK双重免疫组化染色及HE染色对脉管侵犯的检出率分别为42. 9%(57/133)和21. 8%(29/133),差异有显著性(P <0. 001)。(2)鸡尾酒抗体D2-40/CD34-CK双重免疫组化法证实的脉管侵犯与Dukes分期、浸润深度、临床分期、淋巴结转移、肿瘤出芽相关(P <0. 05)。(3)脉管侵犯、侵犯位置深度、程度及脉管侵犯灶数≤2个灶且癌栓细胞数量≥5. 5个与患者OS密切相关(P <0. 05)。结论鸡尾酒抗体D2-40/CD34-CK双重免疫组化染色对脉管侵犯评估优于常规HE染色切片观察,与肿瘤分期、淋巴结转移及出芽情况密切相关,是患者预后的独立影响因素,可作为淋巴结检出数量不足CRC病例的补充检测指标。国家自然科学基金青年科学基金(81301787

MCTミルクが有効であった蛋白漏出性胃腸症の乳児例

We report a case in a previously healthy 5 months boy of protein-losing gastro enteropathy, presenting with poor feeding and hypoalbuminemia. At the hospitalization, although his albumin was 2.0g/dl, he had no proteinuria or cardiac dysfunction. Three days after his hospitalization, he got edema and his albumin dropped 1.3g/dl. Scintigraphy using 99mTc-labeled human serum albumin showed the leakage of albumin from the digestive tract and he was diagnosed as Protein-losing gastro enteropathy. Primary Intestinal Lymphangiectasia was diagnosed, since inflammatory bowel diseases and secondary obstruction of lymph ducts were negative from the results of blood tests, endoscopy and pathology. The present case was successfully treated by MCT milk. Endoscopy is useful not only for differential diagnosis of protein-losing gastro enteropathy but also for the improvement of the compliance of the patient for appropriate diet therapy owing to the final diagnosis. Clinical evaluation of cefixime in the treatment of acute uncomplicated cystitis

成人女子の急性単純性膀胱炎154例に対しCFIXを投与しその治療成績ならびに安全性につき検討した.1) UTI薬効評価基準を満たした94例での総合臨床効果は著効76例, 有効17例, 無効1例であり, 有効率は98.9%であった.2)臨床症状, 膿尿および細菌尿に対する効果はそれぞれ98.9%, 97.9%, 97.9%であった.3)副作用は1例に頭重感がみられたのみで, 臨床検査値異常は3例にみられた.4)急性単純性膀胱炎に対しCFIXは有用かつ安全な薬剤と思われたCefixime (CFIX) was administered 200 mg (2 divided doses) per day to 154 female patients with acute uncomplicated cystitis. Ninety four of the 154 patients who satisfied the criteria proposed by the UTI Committee, Japan were examined for the efficacy of the treatment, but 151 of 154 patients were included in the study of the side effects of the drug. The overall clinical efficacy was excellent in 80.8% and moderate in 18.1% of the patients. Ninety five (97.9%) of 97 strains which were isolated from the patients were eradicated in the urinary specimens by the treatment. Subjective side effects were observed in only one (0.7%) of the 151 patients, who complained of a sensation of heaviness in the head. No drug-related aggravation in the laboratory test was observed except for three patients who showed mild elevation glutamic pyruvic transaminase, alkaline phosphatase and blood urea nitrogen. These results showed that the oral administration of CFIX was excellent and satisfactory in the treatment for acute uncomplicated cystitis

Fishing Trials for Anchovy using Large Mesh Sized Model Purse Seines

For developing large mesh purse seines the specification of net mesh size is required. However, there are no standards methods of combining small and large meshes in the netting panels and fish catch experiments are required to examined the behavior of fish schools to the large mesh netting sections. The experiment for anchovy, Engraulis japonica was carried out in a model net basin using model purse seine nets with six different mesh sizes. The model net had the following general dimensions : cork line length of 14.5m, depth 2.4m with wing side netting comprised of large mesh 60mm~1440mm net and the bunt side netting 18mm mesh. The net was shot and hauled from a carriage situated in the centre of the basin. After the setting the net manually, the purse line was hauled up mechanically and the behavior of the fish school observed by 4 television cameras. Analysis of fish behaviour videos showed that fish did not escape from 60mm - 600mm large mesh net until the end of shooting operation. In 1440mm large mesh net, part of the school escaped after the shooting operation through the large meshes. The remaining fish schooled in the middle of the net until the net volume was reduced during purse line hauling whereupon fish escaped through the space in the leadline. For the purse seine net targeting anchovy, a mesh size of 600 mm can be used in the wingside of the net without loss of fish.articl

Clinical experience of enoxacin complicated urinary tract infection

Enoxacin (ENX) was administered to 69 patients with complicated urinary tract infections (UTI). Clinical efficacy and safety were evaluated by the criteria proposed by the UTI Committee, Japan. The overall clinical efficacy was excellent in 60.9%, moderate in 10.1% and poor in 29% of the patients. Of the 76 strains isolated from the patients 61 strains (80.3%) were eradicated. Subjective side effect was observed in one patient who complained of slight nausea. No drug-related aggravation in the laboratory test was observed. These results showed that ENX was effective and safety for the treatment of complicated urinary tract infection
