척수손상자의 일상생활동작 추적조사

본 연구의 목적은 척수손상자의 퇴원시와 추적시 일상생활동작 수행능력을 비교하여 변
화가 있는지를 알아보기 위함이었다. 연구대상은 1988년 2월부터 1992년 2월까지 연세대
학교 의과대학 세브란스병원 재활의학과에서 퇴원한 척수손상자중 추적조사가 가능했던 3
0명을 대상으로 하였다. 일상생활동작 수준을 평가하기 위해 modified Barthel Index를
사용하였다. 수집된 자료는 짝비교 t-검정과 Mann-Whitney 검정으로 분석하였다.
연구결과는 다음과 같다.
1. 대상자의 손상시 연령분포는 20-30대가 22명(73%)을 보여 가장 많았고, 손상원인은
자동차 사고가 14명(46.7%)으로 가장 많았다.
2. 퇴원시의 일상생활 동작 점수는 평균 67.7 23.0에서 추적시 73.0 22.9로 퇴원시에
비해 추적시 기능 호전이 있었다.
3. 퇴원시에 비해 추적시 일상생활 동작 점수의 증가는 옥외활동을 많이 할수록, 재원
기간이 짧을수록, 통증과 경직이 없을수록 그리고 사지마비 보다는 하지마비에서 높게 나
타났고 통계학적으로 유의한 차이가 있었다.
위와 같은 결과로 볼 때 척수손상자의 일상생활동작은 재활 센터에서 퇴원한 후 사회생
활에 참여함으로서 더욱 호전될 수 있으므로 척수손상자에게 적극적인 사회활동을 할 수
있도록 유도하여야 한다.
The follow-up Study of Activities of Daily Living for Spinal Cord Injured People's

The purpose of this study was to evaluate change between discharge and follow-up
status in spinal cord injured people's ability to perform activities of daily
living(ADL)
The subjects were 30 cases with spinal cord injury who were discharged from
Rehabliitation Center Severance Hospital, Yonsei University College of Medicine,
during the period from February 1988 to February 1992.
The modified Barthel Index(MBI) was used to assess ability to perform ADL.
The data were analysed using paired t-test and Mann-Whitney test.
The results were as follows:
1. The largest age group(22 cases, 73%) at onset was the second and third decade,
and the most common cause of spinal cord injury in this stydy was traffic accident
with 14 cases(46.7%).
2. Mean MBI scores improved from 67.7±23.0 at discharge to 73.0±22,9at for
low-up.
3. The MBI scores at follow-up were significantly higher than those at discharge
in cases with more outdoor activities, in cartes who stayed in the hospital shorter
period and in cases who have no pain and spasticity. Also, MBI scores were
significantly higher for paraplegics compare with quadriplegics.
As seen in the above results, the spinal cord injured people's level of ADL can
improve even more after discharge from a rehabilitation center if the people takes
part in social activities. Therefore, the spinal cord injured people's should be
encouraged to take positive steps towards resuming an active social life.restrictio

Rapid Inflection Point Method of Consolidation Testing
일반적으로 점성토의 압밀계수는 하중단계마다 일차압밀이 100% 완료되기까지 충분한 재하시간(24h)을 필요로 하는 표준압밀시험을 통해서 구할 수 있다. 하지만, 표준압밀시험으로는 간극수압을 직접 측정하기가 어렵고, 압밀완료시간이 최소한 1주일 이상이 소요된다. 따라서, 신뢰성 있는 값을 확보하면서 신속한 압밀시험 방법의 개발이 요구된다.
본 연구에서는 Inflection Point Method(Francis R. Cour 1971)[2]를 이용하여 신속히 압밀계수를 구하는 Rapid Inflection Point Method에 대한 연구를 수행하였다. 본 연구의 결과로 얻어진 압밀계수는 Taylor방법, Casagrande방법, Rectangular Hyperbolar방법으로 구한 압밀계수와 비교하였고[1], Rapid Rectangular Hyperbolar(A. Sridharan 등, 1999)방법[8]으로 구한 압밀계수와 비교·분석하여 그 적용성을 검토하였다. 또한, 본 연구를 통해서 압밀시간 단축에 관한 타당성을 입증하였다.This study presented the new method for evaluating the coefficient of consolidation by using inflection point method which was based on the fact that time factor, T corresponding to the inflection point of a semilogarithmic plot of a time curve is fixed and equals to T = 0.405 at 70% consolidation. In the proposed method, the next load increment is applied as soon as the necessary time reqaired to identify the inflection point. Thus, the coefficient of consolidation may be easily evaluated. The time required to complete the testing using this rapid consolidation method could be as low as 1∼5 hours compared with 1 or 2 weeks in the case of the conventional consolidation test.
