    Randomized Controlled Trial Comparing the Usefulness of Endoscopic Ultrasound Processor

    [Background] Although endoscopic ultrasonography (EUS) is a useful tool for diagnosing pancreatobiliary diseases, not many facilities perform this technique as it is difficult to master. Currently, two new EUS systems exist: EU-ME2/GF-UCT260, manufactured by Olympus, and SU-1/EG-580UT, manufactured by Fujifilm. Some reports have compared new EUS models to older versions, but the operability and image quality of these two latest systems have not been compared. Our study aimed to compare the usefulness of these two types of EUS. [Methods] Forty consecutive patients were recruited and randomized in a two-arm clinical trial; Arm 1: EU-ME2/GF-UCT260 was used only for observation and SU-1/EG-580UT for EUS-fine needle aspiration (FNA); Arm 2: SU-1/EG-580UT was used only for observation and EU-ME2/GF-UCT260 for EUS-FNA. Using a crossover design, we evaluated image findings, ease of scope insertion, and visibility of the gastrointestinal (GI) tract. Each procedure was scored using a 5-point scale (Clinical Trial ID: UMIN000031373). [Results] SU-1/EG-580UT was significantly better in terms of lesion-delineating capacity: lesion border (P < 0.001), internal echo (P < 0.001). Significantly easier scope insertion was observed with SU-1/EG-580UT with respect to any insertion into the piriform recess (P = 0.018), the pylorus ring (P < 0.001), and the superior duodenal angle (P < 0.001). Visibility during gastrointestinal observation was also significantly better with the SU-1/EG-580UT (P < 0.001) than with the EU-ME2/GF-UCT260. [Conclusion] SU-1/EG-580UT EUS demonstrated superior performance during ultrasonic endoscopic GI observation, operability, and ultrasonic image quality. The result of the superior ultrasound imaging quality of SU-1/EG-580UT EUS will aid in the identification of small pancreatic malignancies with unclear borders and prove useful in evaluating mural nodules of IPMN in detail. These findings could result in an increased use of EUS and improve identification and prognosis of patients with pancreatobiliary diseases.journal articl

    2022年度の朝日大学歯学部解剖実習において、81歳男性(死因:老衰)の胸部に、出現率6%とされる破格筋である胸骨筋が両側性かつ稀な形態で観察されたので報告する。右側の胸骨筋は長さ17.4cm、最大幅4.12cm で、腹直筋鞘前葉から起始し、胸鎖乳突筋胸骨部の起始腱、左側大胸筋鎖骨部の起始腱膜,胸骨柄前面の骨膜へ停止した。また、外側胸筋神経と肋間神経前皮枝が後面から進入した。左側の胸骨筋は長さ13.25cm、最大幅4.6cmで、内外側の二つの筋束からなり、内側の筋束は左腹直筋鞘前葉から、外側の筋束は乳頭下で大胸筋筋膜から起始し、合流して大胸筋前面の筋膜へ停止した。大胸筋停止の付近からは、筋性腋窩弓が起始し、広背筋の外側縁へと停止した。胸骨筋の後面へ進入する神経として、外側胸筋神経が観察された。本症例でみられた両側性の胸骨筋は発育良好で、先行研究と比較し、日本人に観察された胸骨筋として最長の可能性がある。また、起始と停止の位置から、胸骨筋の中でも非常に稀な形態であることも明らかになった。胸骨筋の由来として、本症例でみられた筋とその支配神経の形態から、大胸筋の一部、または他の哺乳類にみられる皮幹筋の遺残である可能性が示唆された。In the present report, we describe the finding concerning sternalis muscles, a rare muscle with an appearance rate of 6%, in the chest of an 81-year-old man (cause of death : senility) during an anatomical class at the Asahi University, School of Dentistry in 2022. The length of the right sternalis muscle was 17.4cm and its maximum width was 4.12cm, originating from the anterior lobe of the rectus abdominis muscle sheath and then inserting in the origin tendon of sternocleidomastoid muscle’s sternal part, the origin tendon of the left pectoralis major muscle’s clavicular part, and the periosteum of the front of the manubrium of sternum. The lateral pectoral nerve and the anterior cutaneous branch of the intercostal nerve also entered from the posterior aspect. The length of the left sternalis muscle was 13.25cm and its maximum width was 4.6cm. It consisted of two muscle bundles, inner and outer, with the inner bundle originating from the anterior lobe of the left rectus abdominis muscle sheath and the outer bundle originating from the pectoralis major muscle’s fascia under the nipple, which merged and then stopped at the fascia of the anterior surface of the pectoralis major muscle. From the vicinity of the insertion of the pectoralis major muscle, the muscular axillary arch originated and stopped at the lateral border of the latissimus dorsi muscle. The lateral pectoralis nerve was observed as the nerve entering the posterior aspect of the sternalis muscle. The bilateral sternalis muscles seen in this report were well developed and compared to previous studies, may be the largest ones observed in Japanese. Based on the location of the origin and stop of the muscles, his sternalis muscles clearly showed a very rare form. As for the origin of this muscle, the morphology of the muscle and its innervating nerve found in this report suggests that it may be a part of the pectoralis major muscle or a remnant of the cutaneous muscle of trunk in other mammals.departmental bulletin pape

    YUBAメソッド発声練習用DVD教材による女子中学生の声域拡張に関する検証

    application/pdf三重県内では歌唱活動が非常に盛んである。多くの学校で校内合唱コンクールヘの熱心な取り組みが行われているが、授業において基礎発声指導を行う十分な時間がないため、多くの子どもたちが歌唱に対して何らかの問題を抱えている。また、一斉授業において、教師がピアノを弾きながらの歌唱指導は非常に難しい。このような課題の解決に向け、筆者は発声練習に有効なメソッドを選択した。筆者と1~2名の学校教員により、中学生の声域拡張への効果の検証を行った。YUBA理論(発声制御理論)を基にしたYUBAメソッド発声練習用DVD教材(初級編)を使用して発声練習を1か月間(週に3回)行い、対象者の声域をこの発声練習期間の開始前と終了後に測定した。対象者は女子中学生20名(13.05± 1.05歳)とした。その結果、対象者の声域は平均5.4半音拡がり、3オクターブ以上の声域を持つ者、練習開始前では7名(35%)であつたのに対し、練習終了後には16名(80%)となった。t検定を行ったところ、高音への拡張はP=4.813e-07、低音への拡張はP=0.0006898、そして声域全体の拡張はP=3.691e‐07となり、3項目全てにおいて有意な差が認められた(P<0.01)。また、全対象者の声域が拡張した。本研究の結果、YUBAメソッドのDVD教材は女子中学生の声域拡張に有効であることが検証された。Chorus performances are very popular at junior high schools in Mie prefecture. Although many schools make efforts to set up school chorus competitions, many teachers can't have enough time to provide basic vocal instructions. Therefore, not a few children hold the trouble in singing. Vocal instructions to each student are difficult while a teacher plays the piano. In order to improve this situation, the author selects an effective method of vocal training. We, the author and one or two teachers of the school, examined the effectiveness of expanding a vocal range of junior high school students. We used The YUBA Method DVD for beginners based on The YUBA Theory (control theory for voicing). The vocal exercises using The YUBA Method were carried out for one month (3 times / week). I examined vocal ranges of the subjects before and after this training period. Subjects were 20 junior high school girls (13.05±1.05 years old). As a result, the subjects'vocal ranges expanded 5.4 semitones on the average. Not 7 subject (35%) before the training period but 16 subjects (80%) were able to have the vocal ranges of more than three octaves after the training period. The results of t-test of this research, the grown to high note is P=4.813e-07, the grown to low note is P=0.0006898, and the expansion of a whole range is P=3.691e-07. Each of these had a significant difference (P<0.01). Moreover, all the subjects'vocal ranges were expanded.     Efficacy of Ozone to Reduce Chlorinated Disinfection By-Products in Quebec (Canada) Drinking Water Facilities

    <div><p>The impact of ozonation on the reduction of chlorinated disinfection by-products formation was investigated in 15 full-scale and lab-scale drinking water facilities of Québec (Canada). Total trihalomethanes (TTHM) and the sum of six haloacetic acids (HAA6) were measured after chlorination under uniform formation conditions (UFC). Results showed that before ozonation TTHM and HAA6 average concentrations were 89.4 and 45.3 μg/L, respectively. In full-scale ozonation conditions TTHM-UFC and HAA6-UFC reductions averaged respectively 27 and 32%. After lab-scale ozonation at a O<sub>3</sub>/C of 1:1, a decrease of only 9% of TTHM was calculated, while for HAA6, reduction was not significantly impacted (30%). For BDOC, average concentrations of 0.13, 0.46, and 0.69 mg C/L were measured before and after and lab-scale ozonation, respectively. Chlorine demand (Cl<sub>2</sub>D) and immediate ozone demand (IOD) were found to be the most appropriate indicators to evaluate NOM reactivity after ozonation.</p></div
