    Medical Data Management System Using Blockchain and Proxy Re-Encryption

    In Japan, our medical data is distributed and managed. Those data will not to be using when visiting medical institution. Since it is important to know the patient’s past medical history in medical case, it is necessary to be able to disclose those data at the medical institution where patient is consulted. So, we propose a method that allows patients to autonomously manage medical data and share it with medical institutions. By managing medical data by patients, it is possible to disclose various data such as healthcare information acquired by the patient, history of medical practices received before, and medicines taken by patients to medical institutions trusted by patients. It also has the advantage of encouraging patients to participate in research. However, managing medical data using a general centralized network concentrates costs on the administrator. In addition, it is difficult to obtain patient consensus because all authority is delegated to the administrator. Therefore, in this research, we used a blockchain that realized a decentralized network. There are some precedents for medical data management using blockchain, and there are issues regarding encryption and permission. In this study, encryption and permission were realized by using proxy re-encryption with attack resistance. In the evaluation experiment, from the viewpoint of usability, the effect of proxy re-encryption used in this study on transactions was investigated by focusing on the execution time.     Study on Quantification of Quality Characteristics of Project Schedules

    In the manufacture of products, we are required to meet three requirements, they are QCD (Quality and Cost, Delivery date). And QCD are required in the project management. However, in the present circumstances, it’s just a rough evaluation of the project schedule at delivery date and cost. In recent years, large-scale and complicated projects have become difficult to evaluate even in terms of using delivery date and cost. So, we need a rational method for quantifying the risk of overdue delivery (delay risk). At present, the evaluation of delay risk can only be left to the experience and intuition of administrators because there is no rational method to expose the risk of delay at the work level. Therefore, our laboratory introduced the concept of “delay risk tolerance”. It is difficult to quantification of delay risk, but it is easy to quantification of “delay risk tolerance”. And we proposed a method to evaluate delay risk based of the magnitude of “delay risk tolerance”and convert it into cost. In the evaluation experiment, the validity of this method was verified with a simple example that imitated a certain project. Next, as a case study, we applied this method to four types of realistic project schedules and verified their usefulness.     Effects of tucidinostat in adult T-cell leukemia/lymphoma in clinical practice

    Adult T-cell leukemia/lymphoma (ATL) is a peripheral T-cell malignancy with a poor prognosis. We conducted a retrospective study across six institutions in Miyazaki Prefecture, Japan, to assess the efficacy of tucidinostat in patients with relapsed/refractory ATL who had not undergone transplantation. Between October 2021 and July 2023, 24 patients aged 41 to 88 years (median, 73.4 years) who had undergone prior therapies, including intensive chemotherapy (79.2%) and mogamulizumab immunotherapy (79.2%), received tucidinostat. Objective response rate (ORR), disease control rate (DCR), progression-free survival (PFS), and overall survival (OS) were evaluated as key outcomes. ORR and DCR reached 54.2% and 91.7%, respectively. The median PFS was 3.95 months, and OS was 8.04 months, which were not inferior to the results of a phase IIb study. The influential factors for PFS were age ≥ 75 years and high soluble IL-2 receptor (sIL-2R) levels above 5000 U/mL at the start of treatment. Favorable patients without these factors achieved a PFS of 11.4 months. Treatment-related adverse events were mainly hematologic but were managed over the course of treatment.     TYK2 is essential for the therapeutic effect of IFN-α in Jak2V617F-induced murine myeloproliferative neoplasms

    Interferon-α (IFN-α) exhibits antiviral and antiproliferative effects on normal and neoplastic cells. Intracellular signaling of IFN-α is mediated by tyrosine kinase 2 (TYK2) and janus kinase 1 (JAK1), followed by signal transducers and activators of transcription (STATs). TYK2 is redundant for the antiviral effect of IFN-α; however, the requirements for antiproliferative effects are unknown. We assessed the role of TYK2 in the effects of IFN-α in myeloproliferative neoplasm (MPN) model mice. Jak2V617F transgenic mice develop MPNs resembling human primary myelofibrosis, and ropeginterferon-α-2b ameliorated their features. However, these IFN-α effects were absent in Jak2V617F;Tyk2−/− mice. In mixed wild-type (WT)/Jak2V617F chimeric mice, IFN-α treatment induces Jak2V617F hematopoietic stem cells (HSCs) to enter the cell cycle and skew their differentiation into the megakaryocyte lineage, decreasing the number of Jak2V617F HSCs. The effects of IFN-α on Jak2V617F HSCs were not observed in mixed WT/Jak2V617F;Tyk2−/− mice, indicating that TYK2 is essential for the effects of IFN-α on both Jak2V617F progenitors and HSCs. The mechanism of IFN-α in Jak2V617F HSCs and progenitors differed: genes regulating the cell cycle were enriched in IFN-α–stimulated Jak2V617F HSCs, but not in Jak2V617F progenitors; genes regulating antiproliferation were enriched in IFN-α–stimulated Jak2V617F progenitors but not in Jak2V617F HSCs. The major IFN-α signaling molecule activated by JAKs is STAT1, which is essential for the antiviral effect. Most effects of IFN-α on Jak2V617F cells were preserved in Jak2V617F;Stat1−/− mice but to a moderate degree compared with Jak2V617F mice.     <博士学位論文要旨>重度知的障害を伴う発達障害者の「問題行動」改善を目的とした動物介在介入の試み―行動分析を視点として―

    重度知的障害者には、自閉症の特徴的な行動が認められる場合がある。その行動特性が理解されないことで、暴力行動を含めた「問題行動」が生じる場合がある。その「問題行動」への対処を間違えれば、深刻な状態を引き起こすことがある。その状況を解決するために、対象者にイヌを介入する研究を行った。動物介在介入では、障害者がイヌに対して自発行動が出現するような関わりを作っていった。イヌとの関係が形成されると、イヌと一緒であれば場所を移動することで様々な困難場面に直面していくことが可能になった。その結果、イヌが存在することで様々な刺激に対応できるようになり「問題行動」が減少していった。このような介入が対象者の行動の変化をもたらし、さらに、職員が新たに出現した適切な行動を評価することで、障害者のQOL (生活の質) が向上していった。事例で示した行動の変化が生じる理由について、イヌを用いた個別の研究で明らかにした。さらに、実際に動物介在介入で用いた学習理論による説明を行った。動物介在介入では、レスポンデント条件付けを重視することで、「問題行動」を変容することが可能となっていると考えられた。本研究では、重度知的障害を伴う発達障害者の「問題行動」へ、これまで報告されていない介入方法を提案することができた。departmental bulletin pape

    重回帰モデルを用いた負荷予測 (第1報)* -一般負荷の変化と予測に関する基礎的考察-

    application/pdfSince it is difficult to store and keep electric energy, the supplier of electric power must try to generate just the right amount required by the consumers. A power station cannot change its output easily, because very big equipment must be controlled, for example a boiler, a turbine, a generator etc. Consequently, forecasting electric load becomes important. According to our survey, the forecasting error of existing or developed methods is almost 3% of the load. This error is sometimes bigger than a copacity of a power unit in many power systems. We wanted to improve the precision by developing a new load-forecasting method. This report describes our fundamental investigations concerning our new method. The shapes of the load curve not only for Saturday and Sunday but also for Monday were different from the shapes of other weekdays. The load for Tuesday to Friday deviated about 100 MW from the average taken over other such weekdays with the same temperature and at the same time of day. Considering days with a temperature change from the previous day of a given value, the deviation of the load increment was about 50 MW from the average of such days. This means that the forecast error should be 5 or 10% if the electric load is forecasted only by temperature or temperature increase. We forecast the load using the multiple regression method. We attempted to forecast the load with various combinations of data. These data include past and present temperature and past load values. 0ne example of a combination of data used was load levels of 48 and 24 hours ago and the previous day’s temperature and the forecasted temperature for the following day. The load forecast errors of the result were about 1%. Because we forecasted the load only for the previously described weekdays (Tuesday to Friday) in only June and July of 1986, this error cannot be claimed to be small but we plan to apply artificial intelligence techniques to improve this proposed method.departmental bulletin pape
