2) 神経難病について (〈シンポジウム〉介護保険法と特定疾患)

介護保険が平成12年4月に始まって,第2号被保険者の介護保険給付対象特定疾病に筋萎縮性側索硬化症ALS,パーキンソン病PD,脊髄小脳変性症SCD,シャイ・ドレーガー病SD,後縦鞍帯骨化症OPLL,も含まれる.新潟市の平成13年3月の特定疾患登録者は2382名で,そのうち295名17%が介護保険の申請をし,その大部分を神経難病患者が占める.介護保険の利用によってPD,SCDは若年者でも通所サービス,訪問サービスの増量が可能になり患者家族ともよく制度に適応し,患者のQOLも上がっている.ALSも病初期には種々の訪問サービスが利用できるが,嚥下困難,呼吸困難が始まると長時間滞在型の看護,介護が必要で介護保険で対応困難になる.更に人工呼吸管理になると長期入院も介護療養型医療施設の受け入れも難しく,PD,SCDの医療依存度の高い進行期の患者と共に行く処がない.介護老人保健施設は医療費が介護報酬額の丸めで,その高額な薬剤費を賄えずPDやSCD患者の受け入れが難しい.介護保険の対象にならなかった神経難病患者は障害者や難病対策事業でケアが受けられるが,それらの患者のニーズを拾う窓口が明確でない.今後医療依存度の高い終末期の患者を受け入れる場所の確保が急務であり,病院での長期入院の受け入れと病棟での介護力の強化,合わせて介護老人保健施設と介護療養型医療施設の介護報酬への重症加算の算定が必要である.departmental bulletin pape
