Early Complications After Radical Operations in Breast Cancer Patients

Breast cancer is one of the most common Malignancies in women. In many cases, a major component of complex treatment for breast cancer is surgery - radical mastectomy or radical breast resection.The aim of the work – to investigate the frequency and structure of complications after radical surgery with dissection of axillary lymph nodes in breast cancer patients.Material and methods. The baseline and surgical results of 147 women with breast cancer who underwent radical mastectomy or radical breast resection with lymph node dissection were analysed.Results. Complications in the early period after surgery for breast cancer were found in 76 (51.7 %), including postoperative extremity edema in 60 (40.8 %); lymphorrhea – in 37 (25.2 %), seroma – in 33 (22.4 %); wound infection in 18 (12.2 %), necrosis of the wound edges – in 15 (10.2 %) patients. Correlation of postoperative edema with almost all other complications was found, lymphorrhea and seroma were most associated with swelling and with each other; necrosis of edges with postoperative edema. Wound infection was significantly associated with lymphorrhea. Patients' age, stage of disease, and immunohistochemical type of tumour did not affect the development of complications. With increasing BMI, the incidence of complications increased significantly (χ2=9.530; p=0.009). The tendency to decrease the frequency of complications during reconstructive surgery was revealed (42.6 % versus 58.1 %, p=0.064), and adjuvant radiotherapy, on the contrary, contributed to the increase of complications (57.8 % versus 43.8 %, p=0.090).Conclusion. Radical operations with lymph node dissection in patients with breast cancer are characterized by a high frequency of early postoperative complications, mainly associated with disorders of lymphatic outflow, which indicates the need for a set of measures of preoperative preparation, improvement of surgical technique

The article examines the changes in the radio text in terms of convergence of radio and the Internet on the basis of the analysis of the features of the transcripts of the public-political radio programmes on the website of radio station «Echo Moskvy». The author highlights the changes in the radio text published on the Internet taking into account the visual presentation of text material and specifics of the Internet. The signs of classic radio text stored in the transcript (contact and lack of preparation of speeches, amount of text) are also described, as well as the features of Internet radio, which are brought to it by the visual form of presenting the text material and posting on the Internet (the appearance of the title, lead, features of layout, creolized text, intertextuality, interactivity).Показаны изменения, происходящие с радиотекстом в условиях конвергенции радио и интернета на основе анализа особенностей расшифровок общественно-политических радиопрограмм на сайте радиостанции «Эхо Москвы», выделены изменения, происходящие с радиотекстом при публикации в интернете с учетом визуальной формы подачи текстового материала и специфики глобальной Сети. Electron detachment from negative ions in bichromatic laser field

Negative ion detachment in two-colour laser field is considered within the recent modification of Keldysh model which makes it quantitatively reliable. The general approach is illustrated by calculation of angular differential detachment rates, partial rates for particular ATD (Above Threshold Detachment) channels and total detachment rates for H$^-$ ion in bichromatic field with 1:2 frequency ratio. Both perturbative and strong field regimes are examined. Polar asymmetry and phase effects are quantitatively characterized with some new features revealed. Balance and steadiness correction of the upright posture of patients having withstood an ischemic stroke with the help of stabilographic rehabilitation training equipment with biofeedback

The brain ischemic mortality rate in Russia occupies the third position. As a result, a recovery period after an ischemic stroke could undermine social and economic well-being of patients and their close relatives. One of the major consequences of a stroke includes the firm-motor defects. Their degree can be reduced with the help of rehabilitation measures intended to revive the motor function of paralyzed limbs and to train a patient to remain firm upright. A stabilographic rehabilitation training apparatus with biofeedback represents one of the variants of the posture training. This training in a playful way helps a patient to improve the balance and firmness indices of the upright position. This rehabilitation method improved considerably the patients’ clinical and stabilographic indices of the balance and firmness function in comparison with the patients whose programmes did not include this method. A patient could sense better that he/she was standing on the both lower limbs. The sensitivity in the lower limbs was intensifying or reviving. According to the additional stabilographic control tests, the total scatter of the pressure centre and the scatter in the sagittal plane, the rate of the pressure centre movement were decreasing; Romberg coefficient became normal

Singularities in Speckled Speckle

Speckle patterns produced by random optical fields with two (or more) widely different correlation lengths exhibit speckle spots that are themselves highly speckled. Using computer simulations and analytic theory we present results for the point singularities of speckled speckle fields: optical vortices in scalar (one polarization component) fields; C points in vector (two polarization component) fields. In single correlation length fields both types of singularities tend to be more{}-or{}-less uniformly distributed. In contrast, the singularity structure of speckled speckle is anomalous: for some sets of source parameters vortices and C points tend to form widely separated giant clusters, for other parameter sets these singularities tend to form chains that surround large empty regions. The critical point statistics of speckled speckle is also anomalous. In scalar (vector) single correlation length fields phase (azimuthal) extrema are always outnumbered by vortices (C points). In contrast, in speckled speckle fields, phase extrema can outnumber vortices, and azimuthal extrema can outnumber C points, by factors that can easily exceed $10^{4}$ for experimentally realistic source parameters

Specifics of computer discourse translation from English into Russian

Relevance of the problem under study is caused by need of further development of science and technology in Russia and abroad and it inevitably puts on the agenda the issue of information exchange in the field of scientific and technological achievements. This article is directed on identification and analysis of main features of translation of lexical and grammatical phenomena of computer discourse. The leading approaches to research of this problem are logical and linguistic analysis, method of actualization, selection, systematization and generalization with analysis of the specifics of lexical-grammatical phenomena of computer discourse point of translation studies. The main results of the study are that the concept of computer discourse and the characteristics of technical translation were analyzed; specificity of computer terminology translation was revealed; lexical and grammatical phenomena of computer discourse in terms of the theory of translation were investigated.
