Epirubicin With Cyclophosphamide Followed by Docetaxel With Trastuzumab and Bevacizumab as Neoadjuvant Therapy for HER2-Positive Locally Advanced Breast Cancer or as Adjuvant Therapy for HER2-Positive Pathologic Stage III Breast Cancer: A Phase II Trial of the NSABP Foundation Research Group, FB-5
Background The purpose of this study was to determine the cardiac safety and clinical activity of trastuzumab and bevacizumab with docetaxel after epirubicin with cyclophosphamide (EC) in patients with HER2-positive locally advanced breast cancer (LABC) or pathologic stage 3 breast cancer (PS3BC). Patients and Methods Patients received every 3 week treatment with 4 cycles of EC (90/600 mg/m2) followed by 4 cycles of docetaxel (100 mg/m2). Targeted therapy with standard-dose trastuzumab with bevacizumab 15 mg/kg was given for a total of 1 year. Coprimary end points were (1) rate of cardiac events (CEs) in all patients defined as clinical congestive heart failure with a significant decrease in left ventricular ejection fraction or cardiac deaths; and (2) pathologic complete response (pCR) in breast and nodes in the neoadjuvant cohort. An independent cardiac review panel determined whether criteria for a CE were met. Results A total of 105 patients were accrued, 76 with LABC treated with neoadjuvant therapy and 29 with PS3BC treated with adjuvant therapy. Median follow-up was 59.2 months. Among 99 evaluable patients for cardiac safety, 4 (4%; 95% confidence interval [CI], 1.1%-10.0%) met CE criteria. The pCR percentage in LABC patients was 46% (95% CI, 34%-59%). Five-year recurrence-free survival (RFS) and overall survival (OS) for all patients was 79.9% and 90.8%, respectively. Conclusion The regimen met predefined criteria for activity of interest with an acceptable rate of CEs. Although the pCR percentage was comparable with chemotherapy regimens with trastuzumab alone the high RFS and OS are of interest in these high-risk populations
Beyond deficit-based models of learners' cognition: Interpreting engineering students' difficulties with sense-making in terms of fine-grained epistemological and conceptual dynamics
Researchers have argued against deficit-based explanations of students'
troubles with mathematical sense-making, pointing instead to factors such as
epistemology: students' beliefs about knowledge and learning can hinder them
from activating and integrating productive knowledge they have. In this case
study of an engineering major solving problems (about content from his
introductory physics course) during a clinical interview, we show that "Jim"
has all the mathematical and conceptual knowledge he would need to solve a
hydrostatic pressure problem that we posed to him. But he reaches and sticks
with an incorrect answer that violates common sense. We argue that his lack of
mathematical sense-making-specifically, translating and reconciling between
mathematical and everyday/common-sense reasoning-stems in part from his
epistemological views, i.e., his views about the nature of knowledge and
learning. He regards mathematical equations as much more trustworthy than
everyday reasoning, and he does not view mathematical equations as expressing
meaning that tractably connects to common sense. For these reasons, he does not
view reconciling between common sense and mathematical formalism as either
necessary or plausible to accomplish. We, however, avoid a potential "deficit
trap"-substituting an epistemological deficit for a concepts/skills deficit-by
incorporating multiple, context-dependent epistemological stances into Jim's
cognitive dynamics. We argue that Jim's epistemological stance contains
productive seeds that instructors could build upon to support Jim's
mathematical sense-making: He does see common-sense as connected to formalism
(though not always tractably so) and in some circumstances this connection is

Love at What Price? Estimating the Value of Marriage
Using a law within Social Security that provides clear financial incentives to delay marriage, we estimate the financial value of a month of marriage. Specifically, the law provides that widows who are eligible for Social Security benefits on their deceased spouse\u27s earnings records are eligible for benefits at age 60, unless they remarry before that age. If they remarry before that age, they cannot claim widow benefits and must wait until at least age 62 to claim spousal benefits on their new husband\u27s record, which are typically less generous than widow benefits. To generate an estimate of what this behavior implies about the value of marriage, we use data from five panels of the Survey of Income and Program Participation linked to administrative data from Social Security. We estimate the cost of marrying before age 60 imposed by the Social Security program. We develop a model that reflects the institutional details of Social Security and generate a likelihood function that reflects that model. By taking advantage of the variation in these costs and when or whether widows remarry before age 60, we estimate the benefit of marriage to be $8000/month. These estimates appear to be reasonable in the context of the short length of time widows are willing to wait and the high value of Social Security benefits
Black Stork Down: Military Discourses in Bird Conservation in Malta
Tensions between Maltese hunters and bird conservation NGOs have intensified over the past decade. Conservation NGOs have become frustrated with the Maltese State for conceding to the hunter lobby and negotiating derogations from the European Union’s Bird Directive. Some NGOs have recently started to organize complex field-operations where volunteers are trained to patrol the landscape, operate drones and other surveillance technologies, detect illegalities, and lead police teams to arrest poachers. We describe the sophisticated military metaphors which conservation NGOs have developed to describe, guide and legitimize their efforts to the Maltese public and their fee-paying members. We also discuss why such groups might be inclined to adopt these metaphors. Finally, we suggest that anthropological studies of discourse could help understand delicate contexts such as this where conservation NGOs, hunting associations and the State have ended in political deadlock
Polymeric synthetic nanoparticles for the induction of antigen-specific immunological tolerance
Current treatments to control pathological or unwanted immune responses often use broadly immunosuppressive drugs. New approaches to induce antigen-specific immunological tolerance that control both cellular and humoral immune responses are desirable. Here we describe the use of synthetic, biodegradable nanoparticles carrying either protein or peptide antigens and a tolerogenic immunomodulator, rapamycin, to induce durable and antigen-specific immune tolerance, even in the presence of potent Toll-like receptor agonists. Treatment with tolerogenic nanoparticles results in the inhibition of CD4+ and CD8+ T-cell activation, an increase in regulatory cells, durable B-cell tolerance resistant to multiple immunogenic challenges, and the inhibition of antigen-specific hypersensitivity reactions, relapsing experimental autoimmune encephalomyelitis, and antibody responses against coagulation factor VIII in hemophilia A mice, even in animals previously sensitized to antigen. Only encapsulated rapamycin, not the free form, could induce immunological tolerance. Tolerogenic nanoparticle therapy represents a potential novel approach for the treatment of allergies, autoimmune diseases, and prevention of antidrug antibodies against biologic therapies.Juvenile Diabetes Research Foundation Internationa
Greedy permanent magnet optimization
A number of scientific fields rely on placing permanent magnets in order to
produce a desired magnetic field. We have shown in recent work that the
placement process can be formulated as sparse regression. However, binary,
grid-aligned solutions are desired for realistic engineering designs. We now
show that the binary permanent magnet problem can be formulated as a quadratic
program with quadratic equality constraints (QPQC), the binary, grid-aligned
problem is equivalent to the quadratic knapsack problem with multiple knapsack
constraints (MdQKP), and the single-orientation-only problem is equivalent to
the unconstrained quadratic binary problem (BQP). We then provide a set of
simple greedy algorithms for solving variants of permanent magnet optimization,
and demonstrate their capabilities by designing magnets for stellarator
plasmas. The algorithms can a-priori produce sparse, grid-aligned, binary
solutions. Despite its simple design and greedy nature, we provide an algorithm
that outperforms the state-of-the-art algorithms while being substantially
faster, more flexible, and easier-to-use

A kinetic isotope effect (kD/kH) of 7.4 has been found for the reaction of chlorosilylene with trimethysilane (Me3SiD vs Me3SiH). Such a value can be accounted for by theoretical modelling, but only if an internal rearrangement of the initially form complex is included in the mechanism. This provides the first concrete evidence for such complexes
