Systematic review of the use of financial incentives in treatments for obesity and overweight
Nine studies met the criteria for inclusion in this systematic review of randomized controlled trials of treatments for obesity and overweight involving the use of financial incentives, with reported follow-up of at least 1 year. All included trials were of behavioural obesity treatments. Justification of sample size and blinding procedure were not mentioned in any study. Attrition was well described in three studies and no study was analysed on an intention to treat basis. Participants were mostly women recruited through media advertisements. Mean age ranged from 35.7 to 52.8 years, and mean body mass index from 29.3 to 31.8 kg m−2. Results from meta-analysis showed no significant effect of use of financial incentives on weight loss or maintenance at 12 months and 18 months. Further sub-analysis by mode of delivery and amount of incentives although also non-statistically significant were suggestive of very weak trends in favour of use of amounts greater than 1.2% personal disposable income, rewards for behaviour change rather than for weight, rewards based on group performance rather than for individual performance and rewards delivered by non-psychologists rather than delivered by psychologists.The Health Services Research Unit is funded by the Chief Scientist Office of the Scottish Executive Health Department. Nasal administration of glutamate decarboxylase (GAD65) peptides induces Th2 responses and prevents murine insulin-dependent diabetes.
We previously demonstrated that a spontaneous Th1 response against glutamate decarboxylase (GAD65) arises in NOD mice at four weeks in age and subsequently T cell autoimmunity spreads both intramolecularly and intermolecularly. Induction of passive tolerance to GAD65, through inactivation of reactive T cells before the onset of autoimmunity, prevented determinant spreading and the development of insulin-dependent diabetes mellitus (IDDM). Here, we examined whether an alternative strategy, designed to induce active tolerance via the engagement of Th2 immune responses to GAD65, before the spontaneous onset of autoimmunity, could inhibit the cascade of Th1 responses that lead to IDDM. We observed that a single intranasal administration of GAD65 peptides to 2-3-wk-old NOD mice induced high levels of IgG1 antibodies to GAD65. GAD65 peptide treated mice displayed greatly reduced IFN gamma responses and increased IL-5 responses to GAD65, confirming the diversion of the spontaneous GAD65 Th1 response toward a Th2 phenotype. Consistent with the induction of an active tolerance mechanism, splenic CD4+ (but not CD8+) T cells from GAD65 peptide-treated mice, inhibited the adoptive transfer of IDDM to NOD-scid/scid mice. This active mechanism not only inhibited the development of proliferative T cell responses to GAD65, it also limited the expansion of autoreactive T cell responses to other beta cell antigens (i.e., determinant spreading). Finally, GAD65 peptide treatment reduced insulitis and long-term IDDM incidence. Collectively, these data suggest that the nasal administration of GAD65 peptides induces a Th2 cell response that inhibits the spontaneous development of autoreactive Th1 responses and the progression of beta cell autoimmunity in NOD mice
Hip disarticulation - case series analysis and literature review
OBJECTIVE:
To present a retrospective study of 16 patients submitted to hip disarticulation.
METHODS:
During the period of 16 years, 16 patients who underwent hip disarticulation were identified. All of them were studied based on clinical records regarding the gender, age at surgery, disarticulation cause, postoperative complications, mortality rates and functional status after hip disarticulation.
RESULTS:
Hip disarticulation was performed electively in most cases and urgently in only three cases. The indications had the following origins: infection (n = 6), tumor (n = 6), trauma (n = 3), and ischemia (n = 2). The mean post-surgery survival was 200.5 days. The survival rates were 6875% after six months, 5625% after one year, and 50% after three years. The mortality rates were higher in disarticulations with traumatic (66.7%) and tumoral (60%) causes. Regarding the eight patients who survived, half of them ambulate with crutches and without prosthesis, 25% walk with limb prosthesis, and 25% are bedridden. Complications and mortality were higher in the cases of urgent surgery, and in those with traumatic and tumoral causes.
CONCLUSION:
Hip disarticulation is a major ablative surgery with obvious implications for limb functionality, as well as high rates of complications and mortality. However, when performed at the correct time and with proper indication, this procedure can be life-saving and can ensure the return to the home environment with a certain degree of quality of life.info:eu-repo/semantics/publishedVersio
SIRT1 Activity Is Linked to Its Brain Region-Specific Phosphorylation and Is Impaired in Huntington’s Disease Mice
Huntingtons disease (HD) is a neurodegenerative disorder for which there are no disease-modifying treatments. SIRT1 is a NAD+-dependent protein deacetylase that is implicated in maintaining neuronal health during development, differentiation and ageing. Previous studies suggested that the modulation of SIRT1 activity is neuroprotective in HD mouse models, however, the mechanisms controlling SIRT1 activity are unknown. We have identified a striatum-specific phosphorylation-dependent regulatory mechanism of SIRT1 induction under normal physiological conditions, which is impaired in HD. We demonstrate that SIRT1 activity is down-regulated in the brains of two complementary HD mouse models, which correlated with altered SIRT1 phosphorylation levels. This SIRT1 impairment could not be rescued by the ablation of DBC1, a negative regulator of SIRT1, but was linked to changes in the sub-cellular distribution of AMPK-α1, a positive regulator of SIRT1 function. This work provides insights into the regulation of SIRT1 activity with the potential for the development of novel therapeutic strategies
The Dialectics of Parenting: Changes in the Interplay of Maternal Behaviors during Early and Middle Childhood
Parent and child relationships continuously evolve, part of an ongoing dialectic that derives from developmental changes in both parent and child. The focus of this study is on changes in the strength of association among four types of parenting behaviors considered important for children’s development: supportive presence, respect for autonomy, stimulation, and hostility. Mother–child interaction was observed for 1229 parent–child dyads at 36 months, 54 months, 1st grade, 3rd grade, and 5th grade using similar observational paradigms. The association between respect for autonomy and supportive presence was strong at age three and continued to be strong over time. The association between respect for autonomy and stimulation was modest but also showed little change from age three to 5th grade. Respect for autonomy was negatively associated with maternal hostility, but the relation was complex. It was stronger at 54 months than 36 months but then became weaker through time. Supportive presence showed a moderate relation with stimulation at age 3 but the association became weaker over time. Supportive presence showed an expected negative association with hostility, a relation that changed little over time. The relation between hostility and stimulation also became weaker over time. In effect, there appears to be a shifting pattern of relations between maternal behaviors during early and middle childhood, one that reflects an evolving dialectic in the mother–child relationship

A survey confirmed the presence of the amphibian chytrid fungus, Batrachochytrium dendrobatidis in a number of frog habitats dose to major cities and towns across Tasmania.The detection of chytrid infection in some remote wetlands at high altitude locations on the Tasmanian Central Plateau is of particular concern. The likely presence of chytrid infections was assessed by examining tadpoles for signs of depigmentation, thinning and asymmetry in their keratinized jaw sheaths using a hand lens. Learning Visual Question Answering by Bootstrapping Hard Attention
Attention mechanisms in biological perception are thought to select subsets
of perceptual information for more sophisticated processing which would be
prohibitive to perform on all sensory inputs. In computer vision, however,
there has been relatively little exploration of hard attention, where some
information is selectively ignored, in spite of the success of soft attention,
where information is re-weighted and aggregated, but never filtered out. Here,
we introduce a new approach for hard attention and find it achieves very
competitive performance on a recently-released visual question answering
datasets, equalling and in some cases surpassing similar soft attention
architectures while entirely ignoring some features. Even though the hard
attention mechanism is thought to be non-differentiable, we found that the
feature magnitudes correlate with semantic relevance, and provide a useful
signal for our mechanism's attentional selection criterion. Because hard
attention selects important features of the input information, it can also be
more efficient than analogous soft attention mechanisms. This is especially
important for recent approaches that use non-local pairwise operations, whereby
computational and memory costs are quadratic in the size of the set of
