Reflection on Risk Factors, Ashtma and Tobacco Smoke Exposure
O impacto da asma brônquica nas últimas décadas, nomeadamente em idade pediátrica, associando prevalências significativas a uma tendência, gravidade e custos crescentes, tem levado a que se efectuem múltiplos estudos para esclarecer causas, avaliando riscos, permitindo a elaboração de programas de prevenção. Estudos epidemiológicos
bem desenhados, aplicados a amostras populacionais significativas, permitem identificar determinantes independentes
da asma, viabilizando a actuação. Se muito se tem avançado no conhecimento das bases fisiopatológicas da doença alérgica, é com alguma preocupação que sentimos que a comunidade médica, mesmo a especializada, não sente as evidências epidemiológicas como aplicáveis à sua população de asmáticos. “Precisamos de mais estudos”, é declaração comum. Com este trabalho, centrado em três rastreios efectuados com a coordenação dos autores, pretendemos
demonstrar que existem factores preveníveis, moduláveis, que podem permitir reduzir a morbilidade da doença asmática.
Entre estes, o tabagismo passivo assume uma relevância ímpar, por ser o principal factor de risco para a gravidade da asma pediátrica em Portugal
Singularities of equidistants and global centre symmetry sets of Lagrangian submanifolds
We define the Global Centre Symmetry set (GCS) of a smooth closed
m-dimensional submanifold M of R^n, , which is an affinely invariant
generalization of the centre of a k-sphere in R^{k+1}. The GCS includes both
the centre symmetry set defined by Janeczko and the Wigner caustic defined by
Berry. We develop a new method for studying generic singularities of the GCS
which is suited to the case when M is lagrangian in R^{2m} with canonical
symplectic form. The definition of the GCS, which slightly generalizes one by
Giblin and Zakalyukin, is based on the notion of affine equidistants, so, we
first study singularities of affine equidistants of Lagrangian submanifolds,
classifying all the stable ones. Then, we classify the affine-Lagrangian stable
singularities of the GCS of Lagrangian submanifolds and show that, already for
smooth closed convex curves in R^2, many singularities of the GCS which are
affine stable are not affine-Lagrangian stable.Comment: 26 pages, 2 figure
Allergic conjunctivitis and conjunctival provocation tests in atopic dogs.
INTRODUCTION:
Canine atopic dermatitis (cAD) is a very common disease, but little is known about eye involvement. The conjunctival provocation test (CPT) is used in human to study the ocular response to allergenic stimuli and to evaluate anti-allergic therapy. To our knowledge it has not been used in dogs.
OBJECTIVES:
To evaluate the prevalence of ocular signs in a population of atopic dogs and relate these with clinical cAD scores; and the usefulness of CPT for dust mites in atopic dogs with itchy eyes.
PROCEDURES:
Sixty cAD patients were evaluated for (i) ocular signs of allergic conjunctivitis including conjunctival hyperemia, chemosis, epiphora, ocular discharge, pruritus and corneal involvement, graded 0 to 3 according to severity, and (2) cAD Extent and Severity Index (CADESI-03). Additionally, CPTs for Dermatophagoides farinae (n = 12) and Dermatophagoides pteronyssinus (n = 12) were performed in sensitized atopic dogs and 24 control dogs.
RESULTS:
Periocular and ocular signs of allergy were present in 60% (36/60) of cases. Conjunctival hyperemia (90%) was the most common sign. Severity of ocular signs was significantly correlated with eye pruritus (r(s) = 0.690, P = <0.001) and skin lesions score for head region (r(s) = 0.261, P = 0.04). A highly significant difference (P < 0.001, Fisher test) was found in CPTs between the test and the control groups.
CONCLUSION:
Allergic conjunctivitis signs associated with cAD seem under valuated so these patients would benefit from an ophthalmologic evaluation. Furthermore, we found CPT to be a reliable, easy to perform and safe test for the etiologic diagnosis of allergic conjunctivitis in the dog

A novel multivariate STeady-state index during general ANesthesia (STAN)
The assessment of the adequacy of general anesthesia for surgery, namely the nociception/anti-nociception balance, has received wide attention from the scientific community. Monitoring systems based on the frontal EEG/EMG, or autonomic state reactions (e.g. heart rate and blood pressure) have been developed aiming to objectively assess this balance. In this study a new multivariate indicator of patients' steady-state during anesthesia (STAN) is proposed, based on wavelet analysis of signals linked to noxious activation. A clinical protocol was designed to analyze precise noxious stimuli (laryngoscopy/intubation, tetanic, and incision), under three different analgesic doses; patients were randomized to receive either remifentanil 2.0, 3.0 or 4.0 ng/ml. ECG, PPG, BP, BIS, EMG and [Formula: see text] were continuously recorded. ECG, PPG and BP were processed to extract beat-to-beat information, and [Formula: see text] curve used to estimate the respiration rate. A combined steady-state index based on wavelet analysis of these variables, was applied and compared between the three study groups and stimuli (Wilcoxon signed ranks, Kruskal-Wallis and Mann-Whitney tests). Following institutional approval and signing the informed consent thirty four patients were enrolled in this study (3 excluded due to signal loss during data collection). The BIS index of the EEG, frontal EMG, heart rate, BP, and PPG wave amplitude changed in response to different noxious stimuli. Laryngoscopy/intubation was the stimulus with the more pronounced response [Formula: see text]. These variables were used in the construction of the combined index STAN; STAN responded adequately to noxious stimuli, with a more pronounced response to laryngoscopy/intubation (18.5-43.1 %, [Formula: see text]), and the attenuation provided by the analgesic, detecting steady-state periods in the different physiological signals analyzed (approximately 50 % of the total study time). A new multivariate approach for the assessment of the patient steady-state during general anesthesia was developed. The proposed wavelet based multivariate index responds adequately to different noxious stimuli, and attenuation provided by the analgesic in a dose-dependent manner for each stimulus analyzed in this study.The first author was supported by a scholarship from the Portuguese Foundation for Science and Technology (FCT SFRH/BD/35879/2007). Development Process and Cognitive Testing of CARATkids - Control of Allergic Rhinitis and Asthma Test for Children
Background: Allergic rhinitis and asthma (ARA) are chronic inflammatory diseases of the airways that often coexist in children. The only tool to assess the ARA control, the Control of Allergic Rhinitis and Asthma Test (CARAT) is to be used by adults. We aimed to develop the Pediatric version of Control of Allergic Rhinitis and Asthma Test
(CARATkids) and to test its comprehensibility in children with 4 to 12 years of age.
Methods: The questionnaire development included a literature review of pediatric questionnaires on asthma and/or rhinitis control and two consensus meetings of a multidisciplinary group. Cognitive testing was carried out in a cross-sectional qualitative study using cognitive interviews.
Results: Four questionnaires to assess asthma and none to assess rhinitis control in children were identified. The
multidisciplinary group produced a questionnaire version for children with 17 questions with illustrations and
dichotomous (yes/no) response format. The version for caregivers had 4-points and dichotomous scales. Twenty-nine
children, 4 to 12 years old, and their caregivers were interviewed. Only children over 6 years old could adequately
answer the questionnaire. A few words/expressions were not fully understood by children of 6 to 8 years old. The
drawings illustrating the questions were considered helpful by children and caregivers. Caregivers considered the
questionnaire complete and clear and preferred dichotomous over the 4-points scales. The proportion of agreement between children and their caregivers was 61%. The words/expressions that were difficult to understand were amended.
Conclusion: CARATkids, the first questionnaire to assess a child’s asthma and rhinitis control was developed and its
content validity was assured. Cognitive testing showed that CARATKids is well-understood by children 6 to 12 years
old. The questionnaire’s measurement properties can now be assessed in a validation study
How equal is equality? Discussions about same-sex marriage in Portugal
In Portugal, public and political discussions about same-sex marriage have been going on since the 1990s. In 2010, same-sex marriage was legalized under intense dispute since it excludes same-sex couples from adoption and reproductive rights. During parliamentary debates, political parties and civil organizations linked to the Catholic Church resorted to conflicting ideas of ‘equality’ and ‘difference’ to advance their claims. In this article, we analyse the contents of petitions, bills and parliamentary proceedings concerning the legal recognition of same-sex unions, highlighting the presence of conflicting notions of equality linked to pervasive beliefs about the inadequacy of homo-erotic desire and practices
Cohabitation in Brazil : historical legacy and recent evolution
The availability of the micro data in the IPUMS samples for several censuses spanning a period of 40 years permits a detailed study of differentials and trends in cohabitation in Brazil than has hitherto been the case. The gist of the story is that the historical race/class and religious differentials and the historical spatial contrasts have largely been maintained, but are now operating at much higher levels than in the 1970s. During the last 40 years cohabitation has dramatically increased in all strata of the Brazilian population, and it has spread geographically to all areas in tandem with further expansions in the regions that had historically higher levels to start with. Moreover, the probability of cohabiting depends not only on individual-level characteristics but also on additional contextual effects operating at the level of meso-regions. The rise of cohabitation in Brazil fits the model of the "Second demographic transition", but it is grafted onto a historical pattern which is still manifesting itself in a number of ways
