    Infraestrutura e adesão à higienização das mãos: desafios à segurança do paciente

    Considerando a importância das mãos na cadeia de transmissão de microrganismos, esta pesquisa observacional investigou a infraestrutura material e a adesão à higienização das mãos em unidade de terapia intensiva do sul do Brasil, em 2010. Os dados foram coletados por observação direta não participante e emprego de instrumento autoaplicável a 39 profissionais, analisados com auxílio de Teste do 2, estatística descritiva e análise de discurso quantitativa. Embora os profissionais superestimem a adesão, reconheçam a prática como relevante para a prevenção de infecções e refiram não haver fatores de impedimento, entre 1277 oportunidades observadas, a adesão foi de 28,6%, e significativamente menor antes do contato e dos procedimentos assépticos do que após o contato com o paciente. A infraestrutura apresentou-se deficiente em funcionalidade. Os resultados implicam risco para a segurança dos pacientes, sendo relevante o planejamento de ações corretivas e que promovam essa prática

    Primaquine-thiazolidinones block malaria transmission and development of the liver exoerythrocytic forms

    BACKGROUND: Primaquine is an anti-malarial used to prevent Plasmodium vivax relapses and malaria transmission. However, PQ metabolites cause haemolysis in patients deficient in the enzyme glucose-6-phosphate dehydrogenase (G6PD). Fifteen PQ-thiazolidinone derivatives, synthesized through one-post reactions from primaquine, arenealdehydes and mercaptoacetic acid, were evaluated in parallel in several biological assays, including ability to block malaria transmission to mosquitoes. RESULTS: All primaquine derivatives (PQ-TZs) exhibited lower cell toxicity than primaquine; none caused haemolysis to normal or G6PD-deficient human erythrocytes in vitro. Sera from mice pretreated with the test compounds thus assumed to have drug metabolites, caused no in vitro haemolysis of human erythrocytes, whereas sera from mice pretreated with primaquine did cause haemolysis. The ability of the PQ-TZs to block malaria transmission was evaluated based on the oocyst production and percentage of mosquitoes infected after a blood meal in drug pre-treated animals with experimental malaria caused by either Plasmodium gallinaceum or Plasmodium berghei; four and five PQ-TZs significantly inhibited sporogony in avian and in rodent malaria, respectively. Selected PQ-TZs were tested for their inhibitory activity on P. berghei liver stage development, in mice and in vitro, one compound (4m) caused a 3-day delay in the malaria pre-patent period. CONCLUSIONS: The compound 4m was the most promising, blocking malaria transmissions and reducing the number of exoerythrocytic forms of P. berghei (EEFs) in hepatoma cells in vitro and in mice in vivo. The same compound also caused a 3-day delay in the malaria pre-patent period.     Antimalarial Activity and Mechanisms of Action of Two Novel 4-Aminoquinolines against Chloroquine-Resistant Parasites

    Chloroquine (CQ) is a cost effective antimalarial drug with a relatively good safety profile (or therapeutic index). However, CQ is no longer used alone to treat patients with Plasmodium falciparum due to the emergence and spread of CQ-resistant strains, also reported for P. vivax. Despite CQ resistance, novel drug candidates based on the structure of CQ continue to be considered, as in the present work. One CQ analog was synthesized as monoquinoline (MAQ) and compared with a previously synthesized bisquinoline (BAQ), both tested against P. falciparum in vitro and against P. berghei in mice, then evaluated in vitro for their cytotoxicity and ability to inhibit hemozoin formation. Their interactions with residues present in the NADH binding site of P falciparum lactate dehydrogenase were evaluated using docking analysis software. Both compounds were active in the nanomolar range evaluated through the HRPII and hypoxanthine tests. MAQ and BAQ derivatives were not toxic, and both compounds significantly inhibited hemozoin formation, in a dose-dependent manner. MAQ had a higher selectivity index than BAQ and both compounds were weak PfLDH inhibitors, a result previously reported also for CQ. Taken together, the two CQ analogues represent promising molecules which seem to act in a crucial point for the parasite, inhibiting hemozoin formation

    AbstractThe statement that the habitat complexity and structure govern the abundance and diversity of biological communities has been widely investigated. In this context, we assumed the hypothesis of habitat heterogeneity, that is, the higher habitat complexity leads to greater diversity of Odonata. In addition, we analyzed the influence of habitat structure on the distribution of this community, and evaluated the effects of abiotic variables. Odonata larvae were collected with sieves and by electrofishing in ten neotropical streams belonging to the Pirapó River basin. Forty species of Odonata were registered, which were distributed in eight families, Libellulidae stood out with the highest richness. The high gamma diversity and distribution of Odonata were associated with habitat heterogeneity in these streams. However, the abiotic variables also seem to affect the distribution of Odonata species, in view of the impact of the land use in the vicinity of streams

    Regolamento Didattico del Corso di laurea in Infermieristica (CLI) elaborata da un gruppo di lavoro incaricato dalla Commissione Nazionale CLI e successivamente sottoposta al consenso nazionale (in cui sono stati coinvolti tutti i Presidenti e Coordinatori delle Attività Formative Professionalizzanti). Questa traccia si basa sulle: - componenti tipiche di un Regolamento Didattico di corso, - esperienze maturate nei precedenti Regolamenti di CLI, - indicazioni emerse in Commissione Nazionale dei CLI negli ultimi anni di preparazione alla riforma, raccomandazioni emerse nella Consensus Conference sul tirocinio (che si invita a consultare: le sue componenti non sono state qui riprodotte). Dal punto di vista metodologico, a) in alcune occasioni sono stati riportati contenuti sui quali non si è raggiunto un completo consenso: la decisione/adattamento è stata demandata alla singola sede (simbolo !); b) in altre, sono stati inclusi aspetti di dettaglio (simbo- lo &) riportando la descrizione di quanto si sta affrontando (ad esempio, Descrittori di Dublino) al fine di costruire un riferimento comune a livello nazionale sul significato di alcuni aspetti, c) per brevità sono state inoltre selezionate le questioni più critiche (il testo completo è disponibile su: http://cplps.altervista.org/ blog/).
