    Efficacious, effective, and embedded interventions: Implementation research in infectious disease control

    Background: Research in infectious disease control is heavily skewed towards high end technology; development of new drugs, vaccines and clinical interventions. Oft ignored, is the evidence to inform the best strategies that ensure the embedding of interventions into health systems and amongst populations. In this paper we undertake an analysis of the challenge in the development of research for the sustainable implementation of disease control interventions. Results: We highlight the fundamental differences between the research paradigms associated with the development of technologies and interventions for disease control on the one hand and the research paradigms required for enhancing the sustainable uptake of those very same interventions within the communities on the other. We provide a definition for implementation research in an attempt to underscore its critical role and explore the multidisciplinary science needed to address the challenges in disease control. Conclusion: The greatest value for money in health research lies in the sustainable and effective implementation of already proven, efficacious solutions. The development of implementation research that can help provide some solutions on how this can be achieved is sorely needed

    Development of a quality indicator set to measure and improve quality of ICU care in low- and middle-income countries

    PURPOSE: To develop a set of actionable quality indicators for critical care suitable for use in low- or middle-income countries (LMICs). METHODS: A list of 84 candidate indicators compiled from a previous literature review and stakeholder recommendations were categorised into three domains (foundation, process, and quality impact). An expert panel (EP) representing stakeholders from critical care and allied specialties in multiple low-, middle-, and high-income countries was convened. In rounds one and two of the Delphi exercise, the EP appraised (Likert scale 1–5) each indicator for validity, feasibility; in round three sensitivity to change, and reliability were additionally appraised. Potential barriers and facilitators to implementation of the quality indicators were also reported in this round. Median score and interquartile range (IQR) were used to determine consensus; indicators with consensus disagreement (median < 4, IQR ≤ 1) were removed, and indicators with consensus agreement (median ≥ 4, IQR ≤ 1) or no consensus were retained. In round four, indicators were prioritised based on their ability to impact cost of care to the provider and recipient, staff well-being, patient safety, and patient-centred outcomes. RESULTS: Seventy-one experts from 30 countries (n = 45, 63%, representing critical care) selected 57 indicators to assess quality of care in intensive care unit (ICU) in LMICs: 16 foundation, 27 process, and 14 quality impact indicators after round three. Round 4 resulted in 14 prioritised indicators. Fifty-seven respondents reported barriers and facilitators, of which electronic registry-embedded data collection was the biggest perceived facilitator to implementation (n = 54/57, 95%) Concerns over burden of data collection (n = 53/57, 93%) and variations in definition (n = 45/57, 79%) were perceived as the greatest barrier to implementation. CONCLUSION: This consensus exercise provides a common set of indicators to support benchmarking and quality improvement programs for critical care populations in LMICs

    B7-H1 Blockade Increases Survival of Dysfunctional CD8+ T Cells and Confers Protection against Leishmania donovani Infections

    Experimental visceral leishmaniasis (VL) represents an exquisite model to study CD8+ T cell responses in a context of chronic inflammation and antigen persistence, since it is characterized by chronic infection in the spleen and CD8+ T cells are required for the development of protective immunity. However, antigen-specific CD8+ T cell responses in VL have so far not been studied, due to the absence of any defined Leishmania-specific CD8+ T cell epitopes. In this study, transgenic Leishmania donovani parasites expressing ovalbumin were used to characterize the development, function, and fate of Leishmania-specific CD8+ T cell responses. Here we show that L. donovani parasites evade CD8+ T cell responses by limiting their expansion and inducing functional exhaustion and cell death. Dysfunctional CD8+ T cells could be partially rescued by in vivo B7-H1 blockade, which increased CD8+ T cell survival but failed to restore cytokine production. Nevertheless, B7-H1 blockade significantly reduced the splenic parasite burden. These findings could be exploited for the design of new strategies for immunotherapeutic interventions against VL

    Determination of Groutability Ratio Through Image Processing

    Grouting refers to injection of flowable stabilizers into a soil or rock formation to change its physical properties. The most important characteristic of grouting process is groutability, which is a ratio of particle diameter corresponds to 85% finer and 15% finer of base soil and grout material, respectively. Groutability is said to be very consistent if the ratio is high. However, the existing method to estimate the groutability may mislead the ratios as the conventional sieves may not represent the whole range of particle sizes available in the materials. Besides, state or density of the base soil is utmost important to estimate the groutability, especially if permeation grouting is considered. To overcome these disadvantages of the traditional methods, a new image-based method is introduced in the present study to estimate the groutability. Due to availability of promising imaging technics and latest advancements in image processing technics, an improved estimation of groutability is possible. Series of experiments were conducted on sand specimens at various depths to assess the groutability through imaging technic. Gradation analysis also has been performed to compare the data. The proposed new imaging technic has more promising information about the groutability than the traditional gradation curves. In addition to the time and energy consumed in traditional methods, the image-based method proves it highly accurate and simple than the traditional method

    Dichotomy of protective cellular immune responses to human visceral leishmaniasis

    Healing/protective responses in human visceral leishmaniasis (VL) are associated with stimulation/production of Th1 cytokines, such as interferon IFN-γ, and conversion in the leishmanin skin test (LST). Such responses were studied for 90 days in 44 adult healthy volunteers from VL non-endemic areas, with no past history of VL/cutaneous leishmaniasis (CL) and LST non-reactivity following injection with one of four doses of Alum-precipitated autoclaved Leishmania major (Alum/ALM) ± bacille Calmette–Guérin (BCG), a VL candidate vaccine. The vaccine was well tolerated with minimal localized side-effects and without an increase in antileishmanial antibodies or interleukin (IL)-5. Five volunteers (5/44; 11·4%) had significant IFN-γ production by peripheral blood mononuclear cells (PBMCs) in response to Leishmania antigens in their prevaccination samples (P = 0·001) but were LST non-reactive. On day 45, more than half the volunteers (26/44; 59·0%) had significantly high LST indurations (mean 9·2 ± 2·7 mm) and high IFN-γ levels (mean 1008 ± 395; median 1247 pg/ml). Five volunteers had significant L. donovani antigen-induced IFN-γ production (mean 873 ± 290; median 902; P = 0·001), but were non-reactive in LST. An additional five volunteers (5/44; 11·4%) had low IFN-γ levels (mean 110 ± 124 pg/ml; median 80) and were non-reactive in LST (induration = 00 mm). The remaining eight volunteers had low IFN-γ levels, but significant LST induration (mean 10 ± 2·9 mm; median 11). By day 90 the majority of volunteers (27/44; 61·4%) had significant LST induration (mean 10·8 ± 9·9 mm; P < 0·001), but low levels of L. donovani antigen-induced IFN-γ (mean 66·0 ± 62 pg/ml; P > 0.05). Eleven volunteers (11/44; 25%) had significantly high levels of IFN-γ and LST induration, while five volunteers had low levels of IFN-γ (<100 pg/ml) and no LST reactivity (00 mm). One volunteer was lost to follow-up. In conclusion, it is hypothesized that cellular immune responses to human VL are dichotomatous, and that IFN-γ production and the LST response are not in a causal relationship. Following vaccination and probably cure of VL infection, the IFN-γ response declines with time while the LST response persists.
