Objective: To determine the incidence, risk factors, and prognostic implications of serum creatinine changes following major vascular surgery. Design: Observational study. Settings: University hospital. Patients: Cohort of 599 consecutive patients undergoing elective abdominal aortic surgery. Interventions: Review of prospectively collected data from 1993 to 2004. Measurements and results: The receiver-operator characteristic (ROC) curve analysis was used to detect the best threshold for postoperative elevation in serum creatinine (Δ Creat) in relation to major complications. Acut-off value of +0.5 mg/dl was selected to define renal dysfunction (RD0.5 group, n = 91; no RD0.5, n = 508) that was associated with higher mortality (7.7% in RD0.5 group vs 1.4% in no RD0.5 group, P  40 min; OR, 3.8, 95% CI, 1.9-7.2), blood transfusion (> 5 units; OR, 1.9, 95% CI 1.2-6.1), and rhabdomyolysis (OR, 3.6, 95% CI 1.7-7.9). Conclusions: Postoperative RD0.5 (Δ Creat  > 0.5 mg/dl) occurs in 15% of vascular patients and carries abad prognosis. Preoperative renal insufficiency and factors related to the complexity of surgery are the main predictors of renal dysfunctio

Active Learning of Deterministic Transducers with Outputs in Arbitrary Monoids

    Get PDF
We study monoidal transducers, transition systems arising as deterministic automata whose transitions also produce outputs in an arbitrary monoid, for instance allowing outputs to commute or to cancel out. We use the categorical framework for minimization and learning of Colcombet, Petrişan and Stabile to recover the notion of minimal transducer recognizing a language, and give necessary and sufficient conditions on the output monoid for this minimal transducer to exist and be unique (up to isomorphism). The categorical framework then provides an abstract algorithm for learning it using membership and equivalence queries, and we discuss practical aspects of this algorithm’s implementation

Monotone Weak Distributive Laws over the Lifted Powerset Monad in Categories of Algebras

In both the category of sets and the category of compact Hausdorff spaces, there is a monotone weak distributive law that combines two layers of non-determinism. Noticing the similarity between these two laws, we study whether the latter can be obtained automatically as a weak lifting of the former. This holds partially, but does not generalize to other categories of algebras. We then characterize when exactly monotone weak distributive laws over powerset monads in categories of algebras exist, on the one hand exhibiting a law combining probabilities and non-determinism in compact Hausdorff spaces and showing on the other hand that such laws do not exist in a lot of other cases

In DRC, economic growth remains ineffectively influenced by the dynamics of multiple monetary policy factors. In view of this, an attempt is made in this article, examining the role that the quality of governance could play in the relationship between monetary policy and economic growth in this country during the period 1988- 2018. By developing an econometric analysis using the error correction model, it is revealed that the quality of governance plays a major role in the coordination of monetary policies and this positively impacts the economic growth of the DRC. As a result, government authorities must reform their intervention strategies; they must work with the aim of promoting a harmonious development of economic activities through the gradual approximation of economic policies

Geological Engineering encompasses a body of knowledge and applications that render it indispensable in the pursuit of the Sustainable Development Goals (SDGs) outlined by the United Nations (UN) in 2015. This dissertation aims to shed light on some of these potential interventions, providing an updated overview of associated contributions and presenting recent stu-dies, both international and national, to illustrate them. The goal is to underscore the pivotal role that Geological Engineering currently plays and must continue to play in the immediate and medium-term future for a more sustainable planet. Several interventions are explored within the framework of the UN SDGs. These include the identification and recommendation of potential sources of energy and raw materials, contri- buting to the responsible exploitation and protection of georesources. Geological Engineering also plays a crucial role in supporting the design and construction phases of infrastructures/structures, advocating for solutions that effectively contribute to green engineering, particularly through the reuse/recycling of construction waste. Additionally, this dissertation addresses aspects related to the prevention of geological hazards and the monitoring of the environment or anthropic interventions

Interactions between Temozolomide and Guanine and its S and Se‐Substituted Analogues

Temozolomide was paired with guanine, 6-selenoguanine, and 6-thioguanine, as well as the SH tautomer of the latter. The potential energy surface of each heterodimer was searched for all minima, using Dispersion-Corrected Density Functional Theory and MP2 methods. Among the dozens of minima, three categories were observed. Stacked geometries place the aromatic systems of the two molecules parallel to one another, while the two systems are roughly perpendicular to one another in a second category. Also found are coplanar structures held together by H-bonds. Dispersion proves to be a dominating attractive force for the stacked structures, less so for perpendicular, and smallest for the coplanar dimers. Geometries and energetics are relatively insensitive to S and Se substitution, but tautomerization reverses relative stabilities of different geometries

Background: Road traffic accidents are among the leading causes of morbidity and mortality in Benin Republic, in other words, a serious public health problem here. Consumption of alcoholic drinks is known as an important risk factor for road accidents. The aim of this preliminary study was to determine alcohol ingestion and its relationship with victims of road traffic accidents in Cotonou City, Benin. Methods: It is a descriptive and analytic study which carried out during January-June 2016. In this study, ethanol concentration was measured in the victims of traffic accidents’ blood. Information related to the injured people, their social characteristics, circumstances of accident occurrence, and the outcomes were also collected. Results: In this study, 80 victims of road accidents were included. Most of the victims were men (64%), youth under 40 years (70%), and motorcyclists (83%). The overall prevalence of ethanol ingestion was 55% (64% in motorcyclists and 25% in motorists). Approximately, 16% of them were involved in fatal accidents. There was no association between ethanol ingestion frequency and factors such as the age of the injured, condition of the injured at the time of the accident, time of accident, and road condition on which the accident happened. Conclusion: This study indicated that a lot of traffic accident victims had consumed alcoholic drinks shortly before the accident in Benin
