A genetic cause of Alzheimer disease: mechanistic insights from Down syndrome
Down syndrome, caused by an extra copy of chromosome 21, is associated with a greatly increased risk of early onset Alzheimer disease. It is thought that this risk is conferred by the presence of three copies of the gene encoding amyloid precursor protein (APP), an Alzheimer risk factor, although the possession of extra copies of other chromosome 21 genes may also play a role. Further study of the mechanisms underlying the development of Alzheimer disease in Down syndrome could provide insights into the mechanisms that cause dementia in the general population
Pica associated with iron deficiency or depletion: clinical and laboratory correlates in 262 non-pregnant adult outpatients
<p>Abstract</p> <p>Background</p> <p>There are many descriptions of the association of pica with iron deficiency in adults, but there are few reports in which observations available at diagnosis of iron deficiency were analyzed using multivariable techniques to identify significant predictors of pica. We sought to identify clinical and laboratory correlates of pica in adults with iron deficiency or depletion using univariable and stepwise forward logistic regression analyses.</p> <p>Methods</p> <p>We reviewed charts of 262 non-pregnant adult outpatients (ages ≥18 y) who required treatment with intravenous iron dextran. We tabulated their sex, age, race/ethnicity, body mass index, symptoms and causes of iron deficiency or depletion, serum iron and complete blood count measures, and other conditions at diagnosis before intravenous iron dextran was administered. We excluded patients with serum creatinine >133 μmol/L or disorders that could affect erythrocyte or iron measures. Iron deficiency was defined as both SF <45 pmol/L and TS <10%. Iron depletion was defined as serum ferritin (SF) <112 pmol/L. We performed univariable comparisons and stepwise forward logistic regression analyses to identify significant correlates of pica.</p> <p>Results</p> <p>There were 230 women (184 white, 46 black; ages 19-91 y) and 32 men (31 white, 1 black; ages 24-81 y). 118 patients (45.0%) reported pica; of these, 87.3% reported ice pica (pagophagia). In univariable analyses, patients with pica had lower mean age, black race/ethnicity, and higher prevalences of cardiopulmonary and epithelial manifestations. The prevalence of iron deficiency, with or without anemia, did not differ significantly between patients with and without pica reports. Mean hemoglobin and mean corpuscular volume (MCV) were lower and mean red blood cell distribution width (RDW) and platelet count were higher in patients with pica. Thrombocytosis occurred only in women and was more prevalent in those with pica (20.4% vs. 8.3%; p = 0.0050). Mean total iron-binding capacity was higher and mean serum ferritin was lower in patients with pica. Nineteen patients developed a second episode of iron deficiency or depletion; concordance of recurrent pica (or absence of pica) was 95%. Predictors of pica in logistic regression analyses were age and MCV (negative associations; p = 0.0250 and 0.0018, respectively) and RDW and platelet count (positive associations; p = 0.0009 and 0.02215, respectively); the odds ratios of these predictors were low.</p> <p>Conclusions</p> <p>In non-pregnant adult patients with iron deficiency or depletion, lower age is a significant predictor of pica. Patients with pica have lower MCV, higher RDW, and higher platelet counts than patients without pica.</p

For two weeks prior to the start of the breeding season, the suckling behaviour of beef calves, separated from their dams for 12 h out of 24, was investigated.Fifty-six per cent of the 44 cows which were separated from their calves from 18:00 to 06:00 (day-sucklers)exhibited oestrus within 30 days of the start of the breeding season, compared to 22% of the group separated from 06:00 to 18:00 (P < 0.01). A central nervous system-focused treatment approach for people with frozen shoulder: protocol for a randomized clinical trial
Background: Frozen shoulder (FS) is a musculoskeletal condition of poorly understood etiology that results in shoulder pain and large mobility deficits. Despite some physical therapy interventions, such as joint mobilization and exercise, having shown therapeutic benefit, a definitive treatment does not currently exist. The aim of this study will be to compare the effectiveness of a central nervous system (CNS)-directed treatment program versus a standard medical and physical therapy care program on outcomes in participants with FS.
Methods/design: The study is a two-group, randomized clinical trial with blinding of participants and assessors. Participants will be recruited via referrals from orthopedic surgeons and physical therapists, community-based advertisements, private care practices and hospitals. Participants will be randomized to receive either a CNS-focused treatment program or standard medical and physical therapy care. The Shoulder Pain And Disability Index (SPADI) will be the primary outcome, while the Numeric Pain Rating Scale (NPRS), shoulder range of movement (ROM), The Patient Specific Functional Scale, two-point discrimination threshold and laterality judgement accuracy will be the secondary outcomes. Assessment will occur at baseline, at the end of the treatment program (week 10), and at 3 and 6 months' follow-up.
Discussion: Preliminary data suggest that treatments that target CNS function are a promising approach to the treatment of people with shoulder pain including patients with FS. In the context of modest effects from most available physical therapy treatments for FS, this CNS-focused approach may lead to improved clinical outcomes. The trial should determine if the CNS-directed program is more effective than traditional interventions at reducing pain intensity and improving function in a FS cohort and will follow up participants for 6 months, providing important information on the persistence of any treatment effects.This work is funded by the Conselleria de Educación, Investigación, Cultura y Deporte de la Generalitat Valenciana, Spain (GV/2016).Ye
Climate variability in the SW Indian Ocean from an 8000-yr long multi-proxy record in the Mauritian lowlands shows a middle to late Holocene shift from negative IOD-state to ENSO-state
A multi-proxy reconstruction of a sediment core from the Tatos basin in the Mauritian lowlands reveals a dynamic environmental history during the last 8000 years. Under influence of sea level rise, the basin progressed from a wetland to a shallow lake between 8000 and 2500 cal yr BP and it slowly changed back into a wetland after sea level reached its highest position at around 2500 cal yr BP. The groundwater level in the basin was strongly affected by sea level rise and precipitation-forced runoff through the porous volcanic bedrock.<br>Millennial-scale precipitation changes in the Mauritian lowlands were derived from the pollen records of semi-dry forest and palm woodland. Salinity and environmental reconstructions based on diatoms, ostracods, stable isotopes and sediment composition showed numerous decadal and centennial droughts and wet events. Mauritius experienced wet conditions between similar to 8000 and similar to 6800 cal yr BP, followed by decreasing humidity from 6800 to 6000 cal yr BP. Dry conditions persisted until similar to 1200 cal yr BP, after which wetter conditions have prevailed as recorded from Mauritian lowland and upland records. Climate dynamics reflects northern hemisphere monsoon activity and suggest that Mauritian rainfall and the Indian and Asian summer monsoons are linked, as both receive moisture from the southern equatorial Indian Ocean.<br>The anti-phased relationship of climate dynamics between the Mauritian lowlands and western tropical Australia during the middle Holocene is interpreted as a prolonged configuration of a negative mode of the Indian Ocean Dipole OM. A negative IOD-like state is supported by decreased Asian summer monsoon rainfall, higher Austral-Indonesian summer monsoon rainfall and lower temperatures in the Kilimanjaro record. Conversely, repeated decadal-scale wet events in the Mauritian lowlands occurring every similar to 350 years reflect short positive IOD-like events.<br>The onset of ENSO climate variability followed an anomalously strong negative 100-like event and shifted teleconnections from the tropical Indian Ocean to the Pacific Ocean. A shift in ENSO activity around similar to 2600 cal yr BP signifies the decoupling of ENSO from the Atlantic ITCZ. Subsequently, the influence of ENSO on climate in the western Indian Ocean is indicated by increased storm frequency and drought events after 2660 cal yr BP in Mauritius and reduced monsoon activity in the western and eastern Indian Ocean
