Arriving at the scene, the officers found a husband and wife arguing, but no one was physically hurt. Stephen, the husband, was intoxicated. His wife, Linda, said that he had been out of work for six months, and they were having financial problems. Recently he’d also been depressed and lethargic, and kept saying things like “I’m tired of all this and just want to end it all. ” Joe asked Stephen if he was thinking of killing himself. Stephen admitted that he had considered taking his life. Joe said that Stephen needed to go to the hospital so that he could be evaluated and get help to deal with his problems. After some hesitation and reluctance, Stephen finally agreed to go. At the hospital, Joe told the emergency department physician that Stephen had become depressed after being unemployed for six months and he had been drinking and had admitted that he considered taking his life. Selenium status in beef and dairy cattle at Taman Pertanian Universiti, Universiti Putra Malaysia
Selenium (Se) nutrition forms an integral part of growth and immune function of man and animals. Deficiency of Se in cattle will inevitably lead to poor growth and mastitis causing an economic loss to a farm. This study was carried out to determine the level of Se in beef cattle and dairy cattle reared at the Taman Pertanian Universiti, Universiti Putra Malaysia. A total of 27 dairy and 36 beef cattle were randomly chosen in this study. The age of the cattle used ranged from 1 to 9 years old comprising of both sexes. Blood sample was collected into heparinized and plain tubes for the detection of glutathione peroxidase (GSH-Px) activity and aspartate aminotransferase (AST), creatine kinase (CK) and malonylaldehyde (MDA) concentrations, respectively. The activity of GSH-Px was detected using the dithio-bi-nitrobenzoic acid (DTNB) direct method assay. The concentrations of AST and CK were determined using standard diagnostic kits, while the MDA using the TBA (thio-barbituric acid) test. It was found that although no significant differences existed in the GSH-Px activity between breeds, their Se status were in the deficient state. This is in line with previous reports that the acidic soil of Malaysia renders low Se uptake by plants leading to deficiency state in grazing ruminants. The concentrations of AST and CK were higher in beef cattle due to much more overt muscular damage as the result of foraging and lack of supplementation compared to dairy cattle. It is suggested that diet of both breeds should be supplemented with Se to prevent such deficient status in order to increase the productivity of the farm

The Governance Analyisis on District Integrated Administrative Services (PATEN) Programe: Case Study In West Semarang District of Semarang City aims to explain any innovations that contained in PATEN, how the innovation was done, the underlying reasons of Semarang City Goverment on making decisions about PATEN Programe which only held in non-permission services, and the impact for society as service users in West Semarang District about the implementation of PATEN. The method of research used in this study is Mixed Method Research with concurrent embedded strategy, which both qualitative and quantitative data were collected at the same time. For collecting data, researcher conducted interviews at the office of West Semarang District and the government of Semarang City and then distributed questionnaire to communities in the region of West Semarang District. Researcher also used secondary data from documents of PATEN and other literature sources such as books and journals. The implementation of PATEN brings about changes in performance of employee districts and has impact for the community. The presence of PATEN is not only to improve the quality of service in the district but also for a better and closer service to the community. The changes that occur through of PATEN such as more modern service system, establishment of service standards, and the provision of service facilities. In implementation of PATEN, the government of Semarang City has several considerations include the substantive requirements have not been met, sectoral ego among departments, barriers on standard of implementation, outside of the priority scale, the new constitution of local government, conditions of urban area, and the indecisiveness of sanctions of the central government. Furthermore, as the impact for the community, PATEN was able to provide a positive impact although most people said that they did not know about PATEN in Semarang City. Therefore, the government should also involve the community participation, so that in the future the authorities that are delegated to the districts will be suitable to the needs of society. On the other hand, society should be more active in participating to oversee the implementation of public services in the district level

But now life was too difficult with the new baby and her husband unemployed, and she just couldn’t take it anymore. Jim suggested that Maria and her husband go to the hospital to talk with a doctor to get some help for Maria, and Maria hesitantly agreed. The CEA Second-Look Trial: a randomised controlled trial of carcinoembryonic antigen prompted reoperation for recurrent colorectal cancer
Objective In patients who have undergone a potentially curative resection of colorectal cancer, does a ‘second-look’ operation to resect recurrence, prompted by monthly monitoring of carcinoembryonic antigen, confer a survival benefit?Design A randomised controlled trial recruiting patients from 1982 to 1993 was recovered under the Restoring Invisible and Abandoned Trials (RIAT) initiative.Setting 58 hospitals in the UK.Participants From 1982 to 1993, 1447 patients were enrolled. Of these 216 met the criteria for carcinoembryonic antigen (CEA) elevation and were randomised to ‘Aggressive’ or ‘Conventional’ arms.Interventions ‘Second-look’ surgery with intention to remove any recurrence discovered.Primary outcome measure Survival.Results By February 1993, 91/108 patients had died in the ‘Aggressive arm’ and 88/108 in the ‘Conventional’ arm (relative risk=1.16, 95% CI 0.87 to 1.37). By 2011 a further 25 randomised patients had died. Kaplan-Meier analysis showed no difference in long-term survival.Conclusions The trial was closed in 1993 following a recommendation from the Data Monitoring Committee that it was highly unlikely that any survival advantage would be demonstrated for CEA prompted second-look surgery. This conclusion was confirmed by repeat analysis of survival times after 20 years.Trial registration number ISRCTN76694943
DISCRIMINATION, STIGMATISATION AND RIGHTS OF AUTISTIC CHILDREN TO HEALTH IN UGANDA
Autism spectrum is a neurodevelopmental disorder that has adverse effects on the development of an individual. This condition usually impairs an individual‟s ability to carry out ordinary day-to-day functions such as eating, bathing, socializing and communication. If detected early, some of these skills can be taught to improve the individual‟s quality of life as they grow into adulthood. However, autism is usually misunderstood and misdiagnosed as a mental disorder to the detriment of the autistic child. In fact, some societies in Africa attribute this condition to a curse, evil spirit possession, or sins of the parents which usually leads to discrimination and stigmatisation. Caregivers and parents often hide autistic children from society for fear of judgment or derison. As a result, some autistic children are only let out in the open when it is extremely necessary because of their underdeveloped social skills. Such concealment is detrimental to autistic children and prevents them from accessing life changing help required for their intellectual and social development. In addition, specialist autistic care is often too expensive for parents and guardians. These factors usually leave autistic children at the mercy of fate even where there is a willingness to seek help. This article seeks to establish an understanding of the autistic disorder and the effect it has on the development of an individual.
