Triple Primary Carcinomas: Prostatic Adenocarcinoma, Bladder Urethral Carcinoma and Papillary Thyroid Carcinoma: A Case Report
Introduction: Patients with multiple tumors represent a segment of the cancer survivor population affected more than once by cancer, the phenomenon is still rare.Case presentation: In this report we present a patient who developed primary bladder urethral carcinoma with synchronous prostatic adenocarcinoma and metachronous papillary thyroid carcinoma where diagnosed within nine month period. There was no clear risk factors could explain this combination except smoking history.Conclusion: The diagnosis of cancer should not exclude the existence of other concomitant malignancies. This combination of multiple primary carcinomas, to our knowledge, has never been reported in the literature
Solid Pseudopapillary Tumor of the Pancreas: A Case Report
Introduction: Solid pseudopapillary tumor (SPT) of the pancreas is rare, accounting for 0.13-2.7% of all pancreatic tumors. It has specific clinical, pathological and radiological characters that make it quite different from other pancreatic tumors.Presentation of case: A 15-year-old girl with SPT of the pancreas diagnosed after surgical resection with histopathology and immunohistochemistry confirmation. As no clear role for adjuvant treatment, she was elected for follow up.Conclusion: Although SPT is a rare tumor without notable symptoms. Complete surgical excision is the treatment of choice. The place of chemotherapy or radiotherapy needed to be elucidated
The Effect of Proton Pump Inhibitors on Metastatic Breast Cancer in Patients Treated with CDK4/6i
Background: This study aimed to assess the effect of proton pump inhibitors (PPIs) on hormone receptor-positive (HR+) and her-2-negative (her2-) metastatic breast cancer (MBC) in patients receiving CDK4/6i.Method: In a retrospective study, patients were divided into two groups based on their use of PPIs: concomitant use for at least half the treatment period (C-PPIs) and non-concomitant use for less than half the treatment period (NC-PPIs). Statistical analyses were conducted using SPSS 22.0 and MedCalc Software bvba 13.Results: Out of 217 patients, 114 (52.5%) received palbociclib and 103 (47.5%) received ribociclib. Then, 71 palbociclib recipients were combined with aromatase inhibitors (AIs) (43 C-PPIs and 28 NC-PPIs), while 43 were combined with fulvastrant (25 C-PPIs and 18 NC-PPIs). For ribociclib, 82 patients were combined with AIs (71 C-PPIs and 11 NC-PPIs) and 21 were combined with fulvastrant (10 C-PPIs and 11 NC-PPIs). Patients with C-PPI had lower progression-free survival (PFS) than those without C-PPI, palbociclib recipients with AIs or fulvastrant (8.97 months vs. 19.02 months, P < 0.001; 13.72 months vs. 18.52 months, P = 0.04, respectively) and ribociclib recipients with AIs (10.7 months vs. 19.7 months, P = 0.05), but not in patients who received ribociclib with fulvastrant (P = 0.52).Conclusion: The simultaneous use of PPIs with palbociclib or ribociclib may be associated with a shorter PFS in HR+, HER2- MBC patients. These results have the potential to enhance clinical decision-making by identifying possible drug interactions and optimizing treatment plans

Scope of management of emergencies in palliative care
Patients receiving palliative care may deteriorate suddenly due to their illness or another acute medical or surgical problem. Management options depend on life expectancy, level of intervention needed, and an assessment of risks, benefits, side effects and likely outcome. Symptom control and supportive care may be the most appropriate management if the patient is dying. Treatment options are to be discussed with the patient and family; if possible, discussion and documentation of the patient’s wishes should be made in advance including those about resuscitation, hospital admission and transfer to an intensive care unit

Impact of vitamin D serum levels on clinicopathological features and outcome in advanced pancreatic carcinoma
Abstract
Background
Growing evidence encourages the preventive role of vitamin D in pancreatic carcinoma (PC). Meanwhile, the prognostic or predictive role needs more investigations. This study aimed to evaluate the correlation between serum vitamin D levels and the clinicopathological features with the outcome in advanced pancreatic carcinoma (APC).
Materials and methods
The current prospective study included 176 patients with APC. Assessing 25-hydroxy vitamin D is the most accurate method to measure the serum vitamin D levels. Serum vitamin D levels <20 ng/ml are defined as vitamin D deficiency, while levels ranging from 20 to 29 ng/ml are defined as vitamin D insufficiency. Before any treatment modalities were administered, the serum vitamin D levels were measured by enzyme-linked immunosorbent assay (ELISA).
Results
Serum vitamin D insufficiency and deficiency were detected in 28.4% and 31.8%, respectively. A large tumor size, higher grade, liver metastasis, higher serum level of CA 19-9, poor ECOG PS, and low overall response rate (ORR) were associated with lower serum vitamin D levels (p = 0.000). The median follow-up period was 7.6 months (range 0.6–18.6). The ORR was 23.2%, 54%, and 82.9% of vitamin D deficiency, insufficiency, and normal levels of vitamin D, respectively. The median OS was 11.4 months for patients with normal serum vitamin D levels, compared with 2.7 and 7.03 months for serum vitamin D deficiency and insufficiency, respectively.
Conclusions
Among patients with APC, serum vitamin D levels are considered a promising prognostic factor. It is associated with various poor prognostic features and worse survival outcome.

CDK4/6 inhibitors in advanced breast cancer, what is beyond?
Resistant to hormonal treatment considered the main clinical challenge in the management of advanced breast cancer (ABC). The use of CDK4/6 inhibitors (CDK4/6I) may change the treatment landscape. In this mandated review, we will focus on the applicable role of CDK4/6I in the management of HR+/HER2- ABC, mechanisms of resistance, and promising future implementation.</jats:p
