The Parkinsonian subthalamic network: measures of power, linear, and non-linear synchronization and their relationship to L-DOPA treatment and OFF state motor severity
In this paper we investigated the dopaminergic modulation of neuronal interactions occurring in the subthalamic nucleus (STN) during Parkinson's disease (PD). We utilized linear measures of local and long range synchrony such as power and coherence, as well as Detrended Fluctuation Analysis for Phase Synchrony (DFA-PS)- a recently developed non-linear method that computes the extent of long tailed autocorrelations present in the phase interactions between two coupled signals. Through analysis of local field potentials (LFPs) taken from the STN we seek to determine changes in the neurodynamics that may underpin the pathophysiology of PD in a group of 12 patients who had undergone surgery for deep brain stimulation. We demonstrate up modulation of alpha-theta (5–12 Hz) band power in response to L-DOPA treatment, whilst low beta band power (15–20 Hz) band-power is suppressed. We also find evidence for significant local connectivity within the region surrounding STN although there was evidence for its modulation via administration of L-DOPA. Further to this we present evidence for a positive correlation between the phase ordering of bilateral STN interactions and the severity of bradykinetic and rigidity symptoms in PD. Although, the ability of non-linear measures to predict clinical state did not exceed standard measures such as beta power, these measures may help identify the connections which play a role in pathological dynamics

IGFBPL1 Regulates Axon Growth through IGF-1-mediated Signaling Cascades
Activation of axonal growth program is a critical step in successful optic nerve regeneration following injury. Yet the molecular mechanisms that orchestrate this developmental transition are not fully understood. Here we identified a novel regulator, insulin-like growth factor binding protein-like 1 (IGFBPL1), for the growth of retinal ganglion cell (RGC) axons. Expression of IGFBPL1 correlates with RGC axon growth in development, and acute knockdown of IGFBPL1 with shRNA or IGFBPL1 knockout in vivo impaired RGC axon growth. In contrast, administration of IGFBPL1 promoted axon growth. Moreover, IGFBPL1 bound to insulin-like growth factor 1 (IGF-1) and subsequently induced calcium signaling and mammalian target of rapamycin (mTOR) phosphorylation to stimulate axon elongation. Blockage of IGF-1 signaling abolished IGFBPL1-mediated axon growth, and vice versa, IGF-1 required the presence of IGFBPL1 to promote RGC axon growth. These data reveal a novel element in the control of RGC axon growth and suggest an unknown signaling loop in the regulation of the pleiotropic functions of IGF-1. They suggest new therapeutic target for promoting optic nerve and axon regeneration and repair of the central nervous system
Elective cancer surgery in COVID-19-free surgical pathways during the SARS-CoV-2 pandemic: An international, multicenter, comparative cohort study
PURPOSE As cancer surgery restarts after the first COVID-19 wave, health care providers urgently require data to determine where elective surgery is best performed. This study aimed to determine whether COVID-19–free surgical pathways were associated with lower postoperative pulmonary complication rates compared with hospitals with no defined pathway. PATIENTS AND METHODS This international, multicenter cohort study included patients who underwent elective surgery for 10 solid cancer types without preoperative suspicion of SARS-CoV-2. Participating hospitals included patients from local emergence of SARS-CoV-2 until April 19, 2020. At the time of surgery, hospitals were defined as having a COVID-19–free surgical pathway (complete segregation of the operating theater, critical care, and inpatient ward areas) or no defined pathway (incomplete or no segregation, areas shared with patients with COVID-19). The primary outcome was 30-day postoperative pulmonary complications (pneumonia, acute respiratory distress syndrome, unexpected ventilation). RESULTS Of 9,171 patients from 447 hospitals in 55 countries, 2,481 were operated on in COVID-19–free surgical pathways. Patients who underwent surgery within COVID-19–free surgical pathways were younger with fewer comorbidities than those in hospitals with no defined pathway but with similar proportions of major surgery. After adjustment, pulmonary complication rates were lower with COVID-19–free surgical pathways (2.2% v 4.9%; adjusted odds ratio [aOR], 0.62; 95% CI, 0.44 to 0.86). This was consistent in sensitivity analyses for low-risk patients (American Society of Anesthesiologists grade 1/2), propensity score–matched models, and patients with negative SARS-CoV-2 preoperative tests. The postoperative SARS-CoV-2 infection rate was also lower in COVID-19–free surgical pathways (2.1% v 3.6%; aOR, 0.53; 95% CI, 0.36 to 0.76). CONCLUSION Within available resources, dedicated COVID-19–free surgical pathways should be established to provide safe elective cancer surgery during current and before future SARS-CoV-2 outbreaks

Prospective, multicentre study of screening, investigation and management of hyponatraemia after subarachnoid haemorrhage in the UK and Ireland
Background Hyponatraemia often occurs after subarachnoid haemorrhage (SAH). However, its clinical significance and optimal management are uncertain. We audited the screening, investigation and management of hyponatraemia after SAH.
Methods We prospectively identified consecutive patients with spontaneous SAH admitted to neurosurgical units in the United Kingdom or Ireland. We reviewed medical records daily from admission to discharge, 21 days or death and extracted all measurements of serum sodium to identify hyponatraemia (10 days. Associations of hyponatraemia with outcome were assessed using logistic regression with adjustment for predictors of outcome after SAH and admission duration. We assessed hyponatraemia-free survival using multivariable Cox regression.
Results 175/407 (43%) patients admitted to 24 neurosurgical units developed hyponatraemia. 5976 serum sodium measurements were made. Serum osmolality, urine osmolality and urine sodium were measured in 30/166 (18%) hyponatraemic patients with complete data. The most frequently target daily fluid intake was >3 L and this did not differ during hyponatraemic or non-hyponatraemic episodes. 26% (n/N=42/164) patients with hyponatraemia received sodium supplementation. 133 (35%) patients were dead or dependent within the study period and 240 (68%) patients had hospital admission for over 10 days. In the multivariable analyses, hyponatraemia was associated with less dependency (adjusted OR (aOR)=0.35 (95% CI 0.17 to 0.69)) but longer admissions (aOR=3.2 (1.8 to 5.7)). World Federation of Neurosurgical Societies grade I–III, modified Fisher 2–4 and posterior circulation aneurysms were associated with greater hazards of hyponatraemia.
Conclusions In this comprehensive multicentre prospective-adjusted analysis of patients with SAH, hyponatraemia was investigated inconsistently and, for most patients, was not associated with changes in management or clinical outcome. This work establishes a basis for the development of evidence-based SAH-specific guidance for targeted screening, investigation and management of high-risk patients to minimise the impact of hyponatraemia on admission duration and to improve consistency of patient care

Variation in postoperative outcomes of patients with intracranial tumors: insights from a prospective international cohort study during the COVID-19 pandemic
Background:
This study assessed the international variation in surgical neuro-oncology practice and 30-day outcomes of patients who had surgery for an intracranial tumor during the COVID-19 pandemic.
Methods:
We prospectively included adults aged ≥18 years who underwent surgery for a malignant or benign intracranial tumor across 55 international hospitals from 26 countries. Each participating hospital recorded cases for 3 consecutive months from the start of the pandemic. We categorized patients’ location by World Bank income groups (high [HIC], upper-middle [UMIC], and low- and lower-middle [LLMIC]). Main outcomes were a change from routine management, SARS-CoV-2 infection, and 30-day mortality. We used a Bayesian multilevel logistic regression stratified by hospitals and adjusted for key confounders to estimate the association between income groups and mortality.
Results:
Among 1016 patients, the number of patients in each income group was 765 (75.3%) in HIC, 142 (14.0%) in UMIC, and 109 (10.7%) in LLMIC. The management of 200 (19.8%) patients changed from usual care, most commonly delayed surgery. Within 30 days after surgery, 14 (1.4%) patients had a COVID-19 diagnosis and 39 (3.8%) patients died. In the multivariable model, LLMIC was associated with increased mortality (odds ratio 2.83, 95% credible interval 1.37–5.74) compared to HIC.
Conclusions:
The first wave of the pandemic had a significant impact on surgical decision-making. While the incidence of SARS-CoV-2 infection within 30 days after surgery was low, there was a disparity in mortality between countries and this warrants further examination to identify any modifiable factors
Insights into the neural basis of paradoxical kinesia in Parkinson’s disease
Enhancements in peak motor performance have been demonstrated in response to intense stimuli both in healthy subjects and in the form of ‘paradoxical kinesis’ in patients with Parkinson’s disease. Might the latter phenomenon thus reflect a physiological process? The first study outlined in this thesis suggests this may be the case, as maximal effort grips in healthy subjects undergo dramatic enhancements when the imperative visual cue is accompanied by an intense auditory tone. Analogous enhancements in motor performance are demonstrated in a second study of patients with Parkinson’s disease and age-matched healthy controls. Remarkably, the facilitating effect of loud auditory tones is similar whether patients are off or on dopaminergic medication, suggesting a potentially non-dopaminergic basis for the phenomenon. A role of sub-cortical systems in the performance enhancements engendered by intense stimuli is next considered. Local field potentials recorded from the subthalamic nuclei of patients with Parkinson’s disease, whilst they undertake the above established paradigm, identify both theta/alpha (5-12 Hz) and high gamma/high frequency (55-375 Hz) activity as exhibiting remarkable scaling with maximal motor responses to the visual cue alone, but having little explanatory influence on performance enhancements beyond this. In the final study, a short-latency evoked potential in subthalamic nucleus local field potential recordings, which scales in amplitude with both stimulus intensity and corresponding enhancements in biomechanical measures of maximal handgrips, is identified. Interference with this potential through high frequency deep brain stimulation of the same nucleus, leads to a diminished behavioural effect of stimulus intensity. Recordings of a similar evoked potential in the related pedunculopontine nucleus – a key component of the reticular activating system – provide support for this neural signature as a physiological correlate of ascending arousal, propagated from the reticular activating system to exert an ‘energizing’ influence on motor circuitry through the subthalamic nucleus
IMPACT OF DIGITAL TECHNOLOGY ON HEALTH AND SOCIAL WELL-BEING OF DIGITAL NATIVES
Purpose of the study: The purpose of this research article is to explore how engagement with digital tools affects the health and social well-being of digital natives. The study aims to look into the facts that how youth are getting benefits from this digitalization and how it is also increasing their exposure to the associated health risks.
Methodology: The qualitative research is conducted in three selected universities of Islamabad. Through a closed-ended questionnaire, demographic data about the activities of the youth on digital technology was collected. In-depth interviews, focus group discussions and participant observation were the core techniques used in the research.
Main Findings: Digital natives have improved awareness about health issues and actively educating the digital immigrants to access knowledge about treatments on various health issues at the local and global levels. The research shows that digital youth do realize that these technologies are leaving some bad effects on their health and well-being yet have not been able to get rid of them because their daily life activities are dependent on these tools.
Applications of this study: The study will be of great significance for the policymakers, who are handling the rising health issues due to digitalization impact in the society. It will be an addition to the knowledge in the disciplines of Public health, Anthropology, and Psychology besides the medical sector. Data will be used to address the health issues in the coming days by the medical professionals and social scientists.
Novelty/Originality of this study: This study has generated the information about the impact of digitalization on youth’s health by using the emic approach. Digital technology has taken up the role of socialization agent for the digital natives. So it is very important to listen to the youth's voices rather than assuming what is best for them. This study is novel in the sense that it is reflecting youth and how they perceive the digital technology as a beneficial tool and how it risks their life in many ways.</jats:p
MORTUARY RITES IN COVID-19: MOURNING & BURIAL RITES OF MIGRANTS IN NORTHERN PUNJAB
Purpose of the study: This study aims to highlight how death serves as a central feature of social ties among the natives of Northern Punjab. Death is a great leveller and one of the most curious aspects of human cognition. Bereavement follows the terminal rites de passage; the transition of the deceased from this world to the other world.
Methodology: By using an inductive approach, the ethnographic account of the most significant rite of passage; death was gathered. The primary data is based on case-based narratives and empirical findings gathered during in-depth interviews and participant observation at the locale. A total of thirteen cases of death migrants are discussed in this paper.
Main Findings: The findings reveal the social pressures the family of the deceased encountered in the pre and post-death phase both in the country of origin and in the country of destination, how horrors of COVID-19 infection kept the entire bereaved families at a halt to decide their funerary rituals, pandemic’s effect on the body’s postmortem clearance and death certificate, arrangement for the morgue and grave while the decision of burial was in process, familial politics engaged in decision making, the journey back to the native soil, the burial, mourning, condolence and bereavement rituals of Potohar.
Applications of this study: This paper solely focuses on the death rituals of migrants from the Northern Punjab region in COVID-19. The study provides an understanding of the religio-cultural rituals and their transformation in the global pandemic.
Novelty/Originality of this study: The researcher has prepared an account of the death rituals based on the close observations and in-depth insights during the mortuary rites of migrants who expired during the pandemic COVID-19 during doctoral research. No such research has been carried out in Potohar (Northern Punjab) in this context.</jats:p
