    An adaptive observer-based controller design for active damping of a DC network with a constant power load

    This article explores a nonlinear, adaptive controller aimed at increasing the stability margin of a direct-current (dc), small-scale, electrical network containing an unknown constant power load (CPL). Due to its negative incremental impedance, this load reduces the effective damping of the network, which may lead to voltage oscillations and even to voltage collapse. To overcome this drawback, we consider the incorporation of a controlled dc-dc power converter in parallel with the CPL. The design of the control law for the converter is particularly challenging due to the existence of unmeasured states and unknown parameters. We propose a standard input-output linearization stage, to which a suitably tailored adaptive observer is added. The good performance of the controller is validated through experiments on a small-scale network

    Marketing para comunicar servicios profesionales farmacéuticos: experiencia en una farmacia comunitaria de l’Alfàs del Pi

    Objetivos: Iniciar un nuevo servicio remunerado para el control y cribado de factores de riesgo cardiovascular, promocionándolo mediante una novedosa campaña de marketing. Conocer el nivel de riesgo cardiovascular de nuestros usuarios, así como de algunos de sus principales factores modificables asociados.Material y métodos: Estudio observacional transversal en usuarios adultos que solicitaron participar en la campaña. Esta se denominó “First Date: una cita con tu corazón” (haciendo alusión todos sus materiales al conocido programa televisivo) y se ofreció a los pacientes un Menú Especial a 12€ compuesto por un Entrante (Perímetro abdominal, IMC, PA y Toma de pulso), Plato Principal (Colesterol total y Edad Vascular) y Postre (nivel RCV).Resultados: Realizamos 34 menús: edad media 57,8 años y 53% de mujeres. Encontramos valores por encima de los límites normales, en (% de la población total): perímetro abdominal (50%), IMC (18% sobrepeso y 5% obesidad), colesterol total (70,6%), VOP (23,5%) y PAS (14,7% PAS). Respecto a la PAD se encontraron diferencias significativas entre ambos sexos: 70.0 ± 2.0 mmHg en mujeres y 83.1 ± 3.2 mmHg en hombres (**p=0.0012 t test). También se encontraron diferencias entre sexos en el hábito tabáquico (el 75% de los fumadores eran hombres) y de consumo de alcohol (el 72,7% de consumidores habituales eran hombres). El 29,4% de la población tenían RCV medio y el 20,6% elevado. Encontramos pulso irregular sin diagnóstico previo en el 11,8% de pacientes (2 hombres y 2 mujeres). Se realizaron 4 derivaciones al médico, 7 derivaciones para control de PA en la farmacia, 26 recomendaciones sobre dieta y ejercicio, 6 recomendaciones sobre tabaco y 3 intervenciones en adherencia.Conclusiones: Ayudados por la campaña de marketing, se consiguió comunicar e iniciar adecuadamente nuestro servicio profesional (remunerado) de cribado y control de los factores de riesgo cardiovascular. Los valores encontrados en nuestra población tienen un margen de mejora elevado, sobre todo en cuanto a los niveles de PAD, colesterol total, perímetro abdominal y nivel de RCV

    A transcriptomic analysis of gene expression in the venom gland of the snake Bothrops alternatus (urutu)

    <p>Abstract</p> <p>Background</p> <p>The genus <it>Bothrops </it>is widespread throughout Central and South America and is the principal cause of snakebite in these regions. Transcriptomic and proteomic studies have examined the venom composition of several species in this genus, but many others remain to be studied. In this work, we used a transcriptomic approach to examine the venom gland genes of <it>Bothrops alternatus</it>, a clinically important species found in southeastern and southern Brazil, Uruguay, northern Argentina and eastern Paraguay.</p> <p>Results</p> <p>A cDNA library of 5,350 expressed sequence tags (ESTs) was produced and assembled into 838 contigs and 4512 singletons. BLAST searches of relevant databases showed 30% hits and 70% no-hits, with toxin-related transcripts accounting for 23% and 78% of the total transcripts and hits, respectively. Gene ontology analysis identified non-toxin genes related to general metabolism, transcription and translation, processing and sorting, (polypeptide) degradation, structural functions and cell regulation. The major groups of toxin transcripts identified were metalloproteinases (81%), bradykinin-potentiating peptides/C-type natriuretic peptides (8.8%), phospholipases A<sub>2 </sub>(5.6%), serine proteinases (1.9%) and C-type lectins (1.5%). Metalloproteinases were almost exclusively type PIII proteins, with few type PII and no type PI proteins. Phospholipases A<sub>2 </sub>were essentially acidic; no basic PLA<sub>2 </sub>were detected. Minor toxin transcripts were related to L-amino acid oxidase, cysteine-rich secretory proteins, dipeptidylpeptidase IV, hyaluronidase, three-finger toxins and ohanin. Two non-toxic proteins, thioredoxin and double-specificity phosphatase Dusp6, showed high sequence identity to similar proteins from other snakes. In addition to the above features, single-nucleotide polymorphisms, microsatellites, transposable elements and inverted repeats that could contribute to toxin diversity were observed.</p> <p>Conclusions</p> <p><it>Bothrops alternatus </it>venom gland contains the major toxin classes described for other <it>Bothrops </it>venoms based on trancriptomic and proteomic studies. The predominance of type PIII metalloproteinases agrees with the well-known hemorrhagic activity of this venom, whereas the lower content of serine proteases and C-type lectins could contribute to less marked coagulopathy following envenoming by this species. The lack of basic PLA<sub>2 </sub>agrees with the lower myotoxicity of this venom compared to other <it>Bothrops </it>species with these toxins. Together, these results contribute to our understanding of the physiopathology of envenoming by this species.</p

    Predicting the length of mechanical ventilation in acute respiratory disease syndrome using machine learning: The PIONEER Study

    Background: The ability to predict a long duration of mechanical ventilation (MV) by clinicians is very limited. We assessed the value of machine learning (ML) for early prediction of the duration of MV > 14 days in patients with moderate-to-severe acute respiratory distress syndrome (ARDS). Methods: This is a development, testing, and external validation study using data from 1173 patients on MV ≥ 3 days with moderate-to-severe ARDS. We first developed and tested prediction models in 920 ARDS patients using relevant features captured at the time of moderate/severe ARDS diagnosis, at 24 h and 72 h after diagnosis with logistic regression, and Multilayer Perceptron, Support Vector Machine, and Random Forest ML techniques. For external validation, we used an independent cohort of 253 patients on MV ≥ 3 days with moderate/severe ARDS. Results: A total of 441 patients (48%) from the derivation cohort (n = 920) and 100 patients (40%) from the validation cohort (n = 253) were mechanically ventilated for >14 days [median 14 days (IQR 8–25) vs. 13 days (IQR 7–21), respectively]. The best early prediction model was obtained with data collected at 72 h after moderate/severe ARDS diagnosis. Multilayer Perceptron risk modeling identified major prognostic factors for the duration of MV > 14 days, including PaO2/FiO2, PaCO2, pH, and positive end-expiratory pressure. Predictions of the duration of MV > 14 days showed modest discrimination [AUC 0.71 (95%CI 0.65–0.76)]. Conclusions: Prolonged MV duration in moderate/severe ARDS patients remains difficult to predict early even with ML techniques such as Multilayer Perceptron and using data at 72 h of diagnosis. More research is needed to identify markers for predicting the length of MV. This study was registered on 14 August 2023 at ClinicalTrials.gov (NCT NCT05993377)

    Individual and combined effects of chemical and mechanical power on postoperative pulmonary complications: a secondary analysis of the REPEAT study

    Introduction: Intra-operative supplemental oxygen and mechanical ventilation expose the lungs to potentially injurious energy. This can be quantified as 'chemical power' and 'mechanical power', respectively. In this study, we sought to determine if intra-operative chemical and mechanical power, individually and/or in combination, are associated with postoperative pulmonary complications. Methods: Using an individual patient data analysis of three randomised clinical trials of intra-operative ventilation, we summarised intra-operative chemical and mechanical power using time-weighted averages. We evaluated the association between intra-operative chemical and mechanical power and a collapsed composite of postoperative pulmonary complications using multivariable logistic regression to estimate the odds ratios related to the effect of 1 J.min-1 increase in chemical or mechanical power with adjustment for demographic and intra-operative characteristics. We also included an interaction term to assess for potential synergistic effects of chemical and mechanical power on postoperative pulmonary complications. Results: Of 3837 patients recruited to three individual trials, 2492 with full datasets were included in the analysis. Intra-operative time-weighted average (SD) chemical power was 10.2 (3.9) J.min-1 and mechanical power was 10.5 (4.4) J.min-1. An increase of 1 J.min-1 in chemical power was associated with 8% higher odds of postoperative pulmonary complications (OR 1.08, 95%CI 1.05-1.10, p &lt; 0.001), while the same increase in mechanical power raised odds by 5% (OR 1.05, 95%CI 1.02-1.08, p = 0.003). We did not find evidence of a significant interaction between chemical and mechanical power (p = 0.40), suggestive of an additive rather than synergistic effect on postoperative pulmonary complications. Discussion: Both chemical and mechanical power are independently associated with postoperative pulmonary complications. Further work is required to determine causality

    The Protestant Fiscal Ethic: Religious Confession and Euro Skepticism in Germany

    During the European sovereign debt crisis, most countries that ran into fiscal trouble had Catholic majorities, whereas countries with Protestant majorities were able to avoid fiscal problems. Survey data show that, within Germany, views on theeuro differ between Protestants and Non-Protestants, too. Among Protestants, concerns about the euro have, compared to Non-Protestants, increased during the crisis, and significantly reduce their subjective wellbeing only. We use the timing of survey interviews and news events in 2011 to account for the endogeneity of euro concerns. Emphasis on moral hazard concerns in Protestant theology may, thus, still shape economic preferences
