The Echinococcus canadensis (G7) genome: A key knowledge of parasitic platyhelminth human diseases
Background: The parasite Echinococcus canadensis (G7) (phylum Platyhelminthes, class Cestoda) is one of the causative agents of echinococcosis. Echinococcosis is a worldwide chronic zoonosis affecting humans as well as domestic and wild mammals, which has been reported as a prioritized neglected disease by the World Health Organisation. No genomic data, comparative genomic analyses or efficient therapeutic and diagnostic tools are available for this severe disease. The information presented in this study will help to understand the peculiar biological characters and to design species-specific control tools. Results: We sequenced, assembled and annotated the 115-Mb genome of E. canadensis (G7). Comparative genomic analyses using whole genome data of three Echinococcus species not only confirmed the status of E. canadensis (G7) as a separate species but also demonstrated a high nucleotide sequences divergence in relation to E. granulosus (G1). The E. canadensis (G7) genome contains 11,449 genes with a core set of 881 orthologs shared among five cestode species. Comparative genomics revealed that there are more single nucleotide polymorphisms (SNPs) between E. canadensis (G7) and E. granulosus (G1) than between E. canadensis (G7) and E. multilocularis. This result was unexpected since E. canadensis (G7) and E. granulosus (G1) were considered to belong to the species complex E. granulosus sensu lato. We described SNPs in known drug targets and metabolism genes in the E. canadensis (G7) genome. Regarding gene regulation, we analysed three particular features: CpG island distribution along the three Echinococcus genomes, DNA methylation system and small RNA pathway. The results suggest the occurrence of yet unknown gene regulation mechanisms in Echinococcus. Conclusions: This is the first work that addresses Echinococcus comparative genomics. The resources presented here will promote the study of mechanisms of parasite development as well as new tools for drug discovery. The availability of a high-quality genome assembly is critical for fully exploring the biology of a pathogenic organism. The E. canadensis (G7) genome presented in this study provides a unique opportunity to address the genetic diversity among the genus Echinococcus and its particular developmental features. At present, there is no unequivocal taxonomic classification of Echinococcus species; however, the genome-wide SNPs analysis performed here revealed the phylogenetic distance among these three Echinococcus species. Association between Grape Yeast Communities and the Vineyard Ecosystems
The grape yeast biota from several wine-producing areas, with distinct soil types and grapevine training systems, was assessed on five islands of Azores Archipelago, and differences in yeast communities composition associated with the geographic origin of the grapes were explored. Fifty-seven grape samples belonging to the Vitis vinifera grapevine cultivars Verdelho dos Acores (Verdelho), Arinto da Terceira (Arinto) and Terrantez do Pico (Terrantez) were collected in two consecutive years and 40 spontaneous fermentations were achieved. A total of 1710 yeast isolates were obtained from freshly crushed grapes and 1200 from final stage of fermentations. Twenty-eight species were identified, Hanseniaspura uvarum, Pichia terricola and Metschnikowia pulcherrima being the three most representative species isolated. Candida carpophila was encountered for the first time as an inhabitant of grape or wine-associated environments. In both sampling years, a higher proportion of H. uvarum in fresh grapes from Verdelho cultivar was observed, in comparison with Arinto cultivar. Qualitatively significant differences were found among yeast communities from several locations on five islands of the Archipelago, particularly in locations with distinctive agro-ecological compositions. Our results are in agreement with the statement that grape-associated microbial biogeography is non-randomly associated with interactions of climate, soil, cultivar, and vine training systems in vineyard ecosystems. Our observations strongly support a possible linkage between grape yeast and wine typicality, reinforcing the statement that different viti-cultural terroirs harbor distinctive yeast biota, in particular in vineyards with very distinctive environmental conditions.Joao Drumonde Neves is the recipient of a fellowship of the Azorean Government (M321/006/F/2008) and PROEMPREGO. Proposição de um índice do enfrentamento governamental à violência intrafamiliar contra crianças e adolescentes
O artigo apresenta o processo de construção do Índice de Enfrentamento de Violência Intrafamiliar (IEVI), cujo objetivo é avaliar estratégias da gestão municipal em face dessa violação de direitos infantojuvenis. Sua formulação envolveu análise preliminar de indicadores de estudos anteriores e técnica grupo nominal com especialistas. Foram selecionados quatro indicadores: existência de Plano Municipal de Enfrentamento das Violências contra Crianças e Adolescentes; existência de fluxo intersetorial para atendimento e acompanhamento de crianças e adolescentes em situação de violência intrafamiliar e suas famílias; taxa de Conselhos Tutelares por habitantes do município; e existência, nas redes municipais de educação, assistência social e saúde, de instrumentos padronizados para notificação/comunicação das situações de violência contra crianças e adolescentes. Com base em banco de dados de estudo anterior, foi realizado o exercício de aplicação em quatro capitais brasileiras. O IEVI constitui uma ferramenta para monitorar e mobilizar esforços para a implantação de ações de enfrentamento da violência
A Strong Deletion Bias in Nonallelic Gene Conversion
Gene conversion is the unidirectional transfer of genetic information between orthologous (allelic) or paralogous (nonallelic) genomic segments. Though a number of studies have examined nucleotide replacements, little is known about length difference mutations produced by gene conversion. Here, we investigate insertions and deletions produced by nonallelic gene conversion in 338 Drosophila and 10,149 primate paralogs. Using a direct phylogenetic approach, we identify 179 insertions and 614 deletions in Drosophila paralogs, and 132 insertions and 455 deletions in primate paralogs. Thus, nonallelic gene conversion is strongly deletion-biased in both lineages, with almost 3.5 times as many conversion-induced deletions as insertions. In primates, the deletion bias is considerably stronger for long indels and, in both lineages, the per-site rate of gene conversion is orders of magnitudes higher than that of ordinary mutation. Due to this high rate, deletion-biased nonallelic gene conversion plays a key role in genome size evolution, leading to the cooperative shrinkage and eventual disappearance of selectively neutral paralogs
Resilience and death: the nursing professional in the care of children and adolescents with life-limiting illnesses
O presente artigo dedica-se a analisar a
resiliência da equipe de enfermagem pelo recorte
do processo de cuidar de crianças e adolescentes
com doença crônica, o que inclui lidar com sua
finitude. De aporte qualitativo, a pesquisa teve
como participantes profissionais de enfermagem
que atuam na pediatria de um hospital do Rio de
Janeiro. A produção dos dados se deu a partir da:
aplicação da escala de resiliência, devolução das
escalas em grupo e entrevista semiestruturada. Na
análise dos dados, advindos das entrevistas indi-viduais e em grupo, sobressaiu a relação entre re-siliência profissional e o gerir/cuidar do processo
de morrer de crianças e adolescentes. A assistência
à criança e ao adolescente no processo de finitude
desencadeia respostas relacionadas ao tema da re-siliência no que toca a buscar saídas que oscilam
entre respostas individuais (apoio religioso, psico-lógico) e a busca de um apoio coletivo incipiente
baseado em relações pessoais. Apontamos para a
necessidade de que se encare o tema como estraté-gico para a formação profissional em saúde, com
suporte do ambiente coletivo, assumido no seu
interior e no ambiente das práticas de gestão da
humanização no ambiente hospitalar.The purpose of this article is to analyze
the resilience of the nursing staff in providing care
for children and adolescents with chronic diseas-es, including coping with their deaths. The par-ticipants of this qualitative research were nursing
professionals working in the pediatric ward of a
hospital in the city of Rio de Janeiro, Brazil. The
data collection was obtained by applying the resil-ience scale, by returning the scales in groups, and
by semi-structured interviews. The relationship
between professional resilience and coping with
the process of children and adolescent’s deaths
stood out in the analysis based on data obtained
from group and individual interviews. The care
given to children and adolescents with life-lim-iting illnesses triggers resilience-related answers
concerning alternatives that oscillate between
individual reactions (religious and psychological
support), and the search for an incipient collec-tive support based on personal relationships. This
study points out that this subject must be strategi-cally handled to train this professional, who must
be able to rely on support from the collective envi-ronment, presumed within the professional health
care training and in the management of human-ization at the hospital
Comparative Study of rK39 Leishmania Antigen for Serodiagnosis of Visceral Leishmaniasis: Systematic Review with Meta-Analysis
Visceral Leishmaniasis (VL) is a neglected tropical disease for which serodiagnostic tests are available, but not yet widely implemented in rural areas. The rK39 recombinant protein is derived from a kinesin-like protein of parasites belonging to the Leishmania donovani complex, and has been used in the last two decades for the serodiagnosis of VL. We present here a systematic review and meta-analysis of studies evaluating serologic assays (rK39 strip-test, rK39 ELISA, Direct Agglutination Test [DAT], Indirect Immunofluorescence test [IFAT] and ELISA with a promastigote antigen preparation [p-ELISA]) to diagnose VL to determine the accuracy of rK39 antigen in comparison to the use of other antigen preparations. Fourteen papers fulfilled the inclusion and exclusion selection criteria. The summarized sensitivity for the rK39-ELISA was 92% followed by IFAT 88% and p-ELISA 87%. The summarized specificity for the three diagnostic tests was 81%, 90%, and 77%. Studies comparing the rK39 strip test with DAT found a similar sensitivity (94%) and specificity (89%). However, the rK39 strip test was more specific than the IFAT and p-ELISA. In conclusion, we found the rK39 protein used either in a strip test or in an ELISA is a good choice for the serodiagnosis of VL

Legal and health variations in drug litigation injunctions granted in Minas Gerais
OBJECTIVE To investigate the factors related to the granting of preliminary court orders [injunctions] in drug litigations. METHODS A retrospective descriptive study of drug lawsuits in the State of Minas Gerais, Southeastern Brazil, was conducted from October 1999 to 2009. The database consists of 6,112 lawsuits, out of which 6,044 had motions for injunctions and 5,167 included the requisition of drugs. Those with more than one beneficiary were excluded, which totaled 5,072 examined suits. The variables for complete, partial, and suppressed motions were treated as dependent and assessed in relation to those that were independent – lawsuits (year, type, legal representation, defendant, court in which it was filed, adjudication time), drugs (level five of the anatomical therapeutic chemical classification), and diseases (chapter of the International Classification of Diseases). Statistical analyses were performed using the Chi-square test. RESULTS Out of the 5,072 lawsuits with injunctions, 4,184 (82.5%) had the injunctions granted. Granting varied from 95.8% of the total lawsuits in 2004 to 76.9% in 2008. Where there was legal representation, granting exceeded 80.0% and in lawsuits without representation, it did not exceed 66.9%. In public civil actions (89.1%), granting was higher relative to ordinary lawsuits (82.8%) and injunctions (80.1%). Federal courts granted only 68.6% of the injunctions, while the state courts granted 84.8%. Diseases of the digestive system and neoplasms received up to 87.0% in granting, while diseases of the nervous system, mental and behavioral disorders, and diseases of the skin and subcutaneous tissue received granting below 78.6% and showed a high proportion of suspended injunctions (10.9%). Injunctions involving paroxetine, somatropin, and ferrous sulfate drugs were all granted, while less than 54.0% of those involving escitalopram, sodium diclofenac, and nortriptyline were granted. CONCLUSIONS There are significant differences in the granting of injunctions, depending on the procedural and clinical variances.
