Carpal alignment in distal radial fractures
BACKGROUND: Carpal malalignment following the malunited distal radial fracture is described to develop as an adaptation to realign the hand to the malunion. It worsens gradually after healing of the fracture due to continued loading of the wrist. It is also reported to develop during the immobilization itself rather than after fracture healing. The present work was aimed to study the natural course and the quantitative assessment of such adaptive carpal realignment following distal radial fracture. METHODS: In a prospective study, 118 distal radial fractures treated with different modalities were followed-up with serial radiographs for a year for assessment of various radiological parameters. RESULTS: Two patterns of carpal malalignment were identified depending upon the effective radio-lunate flexion (ERLF) measured on pre-reduction radiographs. The midcarpal malalignment was seen in 98 radial fractures (83%) with the lunate following the dorsiflexed fracture fragment and a measured ERLF of less than 25°. The second pattern of radio-carpal malalignment showed the fracture fragment to dorsiflex without taking the lunate with a measured ERLF of more than 25°. The scaphoid did not follow the fracture fragment in both the patterns of malalignment. CONCLUSION: It is better to assess distal radial fractures for any wrist ligamentous injury on the post-reduction film with the restored radial anatomy than on the pre-reduction film since most carpal malalignments get corrected with the reduction of the fracture. Similar carpal malalignment reappear with the redisplacement of the fracture as seen in pre-reduction radiographs and develops during the immobilization rather than as a later compensatory mechanism for the malunion
Neutrino propagation and spin zero sound in hot neutron matter with Skyrme interactions
We present microscopic calculations of neutrino propagation in hot neutron
matter above nuclear density within the framework of the Random Phase
Approximation . Calculations are performed for non- degenerate neutrinos using
various Skyrme effective interactions. We find that for densities just above
nuclear density, spin zero sound is present at zero temperature for all Skyrme
forces considered. However it disappears rapidly with increasing temperature
due to a strong Landau damping. As a result the mean-free path is given, to a
good approximation, by the mean field value. Because of the renormalization of
the bare mass in the mean field, the medium is more transparent as compared to
the free case. We find, in contrast, that at several times nuclear density, a
new type of behavior sets in due to the vicinity of a magnetic instability. It
produces a strong reduction of the mean free path. The corresponding transition
density however occurs in a region where inputs from more realistic
calculations are necessary for the construction of a reliable Skyrme type

Neuropsychological deficits associated with Complex Regional Pain Syndrome
AbstractWe sought to elucidate the existence of neuropsychological subtypes in Complex Regional Pain Syndrome (CRPS). One hundred thirty seven patients with CRPS were administered tests that assess executive control, naming/lexical retrieval, and declarative memory. A 2-step cluster analysis that does not require any a priori specification regarding the number of clusters, classified patients into three groups. Group 1 obtained scores that were in the average range on all tests (n = 48; normal CRSP group). Group 2 (n = 58; dysexecutive CRSP group) presented with mild impairment or statistically low average test performance on working memory/verbal fluency tests. Group 3 (n = 31; global CRSP group) produced scores in the statistically low average/borderline range on all tests with particularly reduced scores on naming/declarative memory tests. Between-group analyses found that the CRPS group 1 obtained higher scores than CRPS groups 2 and 3 on all tests. However, groups 2 and 3 were equally impaired on executive tests. CRPS group 3 was impaired on tests of naming/memory tests compared to the other groups. Significant neuropsychological deficits are present in 65% of patients, with many patients presenting with elements of a dysexecutive syndrome and some patients presenting with global cognitive impairment. Can carpal malalignment predict early and late instability in nonoperatively managed distal radius fractures?
The purpose of this study was to assess the frequency of carpal instability as a concomitant lesion in distal radius fractures and identify other factors including carpal malalignment in an attempt to predict the final radiological outcome more accurately following conservative treatment of distal radius fractures. An observational study from patient records and standardised radiological follow-up examinations as data sources was used. The alignment of each wrist was checked radiographically immediately post-reduction and subsequently at 1- and 6-week follow-up assessments. Serial measurements of radial length, dorsal tilt, radial inclination, scapho-lunate, radio-lunate, radioscaphoid, scapho-capitate angles and effective radiolunate flexion angle were made. Regression analysis showed high correlation among the severity of axial shortening, pre-reduction dorsal angulation and radio-carpal malalignment pattern with early loss of reduction at 1 week. We found the age, severity of axial radial shortening, dorsal angulation, presence of dorsal comminution and radio-carpal malalignment pattern to be significant predictors of adverse radiological outcome at 6 weeks (late instability). Our study highlights the importance of radio-carpal instability pattern on post-reduction radiographs as a predictor of early and late instability
