    Two years after molecular diagnosis of familial hypercholesterolemia: Majority on cholesterol-lowering treatment but a minority reaches treatment goal

    Background: The risk of premature cardiovascular disease in patients with familial hypercholesterolemia (FH) can be profoundly reduced by cholesterol-lowering therapy, and current guidelines for FH advocate ambitious low-density lipoprotein cholesterol (LDL-C) goals. In the present study, we determined whether these goals are reflected in current clinical practice once FH has been diagnosed. Methodology/Principal Findings: In 2008, we sent questionnaires to all subjects (aged 18-65 years) who were molecularly diagnosed with FH in the year 2006 through the screening program in the Netherlands. Of these 1062 subjects, 781 completed the questionnaire (46% males; mean age: 42±12 years; mean LDL-C at molecular diagnosis (baseline): 4.1±1.3 mmol/L). The number of persons that used cholesterol-lowering therapy increased from 397 (51%) at baseline to 636 (81%) after diagnosis. Mean treated LDL-C levels decreased significantly to 3.2±1.1 mmol/L two years after diagnosis. Only 22% achieved the LDL-C target level of ≤2.5 mmol/L.     Fractionation of eucalyptus globulus wood by glycerol-water pretreatment: optimization and modeling

    A glycerol-organosolv process can be a good alternative for Eucalyptus wood fractionation into its main compounds, improving the enzymatic saccharification of the cellulose. A study of process variables - glycerol−water percent content, temperature, and process time - was carried out using a Box-Behnken experimental design. The cellulose obtained from pretreated solids was recovered almost quantitatively, leading to a solid with a high percentage of cellulose (77 g/100 g of pretreated solid), low lignin content (9 g/100 g of pretreated solid), and 18% of residual hemicellulose in the solid at 200 °C, 56% of glycerol−water and 69 min. The enzymatic saccharification was enhanced achieving 98% cellulose-to-glucose conversion (under conditions: liquid to solid ratio 20 g/g and enzyme loading 20 FPU/g of solid). This study contributes to the improvement of biomass fractionation by exploring an eco-friendly treatment which allows for almost complete wood fractionation into constituents and high levels of glucose recovery available for subsequent yeast fermentation to bioethanol.The authors A.     Hydration of dicalcium silicate and diffusion through neo-formed calcium-silicate-hydrates at weathered surfaces control the long-term leaching behaviour of basic oxygen furnace (BOF) steelmaking slag

    Alkalinity generation and toxic trace metal (such as vanadium) leaching from basic oxygen furnace (BOF) steel slag particles must be properly understood and managed by pre-conditioning if beneficial reuse of slag is to be maximised. Water leaching under aerated conditions was investigated using fresh BOF slag at three different particle sizes (0.5–1.0, 2–5 and 10 × 10 × 20 mm blocks) and a 6-month pre-weathered block. There were several distinct leaching stages observed over time associated with different phases controlling the solution chemistry: (1) free-lime (CaO) dissolution (days 0–2); (2) dicalcium silicate (Ca₂SiO₄) dissolution (days 2–14) and (3) Ca–Si–H and CaCO₃ formation and subsequent dissolution (days 14–73). Experiments with the smallest size fraction resulted in the highest Ca, Si and V concentrations, highlighting the role of surface area in controlling initial leaching. After ~2 weeks, the solution Ca/Si ratio (0.7–0.9) evolved to equal those found within a Ca–Si–H phase that replaced dicalcium silicate and free-lime phases in a 30- to 150-μm altered surface region. V release was a two-stage process; initially, V was released by dicalcium silicate dissolution, but V also isomorphically substituted for Si into the neo-formed Ca–Si–H in the alteration zone. Therefore, on longer timescales, the release of V to solution was primarily controlled by considerably slower Ca–Si–H dissolution rates, which decreased the rate of V release by an order of magnitude. Overall, the results indicate that the BOF slag leaching mechanism evolves from a situation initially dominated by rapid hydration and dissolution of primary dicalcium silicate/free-lime phases, to a slow diffusion limited process controlled by the solubility of secondary Ca–Si–H and CaCO₃ phases that replace and cover more reactive primary slag phases at particle surfaces

    Conditional conservatism in reporting earnings in Brazilian public companies: differences between ADR issuing and not-issuing companies and between accounting systems

    O artigo examina as diferenças, em termos de qualidade informacional (conservadorismo condicional), das demonstrações contábeis de companhias brasileiras de capital aberto emissoras e não emissoras de ADR. Também são examinadas as demonstrações publicadas nos dois sistemas contábeis (PCAB e USGAAP). As hipóteses propõem que não há diferenças nos graus de conservadorismo condicional entre as firmas em razão das similaridades contratuais e institucionais, da inexistência de incentivos para alterar o comportamento dos gestores pelo processo de cross-listing e da insuficiência de motivação pela aplicação de princípios contábeis diversos. Os modelos de componentes transitórios nos lucros (BASU, 1997) e de associação entre apropriações contábeis e fluxos de caixa (BALL; SHIVAKUMAR, 2005) foram utilizados para os testes. Para estimar os coeficientes de associação dos dois modelos, utilizou-se análise de dados em painel. Os resultados confirmaram as hipóteses.The article examines the differences, in terms of information quality (conditional conservatism), of the accounting reports of Brazilian public companies issuing and not issuing ADR. It is also examined the demonstrations published in the two accounting systems (BRGAAP and USGAAP). The hypotheses consider that there are no differences in the degrees of conditional conservatism among the firms due to: contractual and institutional similarities, the inexistence of incentives to modify managers' behavior by the process of cross-listing, and the lack of motivation by application of different accounting principles. The models of Transitory Earnings Components (BASU, 1997) and Association between Accruals and Cash Flows (BALL; SHIVAKUMAR, 2005) were used for the tests.     A Review of the Rationale for Additional Therapeutic Interventions to Attain Lower LDL-C When Statin Therapy Is Not Enough

    Statins alone are not always adequate therapy to achieve low-density lipoprotein (LDL) goals in many patients. Many options are available either alone or in combination with statins that makes it possible to reach recommended goals in a safe and tolerable fashion for most patients. Ezetimibe and bile acid sequestrants reduce cholesterol transport to the liver and can be used in combination. Niacin is very effective at lowering LDL, beyond its ability to raise high-density lipoprotein and shift LDL particle size to a less atherogenic type. When statins cannot be tolerated at all, red yeast rice can be used if proper formulations of the product are obtained. Nutrients can also be added to the diet, including plant stanols and sterols, soy protein, almonds, and fiber, either individually or all together as a portfolio diet. A clear understanding of how each of these strategies works is essential for effective results

    Non-standard employment relations and wages among school-leavers in the Netherlands

However, if such jobs are 'bad', non-standard employment becomes a matter of concern. In addition, non-standard employment may hit the least qualified, excluding them from the primary segment of the labour market, where 'good' jobs are found. We first examine whether less-educated school leavers more often end up in a job with a non-standard employment contract than the higher educated. Then, we investigate the effect of having a non-standard employment contract on job advantages in terms of wages. The data come from three large-scale Dutch school leaver surveys as held in 2001. The results show: (a) less-educated school leavers indeed are more likely to have a non-standard contract than more highly educated ones, while (b) those in non-standard employment earn less in their jobs. A substantial part of these differences can be ascribed to the segment of the labour market in which school leavers work.23 p
