The challenge of evaluating pain and a pre-incisional local anesthetic block.
Background. Our objective was to test the effectiveness of a local anesthetic line block administered before surgery in reducing postoperative pain scores in dogs undergoing ovariohysterectomy (OVHX). Methods. This study is a prospective, randomized, blinded, clinical trial involving 59 healthy female dogs. An algometric pressure-measuring device was used to determine nociceptive threshold, and compared to three subjective pain scales. Group L/B received a line block of lidocaine (4 mg/kg) and bupivacaine (1 mg/kg) subcutaneously in the area of the incision site and saline subcutaneously as premedication; group L/BM (positive control) received a similar block and morphine (0.5 mg/kg) subcutaneously for premedication; and group SS (negative control) received a saline line block and saline premedication. Criteria for rescue analgesia were defined before the study. Dogs were assessed prior to surgery, at extubation (time 0) and at 2, 4, 6, 8 and 24 h post-recovery. The data were analyzed with one-way ANOVA, and a Split Plot Repeated Measures ANOVA with one grouping factor and one repeat factor (time). P < 0.05 was considered statistically significant. Results. Approximately 33% of dogs required rescue analgesia at some point during the study, with no significant difference between groups. There was no significant difference between treatment groups with any assessment method. Conclusions. As there were no statistically significant differences between positive and negative controls, the outcome of this technique cannot be proven
Elevated salivary C-reactive protein predicted by low cardio-respiratory fitness and being overweight in African children
INTRODUCTION: C-reactive protein (CRP) is a sensitive marker of systemic inflammation and is an independent risk factor for cardiovascular disease. The aim of the study was to examine the relationship between salivary CRP, cardio-respiratory fitness and body composition in a paediatric population. METHODS: This was a cross-sectional study of 170 black South African children (age 9.41 ± 1.55 years, 100 females, 70 males) in grades 3 to 7. Unstimulated whole saliva samples were obtained for the analysis of CRP. Height, mass, skin-fold thickness, resting blood pressure, and waist and hip circumference measurements were obtained. Cardio-respiratory fitness was assessed using a 20-m multi-stage shuttle run. Children were classified as overweight/obese according to the Center for Disease Control and Prevention (CDC) body mass index (BMI) percentile ranking, and meeting percentage body fat recommendations, if percentage body fat was ≤ 25% in boys and ≤ 32% in girls. The cut-off point for low cardio-respiratory fitness was a predicted aerobic capacity value ≤ the 50th percentile for the group. Contributions of low cardio-respiratory fitness, overweight/obesity, and not meeting percentage body fat recommendations, to elevated salivary CRP (≥ 75th percentile) concentration and secretion rate were examined using binary logistic regression analysis with a backward stepwise selection technique based on likelihood ratios. RESULTS: Poor cardio-respiratory fitness was independently associated with elevated salivary CRP concentration (OR 3.9, 95% CI: 1.7–8.9, p = 0.001). Poor cardio-respiratory fitness (OR 2.7, 95% CI: 1.2–6.1, p = 0.02) and overweight/obesity (BMI ≥ 85th percentile) (OR 2.5, 95% CI: 1.1–5.9, p = 0.03) were independent predictors of elevated salivary CRP secretion rate. CONCLUSION: The results suggest a strong association between poor cardio-respiratory fitness and/or overweight/obesity and inflammatory status in children, based on elevated salivary CRP levels
Who gets the information? Gender, power and equity considerations in the design of climate services for farmers
Central to understanding the usefulness of climate and weather forecasts in support of agricultural decision-making is addressing the issue of who receives what information. Many contend that improved climate forecasts since the late 1990s have had limited impact on smallholder farming communities in Africa and across the developing world. However, power and privilege may determine who has access to appropriate climate and advisory services within those communities.
In 2011-2012, we tested this hypothesis in three climate-vulnerable farming communities in the CGIAR Research Program on Climate Change, Agriculture and Food Security semi-arid research site of Kaffrine, Senegal. Therein, we assessed gender-specific vulnerabilities to climate-related shocks, endogenous adaptation strategies, and coping mechanisms. From the gap between vulnerability and local capacity, we deduced farmers’ climate service needs, and then assessed whether these systematically differed between distinct vulnerable sub-groups within the community – chiefly, between male and female farmers. In 2011 we introduced a seasonal climate forecast for the first time in the community, and explored perceptions of forecast access, usefulness and value, by both men and women
Beyond the Hit:Muscle and Vascular Tissue Responses to Contact Exposure in Collision Sports—A Narrative Review
Contact events in rugby codes such as tackling, running into contact, scrums, rucks, and contact with the playing surface may expose athletes to muscle damage known as impact-induced muscle damage. These repetitive impacts to muscle tissue have the potential to diminish muscle force production and delay recovery following contact-focused training and match-play. Repetitive exposure to contacts may also affect the surrounding vascular and neuronal tissues, an area that has received little attention in the collision sports. Depending on the severity and duration of tissue damage, repetitive contact exposure without sufficient recovery or noticeable adaptation may predispose collision sport athletes to impaired performance and long-term health complications. The aim of this narrative review is to provide a conceptual framework for understanding the physiological implications of contact exposure in collision sports. We examine the current understanding of impact-induced muscle damage (IIMD), how it differs from exercise-induced muscle damage (EIMD), and its relationship with contact exposure in collision sports. Drawing on both experimental animal models of contusion injury and limited human observational research, we explore the effects of repeated contact exposure on the microvasculature and its implications for both athletic performance and player welfare. To account for all tissues that may be affected by impacts, including muscle, nerve, vascular, connective tissue, skin, other organs and bones, we recommend impact-induced tissue damage (IITD) as the preferred descriptor rather than IIMD. Finally, we discuss the concept of contact adaptation and provide recommendations for future research on IITD in collision sports.</p
Pre-Exercise Whole- or Partial-Body Cryotherapy Exposure to Improve Physical Performance:A Systematic Review
Whole- (WBC) and partial-body cryotherapy (PBC) are commonly used sports medicine modalities for the treatment of injury and exercise recovery. Physiological and perceptual effects have the potential to be utilised in a novel application that involves pre-exercise WBC and PBC exposure to improve physical performance. A systematic literature search of multiple databases was conducted in July 2021 to identify and evaluate the effects of pre-exercise exposure of WBC or PBC on physical performance measures, and any potential translational effects. The following inclusion criteria were applied: (1) use of WBC or PBC exposure pre-exercise, (2) use of WBC or PBC in healthy and/or athletic populations, (3) control group was used in the data collection, and (4) investigated physiological, psychosocial or direct physical performance impacts of pre-exercise cryotherapy exposure. A total of 759 titles were identified, with twelve relevant studies satisfying the inclusion criteria after full-text screening. The twelve studies were categorised into three key areas: performance testing (n = 6), oxidative stress response (n = 4) and lysosomal enzyme activity (n = 2). The potential for eliciting favourable physical and physiological responses from pre-exercise WBC or PBC is currently unclear with a paucity of good quality research available. Health-Related Quality of Life in a National Sample of Caregivers: Findings from the Behavioral Risk Factor Surveillance System
Abstract Purpose Recent national public health agendas, such as Healthy People 2010, call for improved public health surveillance and health promotion programs for people with disabilities and their caregivers. The goal of this study was to understand the public health impact of caregiving on health-related quality of life (HRQoL) using population-level data. Design & Methods A cross-sectional study design was used. 184,450 adults surveyed during the 2000 national Behavioral Risk Factor Surveillance System survey formed the sample. Binary logistic regression models ascertained differences between caregivers and noncaregivers in reporting reduced (''fair'' or ''poor'') health. Ordinary least squares regression (OLS) and multinomial logistic regression models examined the influence of caregiving status on HRQoL, measured as categories of healthy days reported in the last 30 days and the number of days reported as physical and mental health not good in the last 30 days. Results Sixteen percent (16%) of the survey respondents were caregivers. There was an interaction effect between caregiving status and age of the caregiver. In the fully adjusted models, caregivers \55 years old had a 35% increased risk of having fair or poor health (odds ratio to non-caregivers of the same age. In the adjusted models that examined the association of caregiving and healthy days, younger caregivers similarly showed larger deficits in both mental and physical HRQoL compared to older caregivers. For example combining mental and physical days, caregivers \55 had 1.44 fewer healthy days (b = -1.44, standard error (SE) = 0.07), while caregivers 55+ had 0.55 fewer days *b = -0.55, standard error (SE) = 0.13 (compared to non-caregivers in their respective age groups). Implications With increasing population age and the projected increase in caregivers, it is important that we understand the social and public health burden of caregiving and begin to identify interventions to sustain the HRQoL of caregivers. We found that caregivers have a slight to modest decline in HRQoL compared to non-caregivers, and that caregiving affects the HRQoL of younger adults more than older adults. Further research at the population level as to the type and level of burden of caregiving is needed.

Community-Centered Responses to Ebola in Urban Liberia: The View from Below
The West African Ebola epidemic has demonstrated that the existing range of medical and epidemiological responses to emerging disease outbreaks is insufficient, especially in post-conflict contexts with exceedingly poor healthcare infrastructures. This study provides baseline information on community-based epidemic control priorities and identifies innovative local strategies for containing EVD in Liberia.In this study the authors analyzed data from the 2014 Ebola outbreak in Monrovia and Montserrado County, Liberia. The data were collected for the purposes of program design and evaluation by the World Health Organization (WHO) and the Government of Liberia (GOL), in order to identify: (1) local knowledge about EVD, (2) local responses to the outbreak, and (3) community based innovations to contain the virus. At the time of data collection, the international Ebola response had little insight into how much local Liberian communities knew about Ebola, and how communities managed the epidemic when they could not get access to care due to widespread hospital and clinic closures. Methods included 15 focus group discussions with community leaders from areas with active Ebola cases. Participants were asked about best practices and what they were currently doing to manage EVD in their respective communities, with the goal of developing conceptual models of local responses informed by local narratives. Findings reveal that communities responded to the outbreak in numerous ways that both supported and discouraged formal efforts to contain the spread of the disease. This research will inform global health policy for both this, and future, epidemic and pandemic responses
