Health is one of the main factors in human life. Community Health Centers(Puskesmas) is an important health facilities and affordable for the whole community, especially for the lower middle income economies. Puskesmas Kelurahan Sadang Serang, serving adult patients and children. Hours of operation only four hours, causing long queues ofpatients. The purpose of this study is to determine how many seats should be available toaccommodate adults and children during rush hour (peak hour) and determine the appropriatenumber of servers so that the average waiting time is shorter. Penekatan done by usingsimulation. Discrete event simulation (discrete-time simulation) is a simulation by changing thestatus of the simulation model occur at points of discrete time triggered by events. In discreteevent simulation, variables change the status if an event occurs. Meanwhile, continuoussimulation, variables change with the changing status of the time

Background: Despite the prevalence of Non-Alcoholic Fatty Liver Disease (NAFLD) and Non-Alcoholic Steatohepatitis (NASH), there was no treatment has been proven to be effective in these common diseases. Although many studies have shown that lifestyle modifications such as increasing physical activities and exercise could be effective in the treatment of these common diseases, the optimal strategy was still not determined. According to the beneficial effects of antioxidant agents in the treatment of NASH, vitamin E has been used for this purpose by some clinicians. We designed this study for assessing beneficial effects of regular physical activity on the biochemical and imaging responses in patients with NASH and comparing this with vitamin E as an accepted treatment for NASH. Materials and Methods: This study was Randomized and single-blind clinical trials were carried out in Gonbad-e Kavus through which a total of 30 consecutive patients with the ultra sonographic diagnosis of non-alcoholic steatohepatitis (NASH)were enrolled and randomized to one of the three groups: Vitamin E 800 mg/day, regular physical activity, or both. Results: In all treatment groups improvement in liver transaminases level, serum lipids and ultrasonographic grading of fatty liver occurred after three months of treatment. When these decrement was compared between the treatment groups, there was no statistically significant difference in the value of improvement between the three groups (ANOVA: p>0.5). I.e. all three interventions improved the biochemical and ultrasonographic finding of fatty liver in the same way. Both groups with regular exercise had significant mean weight loss in comparison with the vitamin E group (a mean decrease of 3.0 kg in exercise group, 5.8 kg in subjects on regular exercise plus vitamin E and 0.2 kg in vitamin E group, ANOVA: p=0.04). Conclusion: There were no significant differences between exercise and vitamin E alone or in combination regarding the reduction in the level of liver enzymes and sonographic evidences of fatty liver although both resulted in significant improvements in biochemical endpoints. This implies that physical activity could be considered as effective as vitamin E in the improvement of biochemical and ultrasonographic presentations of NASH and the addition of Vitamin E does not offer any benefits. According to the findings of this pilot study a full-powered study with a control group should be designed. © 2015, Iranian Association of Gastroenterology and Hepatology. All rights reserved

Background: E-cigarette use is rapidly increasing among US young adults, heightening their risk for vaping-related illnesses. Yet, little is known about e-cigarette use among young adult Native Hawaiians and Pacific Islanders (NHPI): an indigenous-colonized US racial group rarely described in research literature. This exploratory study provides the first known data on e-cigarette use and potential risk factors in NHPI young adults. Method: Self-report data were collected from 143 NHPI young adults (age 18-30 years) living in two large NHPI communities: Samoans in urban Los Angeles County and Marshallese in rural Arkansas. We assessed rates of e-cigarette, cigarette, alcohol, and marijuana use, and positive and negative outcome expectancies from e-cigarettes, that is expected outcomes from e-cigarette use. To identify potential risk factors for NHPI e-cigarette use, regressions explored associations between participants' current e-cigarette use with current cigarette, alcohol, and marijuana use, and e-cigarette outcome expectancies. Results: Among NHPI young adults, lifetime e-cigarette use rate was 53% and current use rate was 39%. Current rate of dual e-cigarette/cigarette, e-cigarette/alcohol, and e-cigarette/marijuana use was 38%, 35%, and 25%, respectively. In our regression models, current marijuana use and positive e-cigarette outcome expectancies were significantly associated with current e-cigarette use. Conclusions: E-cigarette use is common among NHPI young adults, exceeding rates for other at-risk racial groups. Marijuana use and positive expectations about e-cigarette use may represent potential e-cigarette use risk factors. Collectively, findings underscore the need for additional research to further explore the scope of, and risk and protective factors for, e-cigarette use in this understudied high-risk population

Analysis of determining optimal ordering cost of cover engine YA40003084 for minimizing total inventory cost. Cost is the one of important variables in a company . The level of efficiency of the company is shown from how the company organizes their company expenditure. PT. Hitachi Costruction Machinery Indonesia has increasing in company expenditure mainly at the expense of the company due to overstock inventory component Cover Engine. Economic Order Quantity is one method of determining the size of the reserve that is able to streamline the total cost of inventory. In addition, the EOQ method capable of reducing the Total Inventory Cost by Cost optimal Ordering costs in accordance with the specified reserve size. Based on the calculation result of EOQ method in PT. Hitachi Construction Machinery Indonesia determining optimal lot size of 250 components/order into 538 components/order has decreased Total Inventory Cost from Rp 24.241.553 to Rp 18.527.853 with Ordering Cost is Rp 4.238.292. Total Inventory Cost reduction could be indicated by determine the optimal Cost Ordering on each order size. Total Inventory Cost has decreased Rp 8.609.250 at 250 components/order with Ordering Cost Rp 915.099 and Rp 18.527.106 on order size is 538 components/order and Ordering Cost Rp 4.237.918. Determine the optimum Ordering Cost can be able to increasing the company efficiency in inventory expenses and become reference for the company in regulating Ordering Cost and agreement with suppliers
