Gravity as a tool to improve the hydrologic mass budget in karstic areas
Monitoring the water movements in karstic areas is a fundamental but challenging task due to the complexity of the drainage system and the difficulty in deploying a network of observations. Gravimetry offers a valid complement to classical hydrologic measurements in order to characterize such systems in which the recharge process causes temporarily accumulation of large water volumes in the voids of the epi-phreatic system. We show an innovative integration of gravimetric and hydrologic observations that constrains a hydrodynamic model of the Škocjan cave system (Slovenia). We demonstrate how the inclusion of gravity observations improves water mass budget estimates for the Škocjan area based on hydrological observations only. Finally, the detectability of water storage variations in other karstic contexts is discussed with respect to the noise performances of spring and super-conducting gravimeters
Determinants of inappropriate acute pain management in old people unable to communicate verbally in the emergency department
Objectives: Poor pain management is relevant among individuals unable to communicate verbally (UCV). Analgesia may be due to three determinants: patients' status, physician's characteristics and pain etiology. Our aim is to investigate the association between prescription of ED pain treatment and these determinants. Materials and Methods: An observational prospective study including UCV patients was conducted. Severity of pain was evaluated by ALGOPLUS Scale and a score P â¥Â 2 out of 5 on the pain scale was retained as the threshold for the presence of acute pain in elderly UCV patients. Results: Our data showed that only 31,9% of UCV patients received a pharmacological treatment. The presence of the caregiver would influence the rate of therapy administration [OR 6,19 (95% CI 2,6â14,75)]. The presence of leg pain [OR 0,32 (95% CI 0,12â0,86)] and head pain [OR 0,29 (95% CI 0,10â0,84)] were less likely associated to receive analgesia. Pain related to trauma [OR 4.82 (95% CI 1.17 to 19.78)] and youngest physicians [OR 1.08 (95% CI 1.001 to 1.18)] were variables associated with the administration of drugs opiates. Discussion: Older UCV patients presenting to the ED with pain are at high risk of inadequate analgesia. Providers should always suspect presence of pain and an increasing need for behavioural pain evaluation is necessary for a complete assessment. Conclusions: Presence of a caregiver influences a more appropriate pain management in these patients. Staff training on pain management could result in better assessment, treatment, and interaction with caregivers. Keywords: Emergency department, Pain, Oligoanalgesi
Development and validation of a simplified probability assessment score integrated with age-adjusted d-dimer for diagnosis of acute aortic syndromes
Coordination Chemistry and Sensing Properties Towards Anions and Metal Ions of a Simple Fluorescent Urea
The coordination and sensing properties towards anions and transition metal ions of the simple novel fluorescent urea 1-(2-aminophenyl)-3-(naphthalen-1)-yl)urea (L) were investigated in solution, and in the solid state. An electron donating amine group in the molecular skeleton of L decreased the acidity of the urea NHs that are usually deprotonated by basic anions and allowed for a good degree of affinity towards fluoride in DMSO-d6-0.5 %H2O. Moreover, the amine moiety acted as a further binding group for metal ions. Indeed, L was able to bind Zn2+ both in solution and in the solid state, and to respond to the presence of this metal ion in MeCN with an enhancement of the fluorescence emission. Although solution studies evidenced the formation of a 1 : 1 complex of L with Zn2+, complexes with a 2 : 1 ligand-to-metal stoichiometry were isolated in the solid state.
