    Determinant criteria for designing Health benefit package in selected countries

    Health benefit package described as primary health interventions that provided with government using general funds for all regardless their financial ability. This study was aimed at determine appropriate pattern for Iran using comparative survey of Health benefit package in various countries. A review exploration was done, scholars was selected population of both developed and developing countries, required information was also extracted by articles, searches and reports of reliable sources and date were analyzed by SPSS, in brief. The vast majority frequencies was respectively allocated to accessibility (40.7%), cost- effectiveness (29.6%), prioritize, efficacy and cost (22.2%). most countries located in WHO African region were selected cost-effectiveness and accessibility, WHO southeast Asia region were selected, coverage, prioritize, efficacy and quality and finally most WHO Europeans region were elected effectiveness and services costs for including services in Health benefit package. According to most Health benefit package designer emphasis on criteria including accessibility and costeffectiveness, to design Health benefit package for Iran, these criteria must be noticed

    Extreme context exposure and counterproductive work behaviour: The role of exhaustion, authentic leadership, and spirituality

    In recent years, research on extreme contexts has expanded significantly, focusing on management practices in unusual or atypical work environments. However, individual behavioural responses in these settings have received less attention. Our study addresses this gap by using Job-Demand Resources (JD-R) theory to explore how extreme context exposure (i.e., frequent exposure to extreme events) influences counterproductive work behaviour towards the organisation (CWB-O) in high-risk contexts of aviation and shipping. In Study 1, based on data from 297 flight cadre, we found that extreme context exposure is positively linked to CWB-O, with exhaustion linking the two variables. Study 2 replicated these findings with data from 309 seafarers, reinforcing the robustness of our results. Additionally, we identified authentic leadership and spirituality as key job and personal resources that moderate this relationship, reducing the impact of extreme context exposure on CWB-O. We conclude by discussing the theoretical and practical implications of our findings, emphasising the importance of addressing individual behaviours in extreme work environments

    Association between nucleotide mutation of eNOS gene and serum level of vessel expansion factor (VEF) in non-smoker patients with vascular heart disease

    Various mutation on endothelial nitric oxide synthase (eNOs) gene cause reduced production of NO, the expansion factor (VEF) and may accelerate the process of atherosclerosis. The study was designed to investigate the frequency of T-786C polymorphism of the gene or nucleotide mutation of eNOS gene in patients suffering from vascular heart disease (VHD) or coronary artery disease (CAD) in North West of Iran. 120 subjects including 60 patients with angiographically diagnosed CAD and 60 age and sex matched CAD-free subjects as control were studied. The levels of Nitric oxide in the samples were measured with the Griess method. The genotype studies were carried out using allele specific PCR. Comparing with the control reduced levels of NO were noticed in the patient group (P<0.05) and significantly high frequency of eNOs -786C genotype was found in CAD patients (P<0.05).     Simvastatin attenuates intestinal ischaemia/reperfusion-induced injury in rat

    Ischaemia/reperfusion (I/R) injury is commonly seen in the field of intestine surgical interventions, shock, trauma, and many other clinical conditions. Simvastatin is known to have antioxidant and anti-inflammatory properties. This study investigated the effect of simvastatin administration in a warm intestinal I/R model on TNF-&#945;, antioxidant enzymes and intestinal tissue morphology. Thirty-six male wistar rats underwent laparotomy under general anaesthesia. Simvastatin was administered from four days before ischaemia induction. The rats were divided in to three groups (n = 12): the sham goup, the I/R group, and the I/R + simvastatin group. Intestinal ischaemia was induced by superior mesenteric artery ligation with microvascular clamps for 60 minutes, and after ischaemia, blood perfusion was released into the tissue and a reperfusion phase was started, which lasted for 3 hours. After 3 hours, the animals were sacrificed and serum and tissue obtained for biochemical and histological study. In the simvastatin treated group, intestinal tissue injury, TNF-&#945; level, and tissue malondealdehyde levels were significantly lower than in the I/R group (p < 0.05). Glutathion peroxidase and superoxide dismutase levels were significantly higher in the simvastatin treated group than in the I/R group (p < 0.05). Simvastatin pretreatment reduced intestinal I/R injury and was associated with down- -regulation of serum TNF-&#945; and tissue malondealdehyde level, and simvastatin administration maintained cellular antioxidant enzyme contents compared to the I/R group after 3 hours reperfusion time

    Characterization of the inflammatory cell infiltrate and expression of costimulatory molecules in chronic echinococcus granulosus infection of the human liver

    Background: The local immune responses to chronic echinococcal infections in various organs are largely unknown. Since the liver is the most frequently involved organ in such infections in human we aimed to characterize the inflammatory as well as immune cell infiltrate around hydatid cysts in the liver and compared to common inflammatory processes of the liver. Method: Surgical samples from the liver of 21 cystic echinococcosis (CE) patients were studied and the distribution of different types of inflammatory and immune cells were determined by immunohistochemistry. Furthermore, expression levels of costimulatory CTLA4, CD28, CD80 and CD86 molecules were measured at RNA level by PCR. Liver biopsy samples from patients with steatohepatitis (SH, n = 11) and chronic hepatitis (CH, n = 11) were used as non-inflammatory and chronic inflammatory controls, respectively. The composition and density of the inflammatory and immune cell infiltrates have been compared by using morphometry. Results: CD3+ T cells predominated the inflammatory infiltrate in all pathological processes, while in CE samples CD20+ B cells, in CH samples CD68+ macrophages were also frequent. Both myeloperoxidase (MPO) + leukocytes and CD68+ macrophages were found to be significantly decreased in CE as compared to either SH or CH samples. Concerning T cell subtypes, only CD8+ T cells were found to be significantly decreased in SH samples. CD1a + dendritic cells were almost completely missing from CE biopsies unlike in any other sample types. There were no differences detected in the mRNA expression of costimulatory molecules except decreased expression of CD28 in CE samples. Conclusion: In the hydatid lesions of the liver of chronic echinococcal infections T cell-mediated immunity seems to be impaired as compared to other types of chronic inflammatory processes, suggesting an immunosuppressive role for Echinococcus granulosus, which deserve further attentions

    Microscale geometrical modulation of PIEZO1 mediated mechanosensing through cytoskeletal redistribution.

    The microgeometry of the cellular microenvironment profoundly impacts cellular behaviors, yet the link between it and the ubiquitously expressed mechanosensitive ion channel PIEZO1 remains unclear. Herein, we describe a fluorescent micropipette aspiration assay that allows for simultaneous visualization of intracellular calcium dynamics and cytoskeletal architecture in real-time, under varied micropipette geometries. By integrating elastic shell finite element analysis with fluorescent lifetime imaging microscopy and employing PIEZO1-specific transgenic red blood cells and HEK cell lines, we demonstrate a direct correlation between the microscale geometry of aspiration and PIEZO1-mediated calcium signaling. We reveal that increased micropipette tip angles and physical constrictions lead to a significant reorganization of F-actin, accumulation at the aspirated cell neck, and subsequently amplify the tension stress at the dome of the cell to induce more PIEZO1's activity. Disruption of the F-actin network or inhibition of its mobility leads to a notable decline in PIEZO1 mediated calcium influx, underscoring its critical role in cellular mechanosensing amidst geometrical constraints

    Analytical model for the calculation of lateral velocity distributions in potential cross-sections

    [EN] The hydraulic modeling of water depth and flow velocities in open channel flows that were fitted by power-law cross-section stand out for their versatility, allowing their use in numerous practical applications, both in natural and artificial channels. The determination of the hydraulic variables of depth and average velocity has been widely studied in potential cross-sections; however, the variation seen in these variables along the cross-section was not found in the literature. Knowledge of this variation allows the development of studies (e.g. to know the approximate damage in different areas of the cross-section, to analyse sediment transport, or other applications in river hydraulics). This paper presents a methodology which allows calculation of the hydraulic variables in any area of a power-law cross-section. The methodology is applied to symmetrical cross-sections, comparing its generated results with the obtained values by different computational hydraulic codes, which are thoroughly accepted by scientific community, such as CES, HEC-RAS and IBER. The obtained predictions of hydraulic parameters (using the explicit formulation described in this research) present very low errors when compared with results of other models, with great computational cost. These errors reach a root mean square error (RMSE) of 0.13 and 0.05 in the determination of velocities' lateral distribution and the ratio between velocity and average velocity. These values indicate a very successful validation for the analysed symmetrical sections.[ES] La modelización hidráulica de calados y velocidades de flujo, en cauces con secciones que admiten una representación de tipo potencial, se destaca por su versatilidad, permitiendo su utilización en numerosas aplicaciones prácticas tanto en canales naturales como artificiales. El cálculo de las variables hidráulicas (calado y velocidad media) ha sido ampliamente estudiado para este tipo de secciones. Sin embargo, en la literatura técnica no se han encontrado estudios que muestren la variación de estas magnitudes a lo largo de la sección transversal. El conocimiento de esta variación permite desarrollar estudios (ejemplo: conocer de manera aproximada los daños en diferentes zonas de la sección, analizar el transporte de sedimentos, estudiar los procesos de erosión u otras aplicaciones en hidráulica fluvial). Presentamos una metodología que permite el cálculo de las variables hidráulicas en cualquier zona de una sección tipo potencial. La metodología es aplicada a secciones simétricas, comparando los resultados generados con los obtenidos por diferentes códigos hidráulicos computacionales ampliamente aceptados por la comunidad científica (p-e- CES, HECRAS e IBER). Las predicciones de los parámetros hidráulicos obtenidas (usando la formulación explícita descrita en este artículo) presentan errores muy bajos, en comparación con otros modelos con mayor costo computacional. Estos errores alcanzan un valor promedio para la raíz del error cuadrático medio (RMSE) en el cálculo de la distribución lateral de velocidades de 0.13 y de 0.05, en el cálculo de la relación de velocidades respecto a la velocidad media.
