Comment on “early efficacy of intra-articular HYADD® 4 (Hymovis®) injections for symptomatic knee osteoarthritis”
We read with great interest the study by Priano titled“Early
efficacy of intra-articular HYADD® 4 (Hymovis®) injections
for symptomatic knee osteoarthritis.”

The author would like
to explore the efficacy of intra-articular HYADD 4 (Hymovis)
injections for symptomatic knee osteoarthritis. Results from
this study are very interesting and promising from a clinical
aspect; however, we believe that studying patient’sclinical
status with visual analog scale and Western Ontario and
McMaster Universities Arthritis Index scale should be supported by biomechanical information. From this point of
view, to have more data that could influence the clinical
practice, it is important to note the possible action that intraarticular injections of different kinds of hyaluronic acid could
have on walking biomechanics using an objective measurement tool as gait analysis. In our opinion, the work by Priano1
is promising because it investigates the efficacy of a new
formulation of hyaluronic acid. Nowadays, many hyaluronic
acid formulations are approved for clinical use in Europe and
the United States. Furthermore, hyaluronic acid injections’
efficacy has been demonstrated also in hip osteoarthritis.

However, even if these formulations differ in their chemical–
physical properties, joint space half-life, rheological properties, and clinical efficacy, there are few studies that investigate hyaluronic acid’s possible action from a biomechanical
point of view.

From this point of view, we believe that
osteoarthritis management and rehabilitation should be
prescribed after an objective analysis of functional walking
alterations using gait analysis instrumentations. The use
of gait analysis should be desirable during diagnosis and
follow-up. In fact, it is capable to identify different walking
patterns in patient with osteoarthritis of the lower limbs,
whereas the radiology can evaluate the status of the joint’s
structures
Assessing the cervical range of motion in infants with positional plagiocephaly
Purpose: To determine if infants with positional plagiocephaly
have limitations of active and passive cervical range of motion
measured with simple and reliable methods.
Methods: The examiners assessed bilateral active and passive
cervical rotations and passive cervical lateral flexion. Cervical
assessment was performed twice by 2 different physicians to
assess intertester reliability. To assess intratester reliability the
first investigator performed a second examination 48 hours after
the first one.
Results: One-hundred nine subjects were analyzed; 70.7% of the
sample had head positional preference on the right, while 29.3% had
head positional preference on the left (x2 35.52, P <0.001).
Cervical rotations and lateral flexion showed reliable levels of
agreement for intra and intertester reliability.
Conclusions: The most limited range of motion in infants with
positional plagiocephaly was cervical active rotation which affected
more than 90% of patients. Passive cervical rotations and lateral
flexion were limited in more than 60% of patient

OBJECTIVE: The aim of the study was to identify the main aspects concerning appropriateness and efficacy of Spa therapy for musculoskeletal pathologies.
METHODS: A committee of 8 experts from Italian universities, public hospitals, territorial services, research institutes and patient associations was set up. Clinicians from Italian medical centers specialized in rheumatology, rehabilitation and thermal medicine took part in a Delphi process aimed at obtaining consensus statements among the participants.
RESULTS: Large consensus was obtained for statements grouped under the following main themes: treatment indications; choice of treatment modality and treatment efficacy.
CONCLUSIONS: The experts developed a number of consensus statements which may be used as a practical reference to guide the choice of physicians to treat musculoskeletal diseases with Spa therapy

Efficacy and Accuracy of Ultrasound Guided Injections in the Treatment of Cervical Facet Joint Syndrome: A Systematic Review
Background/Objectives: Cervical facet joint syndrome (CFJS) is a frequent cause of neck pain and motor disability. Among the available therapies for CFJS, ultrasound (US)-guided injections are becoming more and more widespread, but the evidence about their accuracy and effectiveness is still debated in the scientific literature. The aim of this systematic review is to assess efficacy, accuracy and feasibility of US-guided cervical facet injections for the related chronic neck pain treatment. Methods: This review was conducted following the preferred reporting items for systematic reviews and meta-analysis 2020 (PRISMA) statement guidelines. The scientific articles were identified through the PubMed, Google Scholar and Cochrane Library databases. Qualitative assessment of the selected studies was carried out using the modified Oxford quality scoring system. Nine studies with a total of 958 patients were included in this review. The risk of bias was assessed using the Cochrane Collaboration tool. The protocol was registered at PROSPERO 2024 (n°CRD42024512214). Results: The results of this review suggest that the US-guided cervical facet injection for CFJS treatment is an effective technique in terms of accuracy (using the lateral technique it ranges from 92% to 98%), and efficiency (it grants pain relief with a decrease in the procedure time and fewer needle passes in comparison with the X-ray-guided technique, which also involves radiation exposure). Conclusions: US-guided injections are a safe and effective method to treat this musculoskeletal disease, granting a high functional recovery and long-lasting pain relief, net of the used drugs. However, these procedures are strictly operator-dependent and require important training to acquire good expertise

Low Back Pain in Italian Nurses: A Statistical Analysis of Disability and Work Productivity Impairment—An Observational Study
Low back pain (LBP) is a common occupational health issue among
nurses, significantly affecting quality of life and work productivity. Despite awareness, it
remains a major cause of absenteeism and presenteeism, highlighting the need for targeted
interventions. This study aimed to assess the prevalence of LBP among Italian nurses and
its impact on quality of life and work productivity. Methods: A cross-sectional, multicenter
observational study was conducted from May to October 2024 using an online questionnaire
distributed to members of the Provincial Orders of Nursing Professions across Italy. The
questionnaire included sociodemographic variables and three validated instruments: the
Quebec Back Pain Disability Scale (QBPDS), the Oswestry Disability Index (ODI), and the
Work Productivity and Activity Impairment Questionnaire (WPAI). Results: A total of
318 nurses participated, with the majority from Southern Italy (57.1%) and female (74.6%).
LBP was reported by 57.5% of respondents. Nurses working in Critical Care and those
with 30–40 years of experience had significantly higher QBPDS and ODI scores, indicating
moderate disability. Nurses working 12 h shifts and those with job restrictions or medical
prescriptions reported significantly higher disability levels (ODI > 29, p < 0.001). Nurses on
pharmacological therapy reported moderate pain levels, while those engaging in regular
physical activity had significantly lower pain symptoms (<20, p < 0.001). The WPAI results
showed that 67.0% of nurses reported impaired work productivity due to LBP. Conclusions:
LBP is extremely prevalent among Italian nurses, especially affecting physical well-being and, accordingly, the health care quality provided by them. Factors exacerbating this
problem are wrong manual handling of loads, not exercising, poor nutrition, and smoking,
as well as wrong posture. Fundamental in order to avoid the occurrence of this problem
are preventive programs and ergonomic training

OBJECTIVE: The aim of the study was to identify the main aspects involved in patient selection, the choice of therapeutic agents and the safety profile, as well as the medico-legal and organizational aspects of intra-articular injection therapies for osteoarthritis.METHODS: A committee of 10 experts from Italian universities, public hospitals, territorial services, research institutes and patient associations was set up. Fifty-two clinicians from a large number of Italian medical centers specialized in intra-articular injection therapy took part in a Delphi process aimed at obtaining consensus statements among the participants.RESULTS: Large consensus was obtained for statements grouped under the following main themes: treatment indications; drug/medical device choice; treatment efficacy; and appropriate setting.CONCLUSIONS: The consensus statements developed by a large number of experts may be used as a practical reference tool to help physicians treat osteoarthritis patients by means of intra-articular injection therapies
