Skin Lesion Analyser: An Efficient Seven-Way Multi-Class Skin Cancer Classification Using MobileNet
Skin cancer, a major form of cancer, is a critical public health problem with
123,000 newly diagnosed melanoma cases and between 2 and 3 million non-melanoma
cases worldwide each year. The leading cause of skin cancer is high exposure of
skin cells to UV radiation, which can damage the DNA inside skin cells leading
to uncontrolled growth of skin cells. Skin cancer is primarily diagnosed
visually employing clinical screening, a biopsy, dermoscopic analysis, and
histopathological examination. It has been demonstrated that the dermoscopic
analysis in the hands of inexperienced dermatologists may cause a reduction in
diagnostic accuracy. Early detection and screening of skin cancer have the
potential to reduce mortality and morbidity. Previous studies have shown Deep
Learning ability to perform better than human experts in several visual
recognition tasks. In this paper, we propose an efficient seven-way automated
multi-class skin cancer classification system having performance comparable
with expert dermatologists. We used a pretrained MobileNet model to train over
HAM10000 dataset using transfer learning. The model classifies skin lesion
image with a categorical accuracy of 83.1 percent, top2 accuracy of 91.36
percent and top3 accuracy of 95.34 percent. The weighted average of precision,
recall, and f1-score were found to be 0.89, 0.83, and 0.83 respectively. The
model has been deployed as a web application for public use at
(https://saketchaturvedi.github.io). This fast, expansible method holds the
potential for substantial clinical impact, including broadening the scope of
primary care practice and augmenting clinical decision-making for dermatology

The Impact of Bariatric Surgery on Type 2 Diabetes Mellitus and the Management of Hypoglycemic Events
Recent studies discussed the benefit of bariatric surgery on obese patients diagnosed with type 2 diabetes mellitus (T2DM). Several factors play an essential role in predicting the impact of bariatric surgery on T2DM, such as ABCD score (age, BMI, C-peptide, and duration of the disease), HbA1c, and fasting blood glucose, incretins [glucagon-like peptide-1 (GLP-1) and gastric inhibitory peptide (GIP)]. DiaRem score known to include factors such as age, HbA1c, medication, and insulin usage used to predict the remission of T2DM, but it has some limitations. An extensive literature search was conducted on PubMed and Google Scholar using keywords such as gastric bypass, T2DM, bariatric surgery, GLP-1, GIP, and post bariatric hypoglycemia. Restrictive-malabsorptive procedures are most effective in treating T2DM patients based on changes induced in appetite through regulation of gastrointestinal hormones, with decreased hunger and increased satiation. We provide a review of bariatric surgery influence on T2DM and management of post-intervention hypoglycemic events. Post-bariatric surgery hypoglycemia is a serious complication especially when patients develop life-threatening neuroglycopenia with loss of consciousness and seizure. The avoidance of this adverse event may be achieved by strict dietary modification including a restriction on carbohydrates as well as foods with high glycemic index. Further research will provide more information on post-bariatric surgery hyperinsulinemic hypoglycemia pathophysiology and management

Single Pilot Workload Management During Cruise in Entry Level Jets
Advanced technologies and automation are important facilitators of single pilot operations, but they also contribute to the workload management challenges faced by the pilot. We examined task completion, workload management, and automation use in an entry level jet (ELJ) flown by single pilots. Thirteen certificated Cessna Citation Mustang (C510-S) pilots flew an instrument flight rules (IFR) experimental flight in a Cessna Citation Mustang simulator. At one point participants had to descend to meet a crossing restriction prior to a waypoint and prepare for an instrument approach into an un-towered field while facilitating communication from a lost pilot who was flying too low for ATC to hear. Four participants experienced some sort of difficulty with regard to meeting the crossing restriction and almost half (n=6) had problems associated with the instrument approach. Additional errors were also observed including eight participants landing at the airport with an incorrect altimeter setting

thesisAs the physiological effects of smoking cigarettes constitute major health-related disease, researchers must continue to investigate and evaluate programs that attempt to decrease the adaptation of the habit. The school-based education program was conducted in a local Salt Lake City high school with a known higher incidence of smoking behavior. The intervention program was designed specifically for the adolescent with two main objectives: 1. To demonstrate the immediate negative physiological effects of smoking, and 2. To both increase awareness of the social pressures present that encourage adaptation of the smoking behavior and ways to more effectively cope with these pressures. Using the Health Belief Model as the conceptual framework, it was hypothesized that by increasing perceptions of seriousness and susceptibility to the health hazards associated with smoking, the adolescent would decide to take recommended health promotion action, not to adopt the cigarette smoking habit, and/or alter current smoking behavior. This study was divided into two equally important parts. Part I investigated the relationships between the Health Belief Model and the adolescent's smoking-related attitudes and behavior while Part II evaluated the effectiveness of an intervention program on the participant's health beliefs, smoking-related attitudes and smoking behavior. Implementing a quasiexperimental design, pre- and posttesting were administered to both the control and experimental groups. In Part I, relationships were analyzed using a Spearman Rho correlation analysis to determine association between four variables: 2) demographic data, b) perceived susceptibility, c) perceived seriousness, and d) self-esteem, as well as smoking-related attitudes and behaviors. In Part II, using an analysis of variance (ANOVA), the efficacy of the intervention upon the subjects' health beliefs (perceived seriousness and perceived susceptibility), smoking-related attitude and smoking behaviors were measured. The finding as related to Part I revealed two demographic items of importance. Significant associations between employment status and grade in school were found. Both variables correlated positively with an increase in smoking behavior. Items measuring perceived susceptibility and perceived seriousness tended to correlate together especially with reference to the physiological effects of smoking in relationship to the adolescent and his/her environment. Also, lower scores of self-esteem correlated with a higher incidence of smoking behavior. The F-statistic was found to be insignificant with respect to the significance of the intervention. Although the findings (with respect to the intervention) did not reveal statistical significance, there is clinical meaningfulness. Continued research is clearly indicated in an effort to develop smoking education programs, especially designed to meet the unique needs of the adolescent. The adaptation of the Health Belief Model as a useful clinical tool in the development of such a programs is evident
Genetic structure of First Nation communities in the Pacific Northwest
This study presents genetic data for nine Native American populations from northern North America. Analyses of genetic variation focus on the Pacific Northwest (PNW). Using mitochondrial, Y chromosomal and autosomal DNA variants, we aim to more closely address the relationships of geography and language with present genetic diversity among the regional PNW Native American populations. Patterns of genetic diversity exhibited by the three genetic systems were consistent with our hypotheses, in that we expected genetic variation to be more strongly explained by geographic proximity than linguistic structure. Our findings were corroborated through a variety on analytic approaches, with the unrooted trees for the three genetic systems consistently separating inland from coastal PNW populations. Furthermore, the AMOVA tests support the trends exhibited by the unrooted trees, with geographic partitioning of PNW populations (FCT = 19.43%, p = 0.010 ± 0.009) accounting for over twice as much of the observed genetic variation compared with linguistic partitioning of the same populations (FCT = 9.15%, p = 0.193 ± 0.013). These findings demonstrate a consensus with previous PNW population studies examining the relationships of genome-wide variation, mitochondrial haplogroup frequencies, and skeletal morphology with geography and language
An analysis of the relevance of off-balance sheet items in explaining productivity change in European banking
The 1990s have witnessed a significant growth in bank income generated through non-traditional activities, especially for large EU universal banking institutions. Using the non-parametric Malmquist methodology this study analyses the impact of the inclusion of off-balance sheet (OBS) business in the definition of banks' output when estimating total factor productivity change indexes. Whereas the results reinforce the prevalent view in the recent literature, indicating that the exclusion of non-traditional activities leads to a misspecification of banks' output, the impact of the inclusion of these activities varies. Overall, the inclusion of OBS items results in an increase in estimated productivity levels for all countries under study. However, the impact seems to be the biggest on technological change rather than efficiency change. © 2005 Taylor & Francis
In search of the authentic nation: landscape and national identity in Canada and Switzerland
While the study of nationalism and national identity has flourished in the last decade, little attention has been devoted to the conditions under which natural environments acquire significance in definitions of nationhood. This article examines the identity-forming role of landscape depictions in two polyethnic nation-states: Canada and Switzerland. Two types of geographical national identity are identified. The first – what we call the ‘nationalisation of nature’– portrays zarticular landscapes as expressions of national authenticity. The second pattern – what we refer to as the ‘naturalisation of the nation’– rests upon a notion of geographical determinism that depicts specific landscapes as forces capable of determining national identity. The authors offer two reasons why the second pattern came to prevail in the cases under consideration: (1) the affinity between wild landscape and the Romantic ideal of pure, rugged nature, and (2) a divergence between the nationalist ideal of ethnic homogeneity and the polyethnic composition of the two societies under consideration
The epidemic of extended-spectrum-beta-lactamase-producing Escherichia coli ST131 is driven by a single highly pathogenic subclone, H30-Rx
The Escherichia coli sequence type 131 (ST131) clone is notorious for extraintestinal infections, fluoroquinolone resistance, and extended-spectrum beta-lactamase (ESBL) production, attributable to a CTX-M-15-encoding mobile element. Here, we applied pulsed-field gel electrophoresis (PFGE) and whole-genome sequencing to reconstruct the evolutionary history of the ST131 clone. PFGE-based cluster analyses suggested that both fluoroquinolone resistance and ESBL production had been acquired by multiple ST131 sublineages through independent genetic events. In contrast, the more robust whole-genome-sequence-based phylogenomic analysis revealed that fluoroquinolone resistance was confined almost entirely to a single, rapidly expanding ST131 subclone, designated H30-R. Strikingly, 91% of the CTX-M-15-producing isolates also belonged to a single, well-defined clade nested within H30-R, which was named H30-Rx due to its more extensive resistance. Despite its tight clonal relationship with H30Rx, the CTX-M-15 mobile element was inserted variably in plasmid and chromosomal locations within the H30-Rx genome. Screening of a large collection of recent clinical E. coli isolates both confirmed the global clonal expansion of H30-Rx and revealed its disproportionate association with sepsis (relative risk, 7.5; P < 0.001). Together, these results suggest that the high prevalence of CTX-M-15 production among ST131 isolates is due primarily to the expansion of a single, highly virulent subclone, H30-Rx. IMPORTANCE We applied an advanced genomic approach to study the recent evolutionary history of one of the most important Escherichia coli strains in circulation today. This strain, called sequence type 131 (ST131), causes multidrug-resistant bladder, kidney, and bloodstream infections around the world. The rising prevalence of antibiotic resistance in E. coli is making these infections more difficult to treat and is leading to increased mortality. Past studies suggested that many different ST131 strains gained resistance to extended-spectrum cephalosporins independently. In contrast, our research indicates that most extended-spectrum-cephalosporin-resistant ST131 strains belong to a single highly pathogenic subclone, called H30-Rx. The clonal nature of H30-Rx may provide opportunities for vaccine or transmission prevention-based control strategies, which could gain importance as H30-Rx and other extraintestinal pathogenic E. coli subclones become resistant to our best antibiotics
