Introduction: Dry eye is a multifactorial ocular surface disease characterized by symptoms of discomfort, irritation, and visual disturbance. The symptoms of dry eye can be correlated to Shushkakshipaka which is one among the Sarvagata Netra Rogas affecting all parts of the eye as explained in classical Ayurveda texts. Here the patient presenting with signs and symptoms of dry eye disease was treated according to the treatment principles of Shushkakshipaka as explained in Ayurvedic classics. Yashtimadhu Ksheerapaka Tarpana was selected for the treatment and the same has been presented in the article. Materials and Methods: The subject who approached Shalakya Tantra OPD of Sri Jayachamarajendra Institute of Indian Medicine Hospital, Bengaluru, with signs and symptoms of dry eye disease like dryness in both the eyes, foreign body sensation, burning sensation and blurred vision etc. was thoroughly examined and systematically reviewed and treatment was planned based on the Chikitsasutra of Shushkakshipaka. Result: The patient showed considerable improvement subjectively and objectively. Improvement in schirmers reading, visual acuity improvement was observed in both eyes. Discussion: The maintenance of stability of tear film is the main challenge in the present scenario. This can be achieved by properly following the treatment principle and protocol during and after the treatment for the prescribed period

Autoregressive Modeling of Visual Evoked Potentials and Its Applications to Optic Nerve Diseases-Ischemic Optic Neuropathy and Optic Neuritis

    Get PDF
It is important to differentiate the diagnosis of ischemic optic neuropathy (ION) and optic neuritis (ON) for prognostic and therapeutic reasons. In most cases, differentiation is accomplished by assessing the disc appearance, the presence or absence of retrobulbar pain, the age of the patient, the mode of onset and other features of clinical and laboratory evaluation. However, in certain groups of patients, diagnosis may be difficult because of overlapping clinical profiles in these two disorders. In this paper, an attempt is made to overcome clinically overlapping profiles and to evolve indices to classify and delineate clearly ION and ON groups by differential diagnosis of the visual evoked potentials (VEP) using autoregressive (AR) modeling. In the present work of AR modeling, the data sequence x(n) as the output of a linear system has been carried out using digitized VEP waveform. An appropriate optimal order p for the AR model is chosen based on the Akaike information criterion (AIC). Accordingly, AR model has eight coefficients for each data sequence. These AR model coefficients are computed using Burg’s algorithm. These AR coefficients with different combinations were plotted in the feature plane representations, for distinction between the ION and ON group of patients. It was found that, the feature plane plot of a2 verses a7 has a potential to distinguish clearly the ION and ON patients with respect to normal subjects. This novel technique using the AR feature plane representation is more efficient and thus, enables the neurologist in early therapy planning

    Get PDF
INTRODUCTION: Coronary artery disease (CAD) is a major cause of morbidity and mortality in Indian population presenting to EMD with acute coronary syndrome (ACS). TyG index which is a new parameter assessed using fasting plasma glucose and Triglyceride levels is known to predict severity of insulin resistance. This study aims at validating the hypothesis that increase in TyG index is associated with increased risk of multivessel disease.METHODS: This study was conducted in cardiology department in a tertiary care hospital KIMS in Hubballi, Karnataka, over a period of 1 year (2023-24). Triglyceride glucose index was calculated with formula: Log [Fasting triglyceride (mg/dl) X fasting plasma glucose (mg/dl) /2]. Further the severity of CAD was calculated using SYNTAX and GENSINI score which was correlated with TyG index.RESULTS: We have recruited 109 participants with CVD. The mean (SD) TyG index was 5.4 [median (IQR) -5.5 (5.0-5.8]. There was a positive correlation between TyG index and Gensini score (correlation coefficient - 0.33) similarly, a positive correlation was observed between TyG index and SYNTAX score (correlation coefficient - 0.37). The mean (SD) TyG index score was 5.1 (0.32) in SVD group and 5.6 (0.46) in participants with multi vessel diseases (p <0.001). For a TyG index cut-off of 5.4 we observed sensitivity of 80%, specificity of 72.7% with significant AUC of 0.83. CONCLUSION: Our study demonstrated that increased TyG index is associated with increased risk of multi vessel CAD. The results further demonstrated that higher TyG index is associated with more severe CAD

The first and foremost prime aim of Ayurveda is to preservation of positive health in a healthy person.[1] The Sutrasthana lies as the Madhusancaya of the Samhita. This nectar of knowledge has been stored by Acharya Charaka in very beautiful way i.e., Chatushka methodology. The Swastha Chatushka implying Swasthya of Swastha includes the four chapters (1)Matrashiteeya, (2)Tasyashiteeya, (3)Navegandharaneeya, (4)Indriyopakramaneeya, those which deals with different levels of maintenance of health. The Swastha Chatushka stands in 2nd position among the 7 Chatushkas, but it solely elucidates about Swasthya Rakshanam of Swastha Purush among Sapta Chatushka. The concepts which are described in Swastha Chatushka like Matrayukta Ahara, Swasthavritta, Rutucharya Paripalan,, Sadvritta, can be considered as a actions (Karya) which help in maintenance of health in healthy person by balancing Tridoshas, Dhatus, Malas, and contexts of Adharaneeya Vega, Dharaneeya Vega, Hetu Chatushtaya can be considered as responsible causative factors (Karana) for either Swasthya Samrakshana or manifestation of diseases if not followed in the prescribed manner. Swastha Chatushka mainly explores such important principles which should be properly adopted to maintain all dimensions of health. Thus, Swastha Chatushka can be considered as a unique holistic principle of preservation of positive health and life
