Do self-reported intentions predict clinicians behaviour: a systematic review.
Background: Implementation research is the scientific study of methods to promote the systematic uptake of
clinical research findings into routine clinical practice. Several interventions have been shown to be effective in
changing health care professionals' behaviour, but heterogeneity within interventions, targeted behaviours, and
study settings make generalisation difficult. Therefore, it is necessary to identify the 'active ingredients' in
professional behaviour change strategies. Theories of human behaviour that feature an individual's "intention" to
do something as the most immediate predictor of their behaviour have proved to be useful in non-clinical
populations. As clinical practice is a form of human behaviour such theories may offer a basis for developing a
scientific rationale for the choice of intervention to use in the implementation of new practice. The aim of this
review was to explore the relationship between intention and behaviour in clinicians and how this compares to
the intention-behaviour relationship in studies of non-clinicians.
Methods: We searched: PsycINFO, MEDLINE, EMBASE, CINAHL, Cochrane Central Register of Controlled
Trials, Science/Social science citation index, Current contents (social & behavioural med/clinical med), ISI
conference proceedings, and Index to Theses. The reference lists of all included papers were checked manually.
Studies were eligible for inclusion if they had: examined a clinical behaviour within a clinical context, included
measures of both intention and behaviour, measured behaviour after intention, and explored this relationship
quantitatively. All titles and abstracts retrieved by electronic searching were screened independently by two
reviewers, with disagreements resolved by discussion.
Discussion: Ten studies were found that examined the relationship between intention and clinical behaviours in
1623 health professionals. The proportion of variance in behaviour explained by intention was of a similar
magnitude to that found in the literature relating to non-health professionals. This was more consistently the case
for studies in which intention-behaviour correspondence was good and behaviour was self-reported. Though firm
conclusions are limited by a smaller literature, our findings are consistent with that of the non-health professional
literature. This review, viewed in the context of the larger populations of studies, provides encouragement for
the contention that there is a predictable relationship between the intentions of a health professional and their
subsequent behaviour. However, there remain significant methodological challenges

Healthcare professionals' intentions and behaviours: A systematic review of studies based on social cognitive theories
<p>Abstract</p> <p>Background</p> <p>There is an important gap between the implications of clinical research evidence and the routine clinical practice of healthcare professionals. Because individual decisions are often central to adoption of a clinical-related behaviour, more information about the cognitive mechanisms underlying behaviours is needed to improve behaviour change interventions targeting healthcare professionals. The aim of this study was to systematically review the published scientific literature about factors influencing health professionals' behaviours based on social cognitive theories. These theories refer to theories where individual cognitions/thoughts are viewed as processes intervening between observable stimuli and responses in real world situations.</p> <p>Methods</p> <p>We searched psycINFO, MEDLINE, EMBASE, CIHNAL, Index to theses, PROQUEST dissertations and theses and Current Contents for articles published in English only. We included studies that aimed to predict healthcare professionals' intentions and behaviours with a clear specification of relying on a social cognitive theory. Information on percent of explained variance (R<sup>2</sup>) was used to compute the overall frequency-weighted mean R<sup>2 </sup>to evaluate the efficacy of prediction in several contexts and according to different methodological aspects. The cognitive factors most consistently associated with prediction of healthcare professionals' intention and behaviours were documented.</p> <p>Results</p> <p>Seventy eight studies met the inclusion criteria. Among these studies, 72 provided information on the determinants of intention and 16 prospective studies provided information on the determinants of behaviour. The theory most often used as reference was the Theory of Reasoned Action (TRA) or its extension the Theory of Planned Behaviour (TPB). An overall frequency-weighted mean R<sup>2 </sup>of 0.31 was observed for the prediction of behaviour; 0.59 for the prediction of intention. A number of moderators influenced the efficacy of prediction; frequency-weighted mean R<sup>2 </sup>varied from 0.001 to 0.58 for behaviour and 0.19 to 0.81 for intention.</p> <p>Conclusion</p> <p>Our results suggest that the TPB appears to be an appropriate theory to predict behaviour whereas other theories better capture the dynamic underlying intention. In addition, given the variations in efficacy of prediction, special care should be given to methodological issues, especially to better define the context of behaviour performance.</p

Effects of midwife-led maternity services on postpartum wellbeing and clinical outcomes in primiparous women under China’s one-child policy
Background:
The Midwife-led maternity services have been implemented in China in response to the high rates of primiparous women and Caesarean Sections (CS) which may be related to China’s one-child policy. However, few studies in China have been reported on the effectiveness of Midwife-led Care at Delivery (MCD) and the Continuity of Midwife-led Care (CMC) on postpartum wellbeing and other clinical outcomes. Therefore, evidence-based clinical validation is needed to develop an optimal maternity service for childbearing women in China.
Methods:
A concurrent cohort study design was conducted with 1730 pregnant women recruited from 9 hospitals in Shanghai. Among the 1730 participants at baseline, 1568 participants completed the follow-up questionnaire, with a follow-up rate of 90.6%.
Results:
Compared with the routine Obstetrician-led Maternity Care (OMC), Midwife-led Care at Delivery (MCD) was associated with CS rate (OR were 0.16; 95%CI: 0.11 to 0.25) and a higher total score of postpartum wellbeing (βwere 2.70; 95%CI: 0.70 to 4.70) when adjusting for the baseline differences and other confounders during delivery or postpartum period. Moreover, continuity of Midwife-led Care (CMC) was associated with CS rate (OR were 0.30; 95%CI: 0.23 to 0.41), as well as increased rate of breastfeeding within the first 24 h (OR were 2.49; 95% CI: 1.47 to 4.23), higher postpartum satisfaction (β = 4.52; 95% CI: 1.60 to 12.68), lower anxiety (βwere 0.66; 95% CI: 0.16 to 1.17), increased self-control (βwere 0.39; 95% CI: 0.02 to 0.76) and a higher total score of postpartum wellbeing (βwere 3.14; 95% CI: 1.54 to 4.75).
Conclusion:
CMC is the optimal service for low-risk primiparous women under China’s one-child policy, and is worthwhile for a general implementation across China
Preliminary evaluation of a primary care intervention for cry-fuss behaviours in the first 3-4 months of life ('The Possums Approach'): effects on cry-fuss behaviours and maternal mood
Problem crying in the first few months of life is both common and complex, arising out of multiple interacting and co-evolving factors. Parents whose babies cry and fuss a lot receive conflicting advice as they seek help from multiple health providers and emergency departments, and may be admitted into tertiary residential services. Conflicting advice is costly, and arises out of discipline-specific interpretations of evidence. An integrated, interdisciplinary primary care intervention (‘The Possums Approach’) for cry-fuss problems in the first months of life was developed from available peer-reviewed evidence. This study reports on preliminary evaluation of delivery of the intervention. A total of 20 mothers who had crying babies under 16 weeks of age (average age 6.15 weeks) completed questionnaires, including the Crying Patterns Questionnaire and the Edinburgh Postnatal Depression Scale, before and 3-4 weeks after their first consultation with trained primary care practitioners. Preliminary evaluation is promising. The Crying Patterns Questionnaire showed a significant decrease in crying and fussing duration, by 1 h in the evening (P = 0.001) and 30 min at night (P = 0.009). The median total amount of crying and fussing in a 24-h period was reduced from 6.12 to 3 h. The Edinburgh Postnatal Depression Scale showed a significant improvement in depressive symptoms, with the median score decreasing from 11 to 6 (P = 0.005). These findings are corroborated by an analysis of results for the subset of 16 participants whose babies were under 12 weeks of age (average age 4.71 weeks). These preliminary results demonstrate significantly decreased infant crying in the evening and during the night and improved maternal mood, validating an innovative interdisciplinary clinical intervention for cry-fuss problems in the first few months of life. This intervention, delivered by trained health professionals, has the potential to mitigate the costly problem of health professionals giving discipline-specific and conflicting advice post-birth
