    Background: Different recovery patterns are reported for those befallen a whip-lash injury, but little is known about the variability within subgroups. The aims were (1) to compare a self-selected mildly affected sample (MILD) with a self-selected moderately to severely affected sample (MOD/SEV) with regard to background characteristics and pain-related disability, pain intensity, functional self-efficacy, fear of movement/(re) injury, pain catastrophising, post-traumatic stress symptoms in the acute stage (at baseline), (2) to study the development over the first year after the accident for the above listed clinical variables in the MILD sample, and (3) to study the validity of a prediction model including baseline levels of clinical variables on pain-related disability one year after baseline assessments. Methods: The study had a prospective and correlative design. Ninety-eight participants were consecutively selected. Inclusion criteria; age 18 to 65 years, WAD grade I-II, Swedish language skills, and subjective report of not being in need of treatment due to mild symptoms. A multivariate linear regression model was applied for the prediction analysis. Results: The MILD sample was less affected in all study variables compared to the MOD/SEV sample. Pain-related disability, pain catastrophising, and post-traumatic stress symptoms decreased over the first year after the accident, whereas functional self-efficacy and fear of movement/(re) injury increased. Pain intensity was stable. Pain-related disability at baseline emerged as the only statistically significant predictor of pain-related disability one year after the accident (Adj r(2) = 0.67). Conclusion: A good prognosis over the first year is expected for the majority of individuals with WAD grade I or II who decline treatment due to mild symptoms. The prediction model was not valid in the MILD sample except for the contribution of pain-related disability. An implication is that early observations of individuals with elevated levels of pain-related disability are warranted, although they may decline treatment

    In the Sudano-Sahelian zone of Cameroon, the acceleration of drying of landscape has created uncertainty of agricultural practices too related to natural data. The cropping season depends on the climate which is subject to increasingly strong intra annual variability. Farmers' over-reliance on climate results in two major consequences for regional agriculture: replanting in a context of fragility of the seed sector and lower production levels. Public action regarding climate change (erratic rainfall, floods, drought, high temperatures), in the Sudano-Sahelian zone is mainly focused on the preservation of fragile areas (protected areas, reforestation, reducing pressure on the firewood through the promotion of improved stoves). This action is largely supported by international cooperation, and is not compatible with public policies (agriculture, food, land). Also farmers' adaptation strategies to climate change are disconnected from the objectives of innovation and research policies (agricultural intensification, agroecology). The proposed reflection raises the question of the relationship between public policy and farmers' strategies to strengthen the adoption of climate-smart practices that reduce uncertainties. This communication summarizes the results of twenty years of direct observation and field surveys of farmers' perceptions of climate change and environmental degradation. It aims to assess the strategies that farmers have adopted to cope with the new climate and to suggest recommendations for more climatologically smart agricultural practices. (Texte intégral

    Pan‐Arctic river discharge: Prioritizing monitoring of future climate change hot spots

    The Arctic freshwater cycle is changing rapidly, which will require adequate monitoring of river flows to detect, observe, and understand changes and provide adaptation information. There has, however, been little detail about where the greatest flow changes are projected, and where monitoring therefore may need to be strengthened. In this study, we used a set of recent climate model runs and an advanced macro‐scale hydrological model to analyze how flows across the continental pan‐Arctic are projected to change and where the climate models agree on significant changes. We also developed a method to identify where monitoring stations should be placed to observe these significant changes, and compared this set of suggested locations with the existing network of monitoring stations. Overall, our results reinforce earlier indications of large increases in flow over much of the Arctic, but we also identify some areas where projections agree on significant changes but disagree on the sign of change. For monitoring, central and eastern Siberia, Alaska, and central Canada are hot spots for the highest changes. To take advantage of existing networks, a number of stations across central Canada and western and central Siberia could form a prioritized set. Further development of model representation of high‐latitude hydrology would improve confidence in the areas we identify here. Nevertheless, ongoing observation programs may consider these suggested locations in efforts to improve monitoring of the rapidly changing Arctic freshwater cycle

    Attrition and misclassification of drop-outs in the analysis of unemployment duration

    Carling et al (1996) analyze a large data set of unemployed workers in order to examine, inter alia, the effect of unemployment benefits on the escape rate to employment. In this paper we take a closer look at the 20 per cent of workers who were drop-outs and check the empirical justification for modeling attrition as independent right censoring in the analysis of unemployment duration. It may very well be that dropping out, i.e. attrition, often occurs due to employment. In the analysis, we refer to these individuals as misclassified in that they are typically treated as if their unemployment spell went beyond the time of attrition. We propose to follow up the drop-outs by a supplementary sample and apply a Multiple Imputation approach to incorporate the supplementary information. Our follow-up study revealed that 45% dropped out due to employment. The escape rate to employment was as a consequence under-estimated by 20 per cent, implying that the effect of unemployment benefits on the escape rate is likely to be much greater than reported in Carling et al (1996

    Phenotype-genotype correlations for clinical variants caused by CYLD mutations

    Background Studies evaluating acceptability of simplified follow-up after medical abortion have focused on high-resource or urban settings where telephones, road connections, and modes of transport are available and where women have formal education. Objective To investigate women’s acceptability of home-assessment of abortion and whether acceptability of medical abortion differs by in-clinic or home-assessment of abortion outcome in a low-resource setting in India. Design Secondary outcome of a randomised, controlled, non-inferiority trial. Setting Outpatient primary health care clinics in rural and urban Rajasthan, India. Population Women were eligible if they sought abortion with a gestation up to 9 weeks, lived within defined study area and agreed to follow-up. Women were ineligible if they had known contraindications to medical abortion, haemoglobin < 85mg/l and were below 18 years. Methods Abortion outcome assessment through routine clinic follow-up by a doctor was compared with home-assessment using a low-sensitivity pregnancy test and a pictorial instruction sheet. A computerized random number generator generated the randomisation sequence (1:1) in blocks of six. Research assistants randomly allocated eligible women who opted for medical abortion (mifepristone and misoprostol), using opaque sealed envelopes. Blinding during outcome assessment was not possible. Main Outcome Measures Women’s acceptability of home-assessment was measured as future preference of follow-up. Overall satisfaction, expectations, and comparison with previous abortion experiences were compared between study groups. Results 731 women were randomized to the clinic follow-up group (n = 353) or home-assessment group (n = 378). 623 (85%) women were successfully followed up, of those 597 (96%) were satisfied and 592 (95%) found the abortion better or as expected, with no difference between study groups. The majority, 355 (57%) women, preferred home-assessment in the event of a future abortion. Significantly more women, 284 (82%), in the home-assessment group preferred home-assessment in the future, as compared with 188 (70%) of women in the clinic follow-up group, who preferred clinic follow-up in the future (p < 0.001). Conclusion Home-assessment is highly acceptable among women in low-resource, and rural, settings. The choice to follow-up an early medical abortion according to women’s preference should be offered to foster women’s reproductive autonomy

    Quantum effects on Lagrangian points and displaced periodic orbits in the Earth-Moon system

    Recent work in the literature has shown that the one-loop long distance quantum corrections to the Newtonian potential imply tiny but observable effects in the restricted three-body problem of celestial mechanics, i.e., at the Lagrangian libration points of stable equilibrium the planetoid is not exactly at equal distance from the two bodies of large mass, but the Newtonian values of its coordinates are changed by a few millimeters in the Earth-Moon system. First, we assess such a theoretical calculation by exploiting the full theory of the quintic equation, i.e., its reduction to Bring-Jerrard form and the resulting expression of roots in terms of generalized hypergeometric functions. By performing the numerical analysis of the exact formulas for the roots, we confirm and slightly improve the theoretical evaluation of quantum corrected coordinates of Lagrangian libration points of stable equilibrium. Second, we prove in detail that also for collinear Lagrangian points the quantum corrections are of the same order of magnitude in the Earth-Moon system. Third, we discuss the prospects to measure, with the help of laser ranging, the above departure from the equilateral triangle picture, which is a challenging task. On the other hand, a modern version of the planetoid is the solar sail, and much progress has been made, in recent years, on the displaced periodic orbits of solar sails at all libration points, both stable and unstable. The present paper investigates therefore, eventually, a restricted three-body problem involving Earth, Moon and a solar sail. By taking into account the one-loop quantum corrections to the Newtonian potential, displaced periodic orbits of the solar sail at libration points are again found to exist
