Nets, Spray or Both? The Effectiveness of Insecticide-Treated Nets and Indoor Residual Spraying in Reducing Malaria Morbidity and Child Mortality in sub-Saharan Africa.
Malaria control programmes currently face the challenge of maintaining, as well as accelerating, the progress made against malaria with fewer resources and uncertain funding. There is a critical need to determine what combination of malaria interventions confers the greatest protection against malaria morbidity and child mortality under routine conditions. This study assesses intervention effectiveness experienced by children under the age of five exposed to both insecticide-treated nets (ITNs) and indoor residual spraying (IRS), as compared to each intervention alone, based on nationally representative survey data collected from 17 countries in sub-Saharan Africa. Living in households with both ITNs and IRS was associated with a significant risk reduction against parasitaemia in medium and high transmission areas, 53% (95% CI 37% to 67%) and 31% (95% CI 11% to 47%) respectively. For medium transmission areas, an additional 36% (95% CI 7% to 53%) protection was garnered by having both interventions compared with exposure to only ITNs or only IRS. Having both ITNs and IRS was not significantly more protective against parasitaemia than either intervention alone in low and high malaria transmission areas. In rural and urban areas, exposure to both interventions provided significant protection against parasitaemia, 57% (95% CI 48% to 65%) and 39% (95% CI 10% to 61%) respectively; however, this effect was not significantly greater than having a singular intervention. Statistically, risk for all-cause child mortality was not significantly reduced by having both ITNs and IRS, and no additional protectiveness was detected for having dual intervention coverage over a singular intervention. These findings suggest that greater reductions in malaria morbidity and health gains for children may be achieved with ITNs and IRS combined beyond the protection offered by IRS or ITNs alone

This article is the result of a competitively tendered University-funded project, this brings together two major Government Policy areas: sustainable communities and use of carbon fuels, and is aimed at influencing the policy debate on the difficulties of linking remote communities to renewable energy production because of poor distribution networks. Linkage with the Sustainable Communities agenda is an essential ingredient, as the proposal is that the renewable energy technologies will be installed and maintained by the communities themselves

Gendered and generational tensions in increased land commercialisation: rural livelihood diversification, changing land use, and food security in Ghana’s Brong-Ahafo region
Many smallholder farmers in Jaman North District, Brong‐Ahafo Region, Ghana are shifting from food crop production to increased cultivation of cashew, an export cash crop. This paper examines gendered and generational tensions in increased commercialisation of land, livelihood diversification, and household food security in the context of globalisation and environmental change. Using qualitative, participatory research with 60 middle‐generation men and women, young people and key stakeholders, the research found that community members valued the additional income stream. Young people and women, however, were apprehensive about the long‐term consequences for food security of allocating so much land to cashew plantations. Young, middle, and older generations were concerned about their weak bargaining position in negotiating fair prices with export companies and intermediaries. Greater integration into the global economy exposed rural actors to multiple risks and inequalities, such as the uneven effects of economic globalisation, rises in food prices, hunger and food insecurity, growing competition for land, youth outmigration and climate change. The shift towards cashew cultivation appears to be exacerbating gender and generational inequalities in access to land and food insecurity and leading to exploitation within the global agri‐food supply chain among already vulnerable rural communities in the global South. With stronger farmer associations and cooperatives, however, cashew farmers stand the chance of benefitting from greater integration into the global economy, through strengthened bargaining positions. Greater understanding is needed about the complex interactions between sustainable food systems, changing land use and gender and generational inequalities in rural spaces
The transformation of transport policy in Great Britain? 'New Realism' and New Labour's decade of displacement activity
In a 1999 paper, Goodwin announced ‘the transformation of transport policy in Great Britain’. His central point was that consensus was emerging among policy makers and academics based on earlier work including Transport: The New Realism, which rejected previous orthodoxy that the supply of road space could and should be continually expanded to match demand. Instead a combination of investment in public transport, walking and cycling opportunities and – crucially – demand management should form the basis of transport policy to address rising vehicle use and associated increases in congestion and pollution / carbon emissions. This thinking formed the basis of the 1997 Labour government’s ‘sustainable transport’ policy, but after 13 years in power ministers neither transformed policy nor tackled longstanding transport trends. Our main aim in this paper is to revisit the concept of New Realism and re-examine its potential utility as an agent of change in British transport policy. Notwithstanding the outcome of Labour’s approach to transport policy, we find that the central tenets of the New Realism remain robust and that the main barriers to change are related to broader political and governance issues which suppress radical policy innovation
Energy privatisations, business-politics connections and governance under political Islam
The changing connections between business and political Islam under liberalisation can best be studied through an analysis of moral justifications, political connections in business and institutional modifications. Our evidence on energy utility privatisations through a comprehensive analysis of party connections with firm and industry specific data demonstrates that Turkey's Justice and Development Party (AKP) reshaped industry structures to nurture a new breed of politically linked businesses. In order to build a patronage regime in energy markets, the AKP manipulated regulatory institutions, suppressed the media and advocacy groups, and immobilised the judicial process in response to allegations of misconduct. The highly personalised and opaque allocation of assets increased the power of business groups that enjoyed close ties with the prime minister and key party officials. Leading politically connected firms emerged as not only the major beneficiaries but also key supporters of authoritarian politics. Our findings have important implications for the analysis of political Islam, regulatory institutions and the vulnerabilities of energy privatisations in emerging economies
Glycopyrrolate/formoterol fumarate metered dose inhaler for maintenance-naïve patients with chronic obstructive pulmonary disease:a post-hoc analysis of the randomized PINNACLE trials
Background: Glycopyrrolate (GP)/formoterol fumarate (FF; GFF) metered dose inhaler is a fixed-dose combination dual bronchodilator for patients with chronic obstructive pulmonary disease (COPD); however, whether the efficacy in patients without current maintenance treatment is consistent with currently maintenance-treated patients is unclear.Methods: Data from patients who were not maintenance-treated at screening (NMT) (n = 1943) and patients who were maintenance-treated at screening (MT) patients (n = 3040) receiving GFF, FF, GP, or placebo were pooled from the Phase III PINNACLE studies (NCT01854645, NCT01854658, NCT02343458) for post-hoc analysis. MT patients had received long-acting bronchodilators and/or inhaled corticosteroids in the 30 days prior to screening, and/or prior to randomization. NMT patients had received short-acting bronchodilators or no treatment. Outcomes included forced expiratory volume over 1 s (FEV1), clinically important deterioration (CID), rescue medication use, and safety.Results: GFF provided significant lung function improvements at Week 24 versus placebo, GP, and FF for NMT patients, with pre-dose trough FEV1 treatment differences of 152 (117-188) mL, 73 (45-100) mL, and 56 (29-84) mL, respectively (least squares mean change from baseline versus comparators [95% CI]; all P < 0.0001). GFF reduced the risk of CID by 17-43% in NMT (P ≤ 0.0157) and 18-52% (P ≤ 0.0012) in MT patients compared with monotherapy and placebo, and reduced rescue medication use by 1.5 puffs/day over 24 weeks for both cohorts. Safety profiles for all cohorts were consistent with each other and the parent studies.Conclusions: NMT patients achieved better lung function with GFF versus monotherapy and placebo, without increased safety risk. Dual bronchodilator therapy may offer better outcomes than monotherapy for COPD patients when administered as first-line treatment.</p
Cosmopolitan Risk Community and China's Climate Governance
Ulrich Beck asserts that global risks, such as climate change, generate a form of ‘compulsory cosmopolitanism’, which ‘glues’ various actors into collective action. Through an analysis of emerging ‘cosmopolitan risk communities’ in Chinese climate governance, this paper points out a ‘blind spot’ in the theorisation of cosmopolitan belonging and an associated inadequacy in explaining shifting power-relations. The paper addresses this problem by engaging with the intersectionality of the cosmopolitan space. It is argued that cosmopolitan belonging is a form of performative identity. Its key characteristic lies in a ‘liberating prerogative’, which enables individuals to participate in the solution of common problems creatively. It is this liberating prerogative that coerces the state out of political monopoly and marks the cosmopolitan moment
The prevalence and incidence of mental ill-health in adults with autism and intellectual disabilities
The prevalence, and incidence, of mental ill-health in adults with intellectual disabilities and autism were compared with the whole population with intellectual disabilities, and with controls, matched individually for age, gender, ability-level, and Down syndrome. Although the adults with autism had a higher point prevalence of problem behaviours compared with the whole adult population with intellectual disabilities, compared with individually matched controls there was no difference in prevalence, or incidence of either problem behaviours or other mental ill-health. Adults with autism who had problem behaviours were less likely to recover over a two-year period than were their matched controls. Apparent differences in rates of mental ill-health are accounted for by factors other than autism, including Down syndrome and ability level
Plant-RRBS, a bisulfite and next-generation sequencing-based methylome profiling method enriching for coverage of cytosine positions
Background: Cytosine methylation in plant genomes is important for the regulation of gene transcription and transposon activity. Genome-wide methylomes are studied upon mutation of the DNA methyltransferases, adaptation to environmental stresses or during development. However, from basic biology to breeding programs, there is a need to monitor multiple samples to determine transgenerational methylation inheritance or differential cytosine methylation. Methylome data obtained by sodium hydrogen sulfite (bisulfite)-conversion and next-generation sequencing (NGS) provide genome- wide information on cytosine methylation. However, a profiling method that detects cytosine methylation state dispersed over the genome would allow high-throughput analysis of multiple plant samples with distinct epigenetic signatures. We use specific restriction endonucleases to enrich for cytosine coverage in a bisulfite and NGS-based profiling method, which was compared to whole-genome bisulfite sequencing of the same plant material.
Methods: We established an effective methylome profiling method in plants, termed plant-reduced representation bisulfite sequencing (plant-RRBS), using optimized double restriction endonuclease digestion, fragment end repair, adapter ligation, followed by bisulfite conversion, PCR amplification and NGS. We report a performant laboratory protocol and a straightforward bioinformatics data analysis pipeline for plant-RRBS, applicable for any reference-sequenced plant species.
Results: As a proof of concept, methylome profiling was performed using an Oryza sativa ssp. indica pure breeding line and a derived epigenetically altered line (epiline). Plant-RRBS detects methylation levels at tens of millions of cytosine positions deduced from bisulfite conversion in multiple samples. To evaluate the method, the coverage of cytosine positions, the intra-line similarity and the differential cytosine methylation levels between the pure breeding line and the epiline were determined. Plant-RRBS reproducibly covers commonly up to one fourth of the cytosine positions in the rice genome when using MspI-DpnII within a group of five biological replicates of a line. The method predominantly detects cytosine methylation in putative promoter regions and not-annotated regions in rice.
Conclusions: Plant-RRBS offers high-throughput and broad, genome- dispersed methylation detection by effective read number generation obtained from reproducibly covered genome fractions using optimized endonuclease combinations, facilitating comparative analyses of multi-sample studies for cytosine methylation and transgenerational stability in experimental material and plant breeding populations
