Non-dipping nocturnal blood pressure and psychosis parameters in Parkinson disease
Conclusion: These results suggest that, among PD patients, a non-dipping circadian rhythm is associated with more severe symptoms of psychosis than is a dipping circadian rhythm. This association warrants further investigation
Testing for sexually transmitted infections in general practice: cross-sectional study
Background: Primary care is an important provider of sexual health care in England. We sought to explore the extent of testing for chlamydia and HIV in general practice and its relation to associated measures of sexual health in two contrasting geographical settings.Methods: We analysed chlamydia and HIV testing data from 64 general practices and one genitourinary medicine (GUM) clinic in Brent (from mid-2003 to mid-2006) and 143 general practices and two GUM clinics in Avon (2004). We examined associations between practice testing status, practice characteristics and hypothesised markers of population need (area level teenage conception rates and Index of Multiple Deprivation, IMD scores).Results: No HIV or chlamydia testing was done in 19% (12/64) of general practices in Brent, compared to 2.1% (3/143) in Avon. In Brent, the mean age of general practitioners (GPs) in Brent practices that tested for chlamydia or HIV was lower than in those that had not conducted testing. Practices where no HIV testing was done had slightly higher local teenage conception rates (median 23.5 vs. 17.4/1000 women aged 15-44, p = 0.07) and served more deprived areas (median IMD score 27.1 vs. 21.8, p = 0.05). Mean yearly chlamydia and HIV testing rates, in practices that did test were 33.2 and 0.6 (per 1000 patients aged 15-44 years) in Brent, and 34.1 and 10.3 in Avon, respectively. In Brent practices only 20% of chlamydia tests were conducted in patients aged under 25 years, compared with 39% in Avon.Conclusions: There are substantial geographical differences in the intensity of chlamydia and HIV testing in general practice. Interventions to facilitate sexually transmitted infection and HIV testing in general practice are needed to improve access to effective sexual health care. The use of routinely-collected laboratory, practice-level and demographic data for monitoring sexual health service provision and informing service planning should be more widely evaluated
Virtual Compton Scattering off a Spinless Target in AdS/QCD
We study the doubly virtual Compton scattering off a spinless target
within the Anti-de Sitter(AdS)/QCD formalism. We find
that the general structure allowed by the Lorentz invariance and gauge
invariance of the Compton amplitude is not easily reproduced with the standard
recipes of the AdS/QCD correspondence. In the soft-photon regime, where the
semi-classical approximation is supposed to apply best, we show that the
measurements of the electric and magnetic polarizabilities of a target like the
charged pion in real Compton scattering, can already serve as stringent tests.Comment: 21 pages, version to be published in JHEP
Interleukin-1β sequesters hypoxia inducible factor 2α to the primary cilium.
BACKGROUND: The primary cilium coordinates signalling in development, health and disease. Previously we have shown that the cilium is essential for the anabolic response to loading and the inflammatory response to interleukin-1β (IL-1β). We have also shown the primary cilium elongates in response to IL-1β exposure. Both anabolic phenotype and inflammatory pathology are proposed to be dependent on hypoxia-inducible factor 2 alpha (HIF-2α). The present study tests the hypothesis that an association exists between the primary cilium and HIFs in inflammatory signalling. RESULTS: Here we show, in articular chondrocytes, that IL-1β-induces primary cilia elongation with alterations to cilia trafficking of arl13b. This elongation is associated with a transient increase in HIF-2α expression and accumulation in the primary cilium. Prolyl hydroxylase inhibition results in primary cilia elongation also associated with accumulation of HIF-2α in the ciliary base and axoneme. This recruitment and the associated cilia elongation is not inhibited by blockade of HIFα transcription activity or rescue of basal HIF-2α expression. Hypomorphic mutation to intraflagellar transport protein IFT88 results in limited ciliogenesis. This is associated with increased HIF-2α expression and inhibited response to prolyl hydroxylase inhibition. CONCLUSIONS: These findings suggest that ciliary sequestration of HIF-2α provides negative regulation of HIF-2α expression and potentially activity. This study indicates, for the first time, that the primary cilium regulates HIF signalling during inflammation
Turbulence and galactic structure
Interstellar turbulence is driven over a wide range of scales by processes
including spiral arm instabilities and supernovae, and it affects the rate and
morphology of star formation, energy dissipation, and angular momentum transfer
in galaxy disks. Star formation is initiated on large scales by gravitational
instabilities which control the overall rate through the long dynamical time
corresponding to the average ISM density. Stars form at much higher densities
than average, however, and at much faster rates locally, so the slow average
rate arises because the fraction of the gas mass that forms stars at any one
time is low, ~10^{-4}. This low fraction is determined by turbulence
compression, and is apparently independent of specific cloud formation
processes which all operate at lower densities. Turbulence compression also
accounts for the formation of most stars in clusters, along with the cluster
mass spectrum, and it gives a hierarchical distribution to the positions of
these clusters and to star-forming regions in general. Turbulent motions appear
to be very fast in irregular galaxies at high redshift, possibly having speeds
equal to several tenths of the rotation speed in view of the morphology of
chain galaxies and their face-on counterparts. The origin of this turbulence is
not evident, but some of it could come from accretion onto the disk. Such high
turbulence could help drive an early epoch of gas inflow through viscous
torques in galaxies where spiral arms and bars are weak. Such evolution may
lead to bulge or bar formation, or to bar re-formation if a previous bar
dissolved. We show evidence that the bar fraction is about constant with
redshift out to z~1, and model the formation and destruction rates of bars

Theoretical Constraints on the Higgs Effective Couplings
We derive constraints on the sign of couplings in an effective Higgs
Lagrangian using prime principles such as the naturalness principle, global
symmetries, and unitarity. Specifically, we study four dimension-six operators,
O_H, O_y, O_g, and O_gamma, which contribute to the production and decay of the
Higgs boson at the Large Hadron Collider (LHC), among other things. Assuming
the Higgs is a fundamental scalar, we find: 1) the coefficient of O_H is
positive except when there are triplet scalars, resulting in a reduction in the
Higgs on-shell coupling from their standard model (SM) expectations if no other
operators contribute, 2) the linear combination of O_H and O_y controlling the
overall Higgs coupling to fermion is always reduced, 3) the sign of O_g induced
by a new colored fermion is such that it interferes destructively with the SM
top contribution in the gluon fusion production of the Higgs, if the new
fermion cancels the top quadratic divergence in the Higgs mass, and 4) the
correlation between naturalness and the sign of O_gamma is similar to that of
O_g, when there is a new set of heavy electroweak gauge bosons. Next
considering a composite scalar for the Higgs, we find the reduction in the
on-shell Higgs couplings persists. If further assuming a collective breaking
mechanism as in little Higgs theories, the coefficient of O_H remains positive
even in the presence of triplet scalars. In the end, we conclude that the gluon
fusion production of the Higgs boson is reduced from the SM rate in all
composite Higgs models. Our study suggests a wealth of information could be
revealed by precise measurements of the Higgs couplings, providing strong
motivations for both improving on measurements at the LHC and building a

Star forming dwarf galaxies
Star forming dwarf galaxies (SFDGs) have a high gas content and low
metallicities, reminiscent of the basic entities in hierarchical galaxy
formation scenarios. In the young universe they probably also played a major
role in the cosmic reionization. Their abundant presence in the local volume
and their youthful character make them ideal objects for detailed studies of
the initial stellar mass function (IMF), fundamental star formation processes
and its feedback to the interstellar medium. Occasionally we witness SFDGs
involved in extreme starbursts, giving rise to strongly elevated production of
super star clusters and global superwinds, mechanisms yet to be explored in
more detail. SFDGs is the initial state of all dwarf galaxies and the relation
to the environment provides us with a key to how different types of dwarf
galaxies are emerging. In this review we will put the emphasis on the exotic
starburst phase, as it seems less important for present day galaxy evolution

Evaluation of the current knowledge limitations in breast cancer research: a gap analysis
BACKGROUND
A gap analysis was conducted to determine which areas of breast cancer research, if targeted by researchers and funding bodies, could produce the greatest impact on patients.
METHODS
Fifty-six Breast Cancer Campaign grant holders and prominent UK breast cancer researchers participated in a gap analysis of current breast cancer research. Before, during and following the meeting, groups in seven key research areas participated in cycles of presentation, literature review and discussion. Summary papers were prepared by each group and collated into this position paper highlighting the research gaps, with recommendations for action.
RESULTS
Gaps were identified in all seven themes. General barriers to progress were lack of financial and practical resources, and poor collaboration between disciplines. Critical gaps in each theme included: (1) genetics (knowledge of genetic changes, their effects and interactions); (2) initiation of breast cancer (how developmental signalling pathways cause ductal elongation and branching at the cellular level and influence stem cell dynamics, and how their disruption initiates tumour formation); (3) progression of breast cancer (deciphering the intracellular and extracellular regulators of early progression, tumour growth, angiogenesis and metastasis); (4) therapies and targets (understanding who develops advanced disease); (5) disease markers (incorporating intelligent trial design into all studies to ensure new treatments are tested in patient groups stratified using biomarkers); (6) prevention (strategies to prevent oestrogen-receptor negative tumours and the long-term effects of chemoprevention for oestrogen-receptor positive tumours); (7) psychosocial aspects of cancer (the use of appropriate psychosocial interventions, and the personal impact of all stages of the disease among patients from a range of ethnic and demographic backgrounds).
CONCLUSION
Through recommendations to address these gaps with future research, the long-term benefits to patients will include: better estimation of risk in families with breast cancer and strategies to reduce risk; better prediction of drug response and patient prognosis; improved tailoring of treatments to patient subgroups and development of new therapeutic approaches; earlier initiation of treatment; more effective use of resources for screening populations; and an enhanced experience for people with or at risk of breast cancer and their families. The challenge to funding bodies and researchers in all disciplines is to focus on these gaps and to drive advances in knowledge into improvements in patient care
The stellar and sub-stellar IMF of simple and composite populations
The current knowledge on the stellar IMF is documented. It appears to become
top-heavy when the star-formation rate density surpasses about 0.1Msun/(yr
pc^3) on a pc scale and it may become increasingly bottom-heavy with increasing
metallicity and in increasingly massive early-type galaxies. It declines quite
steeply below about 0.07Msun with brown dwarfs (BDs) and very low mass stars
having their own IMF. The most massive star of mass mmax formed in an embedded
cluster with stellar mass Mecl correlates strongly with Mecl being a result of
gravitation-driven but resource-limited growth and fragmentation induced
starvation. There is no convincing evidence whatsoever that massive stars do
form in isolation. Various methods of discretising a stellar population are
introduced: optimal sampling leads to a mass distribution that perfectly
represents the exact form of the desired IMF and the mmax-to-Mecl relation,
while random sampling results in statistical variations of the shape of the
IMF. The observed mmax-to-Mecl correlation and the small spread of IMF
power-law indices together suggest that optimally sampling the IMF may be the
more realistic description of star formation than random sampling from a
universal IMF with a constant upper mass limit. Composite populations on galaxy
scales, which are formed from many pc scale star formation events, need to be
described by the integrated galactic IMF. This IGIMF varies systematically from
top-light to top-heavy in dependence of galaxy type and star formation rate,

Investigation of recast and crack formation in laser trepanning drilling of CMSX-4 angled holes
This paper presents an experimental investigation on the influences of laser trepanning drilling process parameters on the recast layer thickness and surface crack formation in CMSX-4 nickel-based superalloy angled holes. The effects of peak power, pulse frequency and the trepanning speed as input parameters were investigated in details by varying the laser drilling conditions using Taguchi orthogonal array-based design of experiment approach. Analysis of variance identifies the significant parameters affecting the output responses. It is found that the output responses are affected mainly by the peak power and trepanning speed. The experimental results reveal that the recast layer thickness increases with the increase of peak power and trepanning speed whereas the crack number density decreases with the increase of peak power only. Pulse frequency has no significant effect on both output responses within the range of values investigated. The knowledge gained in this parametric study could be used to improve the metallurgical characteristics of laser-drilled nickel-based acute angled holes
