Impact of proctoring on success rates for percutaneous revascularisation of coronary chronic total occlusions.
OBJECTIVE: To assess the impact of proctoring for chronic total occlusion (CTO) percutaneous coronary intervention (PCI) in six UK centres. METHODS: We retrospectively analysed 587 CTO procedures from six UK centres and compared success rates of operators who had received proctorship with success rates of the same operators before proctorship (pre-proctored) and operators in the same institutions who had not been proctored (non-proctored). There were 232 patients in the pre-proctored/non-proctored group and 355 patients in the post-proctored group. Complexity was assessed by calculating the Japanese CTO (JCTO) score for each case. RESULTS: CTO PCI success was greater in the post-proctored compared with the pre-proctored/non-proctored group (77.5% vs 62.1%, p<0.0001). In more complex cases where JCTO≥2, the difference in success was greater (70.7% vs 49.5%, p=0.0003). After proctoring, there was an increase in CTO PCI activity in centres from 2.5% to 3.5%, p<0.0001 (as a proportion of total PCI), and the proportion of very difficult cases with JCTO score ≥3 increased from 15.3% (35/229) to 29.7% (105/354), p<0.0001. CONCLUSIONS: Proctoring resulted in an increase in procedural success for CTO PCI, an increase in complex CTO PCI and an increase in total CTO PCI activity. Proctoring may be a valuable way to improve access to CTO PCI and the likelihood of procedural success
Vortex detection and quantum transport in mesoscopic graphene Josephson-junction arrays
We investigate mesoscopic Josephson junction arrays created by patterning
superconducting disks on monolayer graphene, concentrating on the high-
regime of these devices and the phenomena which contribute to the
superconducting glass state in diffusive arrays. We observe features in the
magnetoconductance at rational fractions of flux quanta per array unit cell,
which we attribute to the formation of flux-quantized vortices. The applied
fields at which the features occur are well described by Ginzburg-Landau
simulations that take into account the number of unit cells in the array. We
find that the mean conductance and universal conductance fluctuations are both
enhanced below the critical temperature and field of the superconductor, with
greater enhancement away from the graphene Dirac point.This work was financially supported by the Engineering

The Underutilization of Mental Health Care Services in the Lives of People with Blindness or Visual Impairment: A Literature Review on Rehabilitation Factors Toward Provision

Despite data demonstrating an elevated prevalence of mental health symptoms in people with BVI, this population remains largely untreated. Given that people with BVI often interface with multiple systems of rehabilitative care that are designed to improve quality of life, the relatively low rates of initiation of mental health care services are particularly alarming. In this review, the systematic processes of mental health care services across pertinent rehabilitation groups are identified and critically examined.Patients and Methods: A comprehensive literature review was conducted. The literature review utilized a critical systems typology, whereby relevant literature was selected, reviewed, and analyzed intra and extra organizationally. In addition, a thorough discussion of disability-related factors was provided, lending a socio-political lens to the problem.Results: Considerable data indicates that short-term mental health care services for people with BVI are ineffective treatment methods, albeit primarily employed in health-rehabilitative processes. Multiple studies offered data indicating the presence of diverse and entangled mental health issues relative to vocational rehabilitation services, mobility tool utility, and emotional reactions to vision loss. Many studies suggested that disability identity work be integrated into health-rehabilitative processes for people with BVI.Conclusion: To improve the utility of mental health care services for people with BVI, a reenergization of the mental health care needs of this population must occur. A critical podcast or forum focused on educating listeners about the health-rehabilitation factors that influence the provision of mental health care services can aid to improve future mental health policy and practice for people with BVI. Disability scholars need to advance research on integrating critical theories in work with people with BVI and accelerate qualitative, community-based methods to enhance understanding of this population and their unique mental health needs.Keywords: blindness, visual impairment, critical theories, vocational rehabilitation, mental health, orientation and mobility, rehabilitation, vision los
A systematic review of randomised controlled trials on the effectiveness of exercise programs on lumbo pelvic pain among postnatal women
Background: A substantial number of women tend to be affected by Lumbo Pelvic Pain (LPP) following child birth.
Physical exercise is indicated as a beneficial method to relieve LPP, but individual studies appear to suggest mixed
findings about its effectiveness. This systematic review aimed to synthesise evidence from randomised controlled trials on the effectiveness of exercise on LPP among postnatal women to inform policy, practice and future research.
Methods: A systematic review was conducted of all randomised controlled trials published between January 1990 and July 2014, identified through a comprehensive search of following databases: PubMed, PEDro, Embase, Cinahl, Medline, SPORTDiscus, Cochrane Pregnancy and Childbirth Group’s Trials Register, and electronic libraries of authors’institutions.
Randomised controlled trials were eligible for inclusion if the intervention comprised of postnatal exercise for women
with LPP onset during pregnancy or within 3 months after delivery and the outcome measures included changes in
LPP. Selected articles were assessed using the PEDro Scale for methodological quality and findings were synthesised narratively as meta-analysis was found to be inappropriate due to heterogeneity among included studies.
Results: Four randomised controlled trials were included, involving 251 postnatal women. Three trials were rated as
of ‘good’ methodological quality. All trials, except one, were at low risk of bias. The trials included physical exercise
programs with varying components, differing modes of delivery, follow up times and outcome measures. Intervention
in one trial, involving physical therapy with specific stabilising exercises, proved to be effective in reducing LPP
intensity. An improvement in gluteal pain on the right side was reported in another trial and a significant difference in
pain frequency in another.
Conclusion: Our review indicates that only few randomised controlled trials have evaluated the effectiveness of
exercise on LPP among postnatal women. There is also a great amount of variability across existing trials in the
components of exercise programs, modes of delivery, follow up times and outcome measures. While there is some
evidence to indicate the effectiveness of exercise for relieving LPP, further good quality trials are needed to ascertain
the most effective elements of postnatal exercise programs suited for LPP treatment
Chronic psychosocial and financial burden accelerates 5-year telomere shortening: findings from the Coronary Artery Risk Development in Young Adults Study.
Leukocyte telomere length, a marker of immune system function, is sensitive to exposures such as psychosocial stressors and health-maintaining behaviors. Past research has determined that stress experienced in adulthood is associated with shorter telomere length, but is limited to mostly cross-sectional reports. We test whether repeated reports of chronic psychosocial and financial burden is associated with telomere length change over a 5-year period (years 15 and 20) from 969 participants in the Coronary Artery Risk Development in Young Adults (CARDIA) Study, a longitudinal, population-based cohort, ages 18-30 at time of recruitment in 1985. We further examine whether multisystem resiliency, comprised of social connections, health-maintaining behaviors, and psychological resources, mitigates the effects of repeated burden on telomere attrition over 5 years. Our results indicate that adults with high chronic burden do not show decreased telomere length over the 5-year period. However, these effects do vary by level of resiliency, as regression results revealed a significant interaction between chronic burden and multisystem resiliency. For individuals with high repeated chronic burden and low multisystem resiliency (1 SD below the mean), there was a significant 5-year shortening in telomere length, whereas no significant relationships between chronic burden and attrition were evident for those at moderate and higher levels of resiliency. These effects apply similarly across the three components of resiliency. Results imply that interventions should focus on establishing strong social connections, psychological resources, and health-maintaining behaviors when attempting to ameliorate stress-related decline in telomere length among at-risk individuals
Astronomical Spectroscopy
Spectroscopy is one of the most important tools that an astronomer has for
studying the universe. This chapter begins by discussing the basics, including
the different types of optical spectrographs, with extension to the ultraviolet
and the near-infrared. Emphasis is given to the fundamentals of how
spectrographs are used, and the trade-offs involved in designing an
observational experiment. It then covers observing and reduction techniques,
noting that some of the standard practices of flat-fielding often actually
degrade the quality of the data rather than improve it. Although the focus is
on point sources, spatially resolved spectroscopy of extended sources is also
briefly discussed. Discussion of differential extinction, the impact of
crowding, multi-object techniques, optimal extractions, flat-fielding
considerations, and determining radial velocities and velocity dispersions
provide the spectroscopist with the fundamentals needed to obtain the best
data. Finally the chapter combines the previous material by providing some
examples of real-life observing experiences with several typical instruments.Comment: An abridged version of a chapter to appear in Planets, Stars and

TRY plant trait database - enhanced coverage and open access
Plant traits-the morphological, anatomical, physiological, biochemical and phenological characteristics of plants-determine how plants respond to environmental factors, affect other trophic levels, and influence ecosystem properties and their benefits and detriments to people. Plant trait data thus represent the basis for a vast area of research spanning from evolutionary biology, community and functional ecology, to biodiversity conservation, ecosystem and landscape management, restoration, biogeography and earth system modelling. Since its foundation in 2007, the TRY database of plant traits has grown continuously. It now provides unprecedented data coverage under an open access data policy and is the main plant trait database used by the research community worldwide. Increasingly, the TRY database also supports new frontiers of trait-based plant research, including the identification of data gaps and the subsequent mobilization or measurement of new data. To support this development, in this article we evaluate the extent of the trait data compiled in TRY and analyse emerging patterns of data coverage and representativeness. Best species coverage is achieved for categorical traits-almost complete coverage for 'plant growth form'. However, most traits relevant for ecology and vegetation modelling are characterized by continuous intraspecific variation and trait-environmental relationships. These traits have to be measured on individual plants in their respective environment. Despite unprecedented data coverage, we observe a humbling lack of completeness and representativeness of these continuous traits in many aspects. We, therefore, conclude that reducing data gaps and biases in the TRY database remains a key challenge and requires a coordinated approach to data mobilization and trait measurements. This can only be achieved in collaboration with other initiatives
How and when plume zonation appeared during the 132 Myr evolution of the Tristan Hotspot
Increasingly, spatial geochemical zonation, present as geographically distinct, subparallel
trends, is observed along hotspot tracks, such as Hawaii and the Galapagos. The origin of this
zonation is currently unclear. Recently zonation was found along the last B70 Myr of the
Tristan-Gough hotspot track. Here we present new Sr–Nd–Pb–Hf isotope data from the older
parts of this hotspot track (Walvis Ridge and Rio Grande Rise) and re-evaluate published data
from the Etendeka and Parana flood basalts erupted at the initiation of the hotspot track. We
show that only the enriched Gough, but not the less-enriched Tristan, component is present in
the earlier (70–132 Ma) history of the hotspot. Here we present a model that can explain the
temporal evolution and origin of plume zonation for both the Tristan-Gough and Hawaiian
hotspots, two end member types of zoned plumes, through processes taking place in the
plume sources at the base of the lower mantle

Consistent phenological shifts in the making of a biodiversity hotspot: the Cape flora
Background
The best documented survival responses of organisms to past climate change on short (glacial-interglacial) timescales are distributional shifts. Despite ample evidence on such timescales for local adaptations of populations at specific sites, the long-term impacts of such changes on evolutionary significant units in response to past climatic change have been little documented. Here we use phylogenies to reconstruct changes in distribution and flowering ecology of the Cape flora - South Africa's biodiversity hotspot - through a period of past (Neogene and Quaternary) changes in the seasonality of rainfall over a timescale of several million years.
Results
Forty-three distributional and phenological shifts consistent with past climatic change occur across the flora, and a comparable number of clades underwent adaptive changes in their flowering phenology (9 clades; half of the clades investigated) as underwent distributional shifts (12 clades; two thirds of the clades investigated). Of extant Cape angiosperm species, 14-41% have been contributed by lineages that show distributional shifts consistent with past climate change, yet a similar proportion (14-55%) arose from lineages that shifted flowering phenology.
Conclusions
Adaptive changes in ecology at the scale we uncover in the Cape and consistent with past climatic change have not been documented for other floras. Shifts in climate tolerance appear to have been more important in this flora than is currently appreciated, and lineages that underwent such shifts went on to contribute a high proportion of the flora's extant species diversity. That shifts in phenology, on an evolutionary timescale and on such a scale, have not yet been detected for other floras is likely a result of the method used; shifts in flowering phenology cannot be detected in the fossil record
Virtual environments as memory training devices in navigational tasks for older adults.
Cognitive training approaches using virtual environments (VEs) might counter age-related visuospatial memory decline and associated difficulties in wayfinding. However, the effects of the visual design of a VE in route learning are not fully understood. Therefore, we created a custom-designed VE optimized for route learning, with adjusted levels of realism and highlighted landmark locations (MixedVE). Herein we tested participants' route recall performance in identifying direction of turn at the intersection with this MixedVE against two baseline alternatives (AbstractVE, RealisticVE). An older vs. a younger group solved the tasks in two stages (immediate vs. delayed recall by one week). Our results demonstrate that the MixedVE facilitates better recall accuracy than the other two VEs for both age groups. Importantly, this pattern persists a week later. Additionally, our older participants were mostly overconfident in their route recall performance, but the MixedVE moderated this potentially detrimental overconfidence. Before the experiment, participants clearly preferred the RealisticVE, whereas after the experiment, most of the younger, and many of the older participants, preferred the MixedVE. Taken together, our findings provide insights into the importance of tailoring visualization design in route learning with VEs. Furthermore, we demonstrate the great potential of the MixedVE and by extension, of similar VEs as memory training devices for route learning, especially for older participants
