The response of a floating ice sheet to an accelerating line load
The two-dimensional response of a thin, floating sheet of ice to a line load that accelerates from rest at to a uniform velocity V for is determined through an integral-transform solution of the linearized equations of motion. If – i.e. if the load is impulsively started with velocity V – the solution exhibits singularities at , the shallow-water-gravity-wave speed, and , the minimum speed for transverse motion of the ice, but these singularities are avoided by the acceleration of the load through the critical speeds
A cross sectional study of water quality from dental unit water lines in dental practices in the West of Scotland
OBJECTIVE: To determine the microbiological quality of water from dental units in a general practice setting and current practice for disinfection of units. DESIGN: A cross-sectional study of the water quality from 40 dental units in 39 general practices and a questionnaire of the disinfection protocols used in those practices. SETTING: NHS practices in primarydental care. SUBJECTS: Thirty-nine general practices from the West of Scotland. METHODS: Water samples were collected on two separate occasions from dental units and analysed for microbiological quality by the total viable count (TVC) method. Water specimens were collected from the triple syringe, high speed outlet, cup filler and surgery tap. Each participating practitioner was asked to complete a questionnaire. Results Microbial contamination was highest from the high speed outlet followed by the triple syringe and cup filler. On average, the TVC counts from the high speed water lines at 37 degrees C and for the high speed lines, triple syringe and cup filler at 22 degrees C were significantly higher than that from the control tap water specimens. The study included units from 11 different manufacturers with ages ranging from under one year to over eight years. The age of the dental unit analysed did not appear to influence the level of microbial contamination. Five of the practices surveyed used disinfectants to clean the dental units but these had no significant effect on the microbiological quality of the water. The majority of dental units (25 out of 40) were never flushed with water between patients. A number of different non-sterile irrigants were used for surgical procedures. CONCLUSION: The microbiological quality of water from dental units in general dental practice is poor compared with that from drinking water sources. Suitable sterile irrigants should be used for surgical procedures in dental practice. Further work is required for pragmatic decontamination regimens of dental unit water lines in a general dental practice setting
The influence of clearance on friction, lubrication and squeaking in large diameter metal-on-metal hip replacements
Large diameter metal-on-metal bearings (MOM) are becoming increasingly popular, addressing the needs of young and more active patients. Clinical data has shown excellent short-to-mid-term results, though incidences of transient squeaking have been noted between implantation and up to 2 years post-operative. Geometric design features, such as clearance, have been significant in influencing the performance of the bearings. Sets of MOM bearings with different clearances were investigated in this study using a hip friction simulator to examine the influence of clearance on friction, lubrication and squeaking. The friction factor was found to be highest in the largest clearance bearings under all test conditions. The incidence of squeaking was also highest in the large clearance bearings, with all bearings in this group squeaking throughout the study. A very low incidence of squeaking was observed in the other two clearance groups. The measured lubricating film was found to be lowest in the large clearance bearings. This study suggests that increasing the bearing clearance results in reduced lubricant film thickness, increased friction and an increased incidence of squeaking
Evaluation of the efficacy of Alpron disinfectant for dental unit water lines
AIMS: To assess the efficacy of a disinfectant, Alpron, for controlling microbial contamination within dental unit water lines. METHODS: The microbiological quality of water emerging from the triple syringe, high speed handpiece, cup filler and surgery hand wash basin from six dental units was assessed for microbiological total viable counts at 22 degrees C and 37 degrees C before and after treatment with Alpron solutions. RESULTS: The study found that the use of Alpron disinfectant solutions could reduce microbial counts in dental unit water lines to similar levels for drinking water. This effect was maintained in all units for up to six weeks following one course of treatment. In four out of six units the low microbial counts were maintained for 13 weeks. CONCLUSIONS: Disinfectants may have a short term role to play in controlling microbial contamination of dental unit water lines to drinking water quality. However, in the longer term attention must be paid to redesigning dental units to discourage the build up of microbial biofilms
Arterial oxygen content is precisely maintained by graded erythrocytotic responses in settings of high/normal serum iron levels, and predicts exercise capacity: an observational study of hypoxaemic patients with pulmonary arteriovenous malformations.
Oxygen, haemoglobin and cardiac output are integrated components of oxygen transport: each gram of haemoglobin transports 1.34 mls of oxygen in the blood. Low arterial partial pressure of oxygen (PaO2), and haemoglobin saturation (SaO2), are the indices used in clinical assessments, and usually result from low inspired oxygen concentrations, or alveolar/airways disease. Our objective was to examine low blood oxygen/haemoglobin relationships in chronically compensated states without concurrent hypoxic pulmonary vasoreactivity.165 consecutive unselected patients with pulmonary arteriovenous malformations were studied, in 98 cases, pre/post embolisation treatment. 159 (96%) had hereditary haemorrhagic telangiectasia. Arterial oxygen content was calculated by SaO2 x haemoglobin x 1.34/100.There was wide variation in SaO2 on air (78.5-99, median 95)% but due to secondary erythrocytosis and resultant polycythaemia, SaO2 explained only 0.1% of the variance in arterial oxygen content per unit blood volume. Secondary erythrocytosis was achievable with low iron stores, but only if serum iron was high-normal: Low serum iron levels were associated with reduced haemoglobin per erythrocyte, and overall arterial oxygen content was lower in iron deficient patients (median 16.0 [IQR 14.9, 17.4]mls/dL compared to 18.8 [IQR 17.4, 20.1]mls/dL, p<0.0001). Exercise tolerance appeared unrelated to SaO2 but was significantly worse in patients with lower oxygen content (p<0.0001). A pre-defined athletic group had higher Hb:SaO2 and serum iron:ferritin ratios than non-athletes with normal exercise capacity. PAVM embolisation increased SaO2, but arterial oxygen content was precisely restored by a subsequent fall in haemoglobin: 86 (87.8%) patients reported no change in exercise tolerance at post-embolisation follow-up.Haemoglobin and oxygen measurements in isolation do not indicate the more physiologically relevant oxygen content per unit blood volume. This can be maintained for SaO2 ≥78.5%, and resets to the same arterial oxygen content after correction of hypoxaemia. Serum iron concentrations, not ferritin, seem to predict more successful polycythaemic responses
Memory-guided force output is associated with self-reported ADHD symptoms in young adults

disorder in childhood and persists into adulthood in up to 65% of cases. ADHD is associated with
adverse outcomes such as the ability to gain and maintain employment (Kessler et al. 2009;
Kupper et al. 2012), and is associated with an increased risk for substance abuse (Groenman et al.
2013; Upadhyaya 2008; Wilens et al. 1995), obesity (Albayrak et al. 2013; Cortese et al. 2008;
Nazar et al. 2012; Nazar et al. 2014), workplace injuries (Breslin and Pole 2009; Hodgkins et al.
2011; Swensen et al. 2004), and traffic accidents (Barkley and Cox 2007; Barkley et al. 1993;
Jerome et al. 2006a; Jerome et al. 2006b; Merkel et al. 2013). A majority of diagnosed children
have motor deficits, however few studies have examined motor deficits in young adults. This study
provides a novel examination of visuomotor control of grip force in young adults with and without
ADHD. Participants were instructed to maintain force production over a 20-second trial with and
without real-time visual feedback about their performance. The results demonstrated that when
visual feedback was available, adults with ADHD produced slightly higher grip force than
controls. However, when visual feedback was removed, adults with ADHD had a faster rate of
decay of force, which was associated with ADHD symptom severity and trait impulsivity. These
findings suggest that there may be important differences in the way that adults with ADHD
integrate visual feedback during continuous motor tasks. These may account for some of the motor
impairments reported in children with ADHD. These deficits could result from (1) dysfunctional

Foot health education for people with rheumatoid arthritis : the practitioner's perspective
Background: Patient education is considered to be a key role for podiatrists in the management of patients with rheumatoid arthritis (RA). Patient education has undoubtedly led to improved clinical outcomes, however no attempts have been made to optimise its content or delivery to maximise benefits within the context of the foot affected by rheumatoid arthritis. The aim of this study was to identify the nature and content of podiatrists' foot health education for people with RA. Any potential barriers to its provision were also explored.
Methods: A focus group was conducted. The audio dialogue was recorded digitally, transcribed verbatim and analysed using a structured, thematic approach. The full transcription was verified by the focus group as an accurate account of what was said. The thematic analysis framework was verified by members of the research team to ensure validity of the data.
Results: Twelve members (all female) of the north west Podiatry Clinical Effectiveness Group for Rheumatology participated. Six overarching themes emerged: (i) the essence of patient education; (ii) the content; (iii) patient-centred approach to content and timing; (iv) barriers to provision; (v) the therapeutic relationship; and (vi) tools of the trade.
Conclusion: The study identified aspects of patient education that this group of podiatrists consider most important in relation to its: content, timing, delivery and barriers to its provision. General disease and foot health information in relation to RA together with a potential prognosis for foot health, the role of the podiatrist in management of foot health, and appropriate self-management strategies were considered to be key aspects of content, delivered according to the needs of the individual. Barriers to foot health education provision, including financial constraints and difficulties in establishing effective therapeutic relationships, were viewed as factors that strongly influenced foot health education provision. These data will contribute to the development of a patient-centred, negotiated approach to the provision of foot health education for people with RA
Measurement properties of quality of life measurement instruments for infants, children and adolescents with eczema: protocol for a systematic review
Background: Eczema is a common chronic or chronically relapsing skin disease that has a substantial impact on quality of life (QoL). By means of a consensus-based process, the Harmonising Outcome Measures in Eczema (HOME) initiative has identified QoL as one of the four core outcome domains to be assessed in all eczema trials. Few measurement instruments exist to measure QoL in infants and children with eczema, but there is a great variability in both content and quality (for example, reliability and validity) of the instruments used, and it is not always clear if the best instrument is being used. Therefore, the aim of the proposed research is a comprehensive systematic assessment of the measurement properties of the existing measurement instruments that were developed and/or validated for the measurement of patient-reported QoL in infants and children with eczema. Methods/Design: This study is a systematic review of the measurement properties of patient-reported measures of QoL developed and/or validated for infants and children with eczema. Medline via PubMed and EMBASE will be searched using a selection of relevant search terms. Eligible studies will be primary empirical studies evaluating, describing, or comparing measurement properties of QoL instruments for infants and children with eczema. Eligibility assessment and data abstraction will be performed independently by two reviewers. Evidence tables will be generated for study characteristics, instrument characteristics, measurement properties, and interpretability. The adequacy of the measurement properties will be assessed using predefined criteria. Methodological quality of studies will be assessed using the COnsensus-based Standards for the selection of health Measurement INstruments (COSMIN) checklist. A best evidence synthesis will be undertaken if more than one study has investigated a particular measurement property. Discussion: The proposed systematic review will produce a comprehensive assessment of measurement properties of existing QoL instruments in infants and children with eczema. We aim to identify one best currently available instrument to measure QoL in infants and/or children with eczema

Constraining the nature of two Lyα emitters detected by ALMA at z = 4.7
We report optical VLT FORS2 spectroscopy of the two Ly-alpha emitters (LAEs)
companions to the quasi-stellar object (QSO) - sub-millimetre galaxy (SMG)
system BRI1202-0725 at z = 4.7, which have recently been detected in the
[CII]158um line by the Atacama Large Millimetre/Sub-millimetre Array (ALMA). We
detect Ly-alpha emission from both sources and so confirm that these Ly-alpha
emitter candidates are physically associated with the BRI1202- 0725 system. We
also report the lack of detection of any high ionisation emission lines (N V,
Si IV, C IV and He II) and find that these systems are likely not photoionised
by the quasar, leaving in situ star formation as the main powering source of
these LAEs. We also find that both LAEs have Ly-alpha emission much broader
(1300 km/s) than the [CII] emission and broader than most LAEs. In addition,
both LAEs have roughly symmetric Ly-alpha profiles implying that both systems
are within the HII sphere produced by the quasar. This is the first time that
the proximity zone of a quasar is probed by exploiting nearby Ly-alpha
emitters. We discuss the observational properties of these galaxies in the

The Architectural Design Rules of Solar Systems based on the New Perspective
On the basis of the Lunar Laser Ranging Data released by NASA on the Silver
Jubilee Celebration of Man Landing on Moon on 21st July 1969-1994, theoretical
formulation of Earth-Moon tidal interaction was carried out and Planetary
Satellite Dynamics was established. It was found that this mathematical
analysis could as well be applied to Star and Planets system and since every
star could potentially contain an extra-solar system, hence we have a large
ensemble of exoplanets to test our new perspective on the birth and evolution
of solar systems. Till date 403 exoplanets have been discovered in 390
extra-solar systems. I have taken 12 single planet systems, 4 Brown Dwarf -
Star systems and 2 Brown Dwarf pairs. Following architectural design rules are
corroborated through this study of exoplanets. All planets are born at inner
Clarke Orbit what we refer to as inner geo-synchronous orbit in case of
Earth-Moon System. By any perturbative force such as cosmic particles or
radiation pressure, the planet gets tipped long of aG1 or short of aG1. Here
aG1 is inner Clarke Orbit. The exoplanet can either be launched on death spiral
as CLOSE HOT JUPITERS or can be launched on an expanding spiral path as the
planets in our Solar System are. It was also found that if the exo-planet are
significant fraction of the host star then those exo-planets rapidly migrate
from aG1 to aG2 and have very short Time Constant of Evolution as Brown Dwarfs
have. This vindicates our basic premise that planets are always born at inner
Clarke Orbit. This study vindicates the design rules which had been postulated
at 35th COSPAR Scientific Assembly in 2004 at Paris, France, under the title
