Factors associated with alcohol reduction in harmful and hazardous drinkers following alcohol brief intervention in Scotland: a qualitative enquiry
Background: Alcohol Brief Intervention (ABI) uses a motivational counselling approach to support individuals to
reduce excessive alcohol consumption. There is growing evidence on ABI’s use within various health care settings,
although how they work and which components enhance success is largely unknown. This paper reports on the
qualitative part of a mixed methods study. It explores enablers and barriers associated with alcohol reduction
following an ABI. It focuses on alcohol’s place within participants’ lives and their personal perspectives on reducing
consumption. There are a number of randomised controlled trials in this field though few ABI studies have
addressed the experiences of hazardous/harmful drinkers. This study examines factors associated with alcohol
reduction in harmful/hazardous drinkers following ABI.
Methods: This qualitative study was underpinned by a realist evaluation approach and involved semistructured
interviews with ten harmful or hazardous alcohol drinkers. Participants (n = 10) were from the
intervention arm of a randomised controlled trial (n = 124). All had received ABI, a 20 min motivational
counselling interview, six months previously, and had reduced their alcohol consumption. Interviews were
recorded, transcribed verbatim and thematically analysed.
Results: Participants described their views on alcohol, its’ place in their lives, their personal perspectives on
reducing their consumption and future aspirations.
Conclusions: The findings provide an insight into participants’ views on alcohol, ABI, and the barriers and enablers
to change. Participants described a cost benefit analysis, with some conscious consideration of the advantages and
disadvantages of reducing intake or abstaining from alcohol. Findings suggest that, whilst hospital admission can act
as a catalyst, encouraging individuals to reflect on their alcohol consumption through ABI may consolidate this, turning
this reflective moment into action. Sustainability may be enhanced by the presence of a ‘significant other’ who
encourages and experiences benefit. In addition having a purpose or structure with activities linked to employment
and/or social and leisure pursuits offers the potential to enhance and sustain reduced alcohol consumption.

Five-year follow-up results of the PROFHER trial comparing operative and non-operative treatment of adults with a displaced fracture of the proximal humerus
AIMS: The PROximal Fracture of the Humerus Evaluation by Randomisation (PROFHER) randomised clinical trial compared the operative and non-operative treatment of adults with a displaced fracture of the proximal humerus involving the surgical neck. The aim of this study was to determine the long-term treatment effects beyond the two-year follow-up. PATIENTS AND METHODS: Of the original 250 trial participants, 176 consented to extended follow-up and were sent postal questionnaires at three, four and five years after recruitment to the trial. The Oxford Shoulder Score (OSS; the primary outcome), EuroQol 5D-3L (EQ-5D-3L), and any recent shoulder operations and fracture data were collected. Statistical and economic analyses, consistent with those of the main trial were applied. RESULTS: OSS data were available for 164, 155 and 149 participants at three, four and five years, respectively. There were no statistically or clinically significant differences between operative and non-operative treatment at each follow-up point. No participant had secondary shoulder surgery for a new complication. Analyses of EQ-5D-3L data showed no significant between-group differences in quality of life over time. CONCLUSION: These results confirm that the main findings of the PROFHER trial over two years are unchanged at five years. Cite this article: Bone Joint J 2017;99-B:383-92
Phytoestrogens
Collectively, plants contain several different families of natural products among which are compounds with weak estrogenic or antiestrogenic activity toward mammals. These compounds, termed phytoestrogens, include certain isoflavonoids, flavonoids, stilbenes, and lignans. The best-studied dietary phytoestrogens are the soy isoflavones and the flaxseed lignans. Their perceived health beneficial properties extend beyond hormone-dependent breast and prostate cancers and osteoporosis to include cognitive function, cardiovascular disease, immunity and inflammation, and reproduction and fertility. In the future, metabolic engineering of plants could generate novel and exquisitely controlled dietary sources with which to better assess the potential health beneficial effects of phytoestrogens
Sociological and Communication-Theoretical Perspectives on the Commercialization of the Sciences
Both self-organization and organization are important for the further
development of the sciences: the two dynamics condition and enable each other.
Commercial and public considerations can interact and "interpenetrate" in
historical organization; different codes of communication are then
"recombined." However, self-organization in the symbolically generalized codes
of communication can be expected to operate at the global level. The Triple
Helix model allows for both a neo-institutional appreciation in terms of
historical networks of university-industry-government relations and a
neo-evolutionary interpretation in terms of three functions: (i) novelty
production, (i) wealth generation, and (iii) political control. Using this
model, one can appreciate both subdynamics. The mutual information in three
dimensions enables us to measure the trade-off between organization and
self-organization as a possible synergy. The question of optimization between
commercial and public interests in the different sciences can thus be made

The Citation Field of Evolutionary Economics
Evolutionary economics has developed into an academic field of its own,
institutionalized around, amongst others, the Journal of Evolutionary Economics
(JEE). This paper analyzes the way and extent to which evolutionary economics
has become an interdisciplinary journal, as its aim was: a journal that is
indispensable in the exchange of expert knowledge on topics and using
approaches that relate naturally with it. Analyzing citation data for the
relevant academic field for the Journal of Evolutionary Economics, we use
insights from scientometrics and social network analysis to find that, indeed,
the JEE is a central player in this interdisciplinary field aiming mostly at
understanding technological and regional dynamics. It does not, however, link
firmly with the natural sciences (including biology) nor to management
sciences, entrepreneurship, and organization studies. Another journal that
could be perceived to have evolutionary acumen, the Journal of Economic Issues,
does relate to heterodox economics journals and is relatively more involved in
discussing issues of firm and industry organization. The JEE seems most keen to
develop theoretical insights
Measurement of the inclusive and dijet cross-sections of b-jets in pp collisions at sqrt(s) = 7 TeV with the ATLAS detector
The inclusive and dijet production cross-sections have been measured for jets
containing b-hadrons (b-jets) in proton-proton collisions at a centre-of-mass
energy of sqrt(s) = 7 TeV, using the ATLAS detector at the LHC. The
measurements use data corresponding to an integrated luminosity of 34 pb^-1.
The b-jets are identified using either a lifetime-based method, where secondary
decay vertices of b-hadrons in jets are reconstructed using information from
the tracking detectors, or a muon-based method where the presence of a muon is
used to identify semileptonic decays of b-hadrons inside jets. The inclusive
b-jet cross-section is measured as a function of transverse momentum in the
range 20 < pT < 400 GeV and rapidity in the range |y| < 2.1. The bbbar-dijet
cross-section is measured as a function of the dijet invariant mass in the
range 110 < m_jj < 760 GeV, the azimuthal angle difference between the two jets
and the angular variable chi in two dijet mass regions. The results are
compared with next-to-leading-order QCD predictions. Good agreement is observed
between the measured cross-sections and the predictions obtained using POWHEG +
Pythia. MC@NLO + Herwig shows good agreement with the measured bbbar-dijet
cross-section. However, it does not reproduce the measured inclusive
cross-section well, particularly for central b-jets with large transverse

Search for direct pair production of the top squark in all-hadronic final states in proton-proton collisions at s√=8 TeV with the ATLAS detector
The results of a search for direct pair production of the scalar partner to the top quark using an integrated luminosity of 20.1fb−1 of proton–proton collision data at √s = 8 TeV recorded with the ATLAS detector at the LHC are reported. The top squark is assumed to decay via t˜→tχ˜01 or t˜→ bχ˜±1 →bW(∗)χ˜01 , where χ˜01 (χ˜±1 ) denotes the lightest neutralino (chargino) in supersymmetric models. The search targets a fully-hadronic final state in events with four or more jets and large missing transverse momentum. No significant excess over the Standard Model background prediction is observed, and exclusion limits are reported in terms of the top squark and neutralino masses and as a function of the branching fraction of t˜ → tχ˜01 . For a branching fraction of 100%, top squark masses in the range 270–645 GeV are excluded for χ˜01 masses below 30 GeV. For a branching fraction of 50% to either t˜ → tχ˜01 or t˜ → bχ˜±1 , and assuming the χ˜±1 mass to be twice the χ˜01 mass, top squark masses in the range 250–550 GeV are excluded for χ˜01 masses below 60 GeV
Scaphoid Waist Internal Fixation for Fractures Trial (SWIFFT) protocol : a pragmatic multi-centre randomised controlled trial of cast treatment versus surgical fixation for the treatment of bi-cortical, minimally displaced fractures of the scaphoid waist in adults
BACKGROUND: A scaphoid fracture is the most common type of carpal fracture affecting young active people. The optimal management of this fracture is uncertain. When treated with a cast, 88 to 90 % of these fractures unite; however, for the remaining 10-12 % the non-union almost invariably leads to arthritis. The alternative is surgery to fix the scaphoid with a screw at the outset. METHODS/DESIGN: We will conduct a randomised controlled trial (RCT) of 438 adult patients with a "clear" and "bicortical" scaphoid waist fracture on plain radiographs to evaluate the clinical effectiveness and cost-effectiveness of plaster cast treatment (with fixation of those that fail to unite) versus early surgical fixation. The plaster cast treatment will be immobilisation in a below elbow cast for 6 to 10 weeks followed by mobilisation. If non-union is confirmed on plain radiographs and/or Computerised Tomogram at 6 to 12 weeks, then urgent surgical fixation will be performed. This is being compared with immediate surgical fixation with surgeons using their preferred technique and implant. These treatments will be undertaken in trauma units across the United Kingdom. The primary outcome and end-point will be the Patient Rated Wrist Evaluation (a patient self-reported assessment of wrist pain and function) at 52 weeks and also measured at 6, 12, 26 weeks and 5 years. Secondary outcomes include an assessment of radiological union of the fracture; quality of life; recovery of wrist range and strength; and complications. We will also qualitatively investigate patient experiences of their treatment. DISCUSSION: Scaphoid fractures are an important public health problem as they predominantly affect young active individuals in the more productive working years of their lives. Non-union, if untreated, can lead to arthritis which can disable patients at a very young age. There is a rapidly increasing trend for immediate surgical fixation of these fractures but there is insufficient evidence from existing RCTs to support this. The SWIFFT Trial is a rigorously designed and adequately powered study which aims to contribute to the evidence-base to inform clinical decisions for the treatment of this common fracture in adults. Search for R-parity-violating supersymmetry in events with four or more leptons in sqrt(s) =7 TeV pp collisions with the ATLAS detector
A search for new phenomena in final states with four or more leptons (electrons or muons) is presented. The analysis is based on 4.7 fb−1 of proton-proton collisions delivered by the Large Hadron Collider and recorded with the ATLAS detector. Observations are consistent with Standard Model expectations in two signal regions: one that requires moderate values of missing transverse momentum and another that requires large effective mass. The results are interpreted in a simplified model of R-parity-violating supersymmetry in which a 95% CL exclusion region is set for charged wino masses up to 540 GeV. In an R-parity-violating MSUGRA/CMSSM model, values of m 1/2 up to 820 GeV are excluded for 10 < tan β < 40
Measurement of the cross-section of high transverse momentum vector bosons reconstructed as single jets and studies of jet substructure in pp collisions at √s = 7 TeV with the ATLAS detector
This paper presents a measurement of the cross-section for high transverse momentum W and Z bosons produced in pp collisions and decaying to all-hadronic final states. The data used in the analysis were recorded by the ATLAS detector at the CERN Large Hadron Collider at a centre-of-mass energy of √s = 7 TeV;{\rm Te}{\rm V}4.6\;{\rm f}{{{\rm b}}^{-1}}{{p}_{{\rm T}}}\gt 320\;{\rm Ge}{\rm V}|\eta |\lt 1.9{{\sigma }_{W+Z}}=8.5\pm 1.7$ pb and is compared to next-to-leading-order calculations.
