Recommendations for coronavirus infection in rheumatic diseases treated with biologic therapy.
The Coronavirus-associated disease, that was first identified in 2019 in China (CoViD-19), is a pandemic caused by a bat-derived beta-coronavirus, named SARS-CoV2. It shares homology with SARS and MERS-CoV, responsible for past outbreaks in China and in Middle East. SARS-CoV2 spread from China where the first infections were described in December 2019 and is responsible for the respiratory symptoms that can lead to acute respiratory distress syndrome. A cytokine storm has been shown in patients who develop fatal complications, as observed in past coronavirus infections. The management includes ventilatory support and broad-spectrum antiviral drugs, empirically utilized, as a targeted therapy and vaccines have not been developed. Based upon our limited knowledge on the pathogenesis of CoViD-19, a potential role of some anti-rheumatic drugs may be hypothesized, acting as direct antivirals or targeting host immune response. Antimalarial drugs, commonly used in rheumatology, may alter the lysosomal proteases that mediates the viral entry into the cell and have demonstrated efficacy in improving the infection. Anti-IL-1 and anti-IL-6 may interfere with the cytokine storm in severe cases and use of tocilizumab has shown good outcomes in a small cohort. Baricitinib has both antiviral and anti-inflammatory properties. Checkpoints inhibitors such as anti-CD200 and anti-PD1 could have a role in the treatment of CoViD-19. Rheumatic disease patients taking immunosuppressive drugs should be recommended to maintain the chronic therapy, prevent infection by avoiding social contacts and pausing immunosuppressants in case of infection. National and international registries are being created to collect data on rheumatic patients with CoViD-19
A mediação de leitura como recurso de comunicação com crianças hospitalizadas
The Live Library in Hospitals Project is a strategy adopted by several health institutions for the purpose of providing hospitalized children and adolescents the reading mediation of infant-juvenile stories through professionals and volunteers capable of this function. This study aimed to find out to what extent the reading of stories strategy proposed by this project in hospitals can be a communication resource to use with hospitalized children. In order to do that, empirical data collection was carried out through the use of semi structured interviews with the reader and children above the age of seven years and observation of reading sections. The qualitative data analysis was doing it and it was verified that the reading mediation favors the dialogs and relationships; contribute for the expansion of the diagnostic and therapeutic and development processes of children, relatives and health professionals.El Proyecto Biblioteca Viva en Hospitales es una estrategia adoptada por diversas instituciones de salud con el objetivo de llevar al niño y al adolescente hospitalizados a la mediación de la lectura de historias infante-juveniles, por intermedio de profesionales y voluntarios capacitados para esa función. El objetivo de este trabajo es aprender en que medida la estrategia de la mediación de historias, propuesta por lo Proyecto Biblioteca Viva en Hospitales, puede ser un recurso de comunicación con el niño hospitalizado. Para esto, la recolección de datos empíricos fue realizada mediante una entrevista semiestructurada con mediadores y niños mayores de siete años y observando las sesiones de mediación de lectura. Se realizó un análisis cualitativo de los datos y se constató que la mediación de lectura facilita los diálogos y la relación, amplía el proceso diagnóstico y terapéutico y valoriza el proceso de desarrollo de los niños, de los familiares y del equipo de salud.O Projeto Biblioteca Viva em Hospitais é estratégia adotada por diversas instituições de saúde com o objetivo de levar à criança e ao adolescente hospitalizados a mediação de leitura de histórias infanto-juvenis, por intermédio de profissionais e voluntários capacitados para tal função. ANALISIS INTERDEPENDENSI FOREIGN DIRECT INVESTMENT (FDI) DENGAN VARIABEL MAKRO EKONOMI
ABSTRAK Tujuan utama dari penelitian ini adalah untuk menganalisis interdependensi antara FDI dengan beberapa variabel yang lain, seperti PDB, Trade, Nilai Tukar, dan Tingkat bunga. Model VAR digunakan untuk menunjukkan pandangan yang komprehensif dari interdependensi ini. Hasil empiris menunjukkan bahwa melalui model VAR, interdependensi antara variabel FDI, PDB, Trade, Nilai Output Industri, Nilai Tukar dan Tingkat Suku Bunga telah diteliti dalam hubungan jangka panjang melalui kointegrasi vektor dan jangka pendek yang berdampak pada model VAR. Korelasi dinamis variabel telah diperoleh dengan analisis varian dan analisis respon impuls. Beberapa implikasi besar muncul dari hasil penelitian. Jika pemerintah Indonesia berkeinginan mendorong FDI dan pertumbuhan ekonomi, hal ini dapat dilakukan dengan output dan nilai tukar. Phase II trial of neoadjuvant pemetrexed plus cisplatin followed by surgery and radiation in the treatment of pleural mesothelioma
BACKGROUND: Malignant pleural mesothelioma is an aggressive tumor that has a poor prognosis and is resistant to unimodal approaches. Multimodal treatment has provided encouraging results. METHODS: Phase II, open-label study of the combination of chemotherapy (pemetrexed 500 mg/m(2)+cisplatin 75 mg/m(2) IV every 21 days × 3 cycles), followed by surgery (en-bloc extrapleural pneumonectomy, 3–8 weeks after chemotherapy) and hemithoracic radiation (total radiation beam 54 Gy, received 4–8 weeks post-surgery). The primary endpoint was event-free survival, defined as the time from enrollment to time of first observation of disease progression, death due to any cause, or early treatment discontinuation. RESULTS: Fifty-four treatment-naïve patients with T1-3 N0-2 malignant pleural mesothelioma were enrolled, 52 (96.3%) completed chemotherapy, 45 (83.3%) underwent surgery, 22 (40.7%) completed the whole treatment including 90-day post-radiation follow-up. The median event-free survival was 6.9 months (95%CI: 5.0-10.5), median overall survival was 15.5 months (95%CI 11.0-NA) while median time-to-tumor response was 4.8 months (95%CI: 2.5-8.0). Eighteen (33.3%) and 13 (24.1%) patients were still event-free after 1 and 2 years, respectively. The most common treatment-emergent adverse events were nausea (63.0%), anemia (51.9%) and hypertension (42.6%). Following two cardiopulmonary radiation-related deaths the protocol was amended (21 [38.9%] patients were already enrolled in the study): the total radiation beam was reduced from 54 Gy to 50.4 Gy and a more accurate selection of patients was recommended. CONCLUSIONS: The combination of pemetrexed plus cisplatin followed by surgery and hemithoracic radiation is feasible and has a manageable toxicity profile in carefully selected patients. Epigenetic gene regulation by Janus kinase 1 in diffuse large B-cell lymphoma
Janus kinases (JAKs) classically signal by activating STAT transcription factors but can also regulate gene expression by epigenetically phosphorylating histone H3 on tyrosine 41 (H3Y41-P). In diffuse large B-cell lymphomas (DLBCLs), JAK signaling is a feature of the activated B-cell (ABC) subtype and is triggered by autocrine production of IL-6 and IL-10. Whether this signaling involves STAT activation, epigenetic modification of chromatin, or both mechanisms is unknown. Here we use genetic and pharmacological inhibition to show that JAK1 signaling sustains the survival of ABC DLBCL cells. Whereas STAT3 contributed to the survival of ABC DLBCL cell lines, forced STAT3 activity could not protect these cells from death following JAK1 inhibition, suggesting epigenetic JAK1 action. JAK1 regulated the expression of nearly 3,000 genes in ABC DLBCL cells, and the chromatin surrounding many of these genes was modified by H3Y41-P marks that were diminished by JAK1 inhibition. These JAK1 epigenetic target genes encode important regulators of ABC DLBCL proliferation and survival, including IRF4, MYD88, and MYC. Directional Speaker
Parametric Acoustic Arrays (PAA) are highly directional soundwaves whose attenuation and spread are much less affected by distance from the source as compared to traditional speakers. A well-known application for PAA is the directional speaker. Studies show that noise directly affects health and decreasing noise pollution can help improve the quality of life for millions of people. Directional speakers can revolutionize society by coupling space with sound. This project encodes audio into two RF-Pulse Width Modulated (PWM) waves to drive a transducer array. Each RF-PWM wave interferes with the other in air to generate a Single Sideband (SSB) frequency spectrum that is audible with an inherent directionality. The speaker was made to be cheaper than contemporary alternatives on the market, costing less than $200 and allowing customers with lower budgets to access this modern technology. This cheaper approach using RF-PWM has proven to be viable, producing clear, audible, and extremely directional sound over three feet away from the source
Effective and Efficient Implementation of Alternative Project Delivery Methods
The multiple project delivery methods that exist today each have their own advantages and disadvantages. Some procedures within the methods have also been proven to be effective when applied to most project formats, specifically transportation projects. In order to improve the overall capabilities of a transportation agency, it would be advantageous to seek out these effective procedures and gather them in purpose to refine the existing procedures of the said agency. Through our own methods we, The Center of Excellence in Project Management at the Department for Civil/Environmental Engineering at the University of Maryland, have done this very action. We have identified a multitude of best practices throughout the transportation construction industry and assembled them in a reviewable format in order to aid the Maryland State Highway Association (MDSHA). We conducted a thorough, comprehensive and objective literary review over many other state’s departments of transportation (DOTs) best practices involving each project delivery methods in use. In the following white paper review, we plan on achieving a few different objectives. Our first objective is to describe in detail what exactly we mean by the term ‘best practice’ and other related terminology. Our second objective is to describe what we found to be best practices in other states and to determine how those other states have employed them. Our final objective is to determine how the MDSHA could employ these techniques into their own repertoire in order to improve their overall effectiveness and abilities

First Report of Circulating MicroRNAs in Tumour Necrosis Factor Receptor-Associated Periodic Syndrome (TRAPS)
Tumor necrosis factor-receptor associated periodic syndrome (TRAPS) is a rare autosomal dominant autoinflammatory disorder characterized by recurrent episodes of long-lasting fever and inflammation in different regions of the body, such as the musculo-skeletal system, skin, gastrointestinal tract, serosal membranes and eye. Our aims were to evaluate circulating microRNAs (miRNAs) levels in patients with TRAPS, in comparison to controls without inflammatory diseases, and to correlate their levels with parameters of disease activity and/or disease severity. Expression levels of circulating miRNAs were measured by Agilent microarrays in 29 serum samples from 15 TRAPS patients carrying mutations known to be associated with high disease penetrance and from 8 controls without inflammatory diseases. Differentially expressed and clinically relevant miRNAs were detected using GeneSpring GX software. We identified a 6 miRNAs signature able to discriminate TRAPS from controls. Moreover, 4 miRNAs were differentially expressed between patients treated with the interleukin (IL)-1 receptor antagonist, anakinra, and untreated patients. Of these, miR-92a-3p and miR-150-3p expression was found to be significantly reduced in untreated patients, while their expression levels were similar to controls in samples obtained during anakinra treatment. MiR-92b levels were inversely correlated with the number of fever attacks/year during the 1st year from the index attack of TRAPS, while miR-377-5p levels were positively correlated with serum amyloid A (SAA) circulating levels. Our data suggest that serum miRNA levels show a baseline pattern in TRAPS, and may serve as potential markers of response to therapeutic intervention
