Hollow Gaussian Schell-model beam and its propagation
In this paper, we present a new model, hollow Gaussian-Schell model beams
(HGSMBs), to describe the practical dark hollow beams. An analytical
propagation formula for HGSMBs passing through a paraxial first-order optical
system is derived based on the theory of coherence. Based on the derived
formula, an application example showing the influence of spatial coherence on
the propagation of beams is illustrated. It is found that the beam propagating
properties of HGSMBs will be greatly affected by their spatial coherence. Our
model provides a very convenient way for analyzing the propagation properties
of partially coherent dark hollow beams.Comment: 13pages, 2 figure
Effects of traumatic brain injury on cognitive functioning and cerebral metabolites in HIV-infected individuals.
We explored the possible augmenting effect of traumatic brain injury (TBI) history on HIV (human immunodeficiency virus) associated neurocognitive complications. HIV-infected participants with self-reported history of definite TBI were compared to HIV patients without TBI history. Groups were equated for relevant demographic and HIV-associated characteristics. The TBI group evidenced significantly greater deficits in executive functioning and working memory. N-acetylaspartate, a putative marker of neuronal integrity, was significantly lower in the frontal gray matter and basal ganglia brain regions of the TBI group. Together, these results suggest an additional brain impact of TBI over that from HIV alone. One clinical implication is that HIV patients with TBI history may need to be monitored more closely for increased risk of HIV-associated neurocognitive disorder signs or symptoms
A California Statewide Three-Dimensional Seismic Velocity Model from Both Absolute and Differential Times
We obtain a seismic velocity model of the California crust and uppermost mantle using a regional-scale double-difference tomography algorithm. We begin by using absolute arrival-time picks to solve for a coarse three-dimensional (3D) P velocity (V_P) model with a uniform 30 km horizontal node spacing, which we then use as the starting model for a finer-scale inversion using double-difference tomography applied to absolute and differential pick times. For computational reasons, we split the state into 5 subregions with a grid spacing of 10 to 20 km and assemble our final statewide V_P model by stitching together these local models. We also solve for a statewide S-wave model using S picks from both the Southern California Seismic Network and USArray, assuming a starting model based on the VP results and a V_P/V_S ratio of 1.732. Our new model has improved areal coverage compared with previous models, extending 570 km in the SW–NE direction and 1320 km in the NW–SE direction. It also extends to greater depth due to the inclusion of substantial data at large epicentral distances. Our V_P model generally agrees with previous separate regional models for northern and southern California, but we also observe some new features, such as high-velocity anomalies at shallow depths in the Klamath Mountains and Mount Shasta area, somewhat slow velocities in the northern Coast Ranges, and slow anomalies beneath the Sierra Nevada at midcrustal and greater depths. This model can be applied to a variety of regional-scale studies in California, such as developing a unified statewide earthquake location catalog and performing regional waveform modeling
A three-dimensional crustal seismic velocity model for southern California from a composite event method
We present a new crustal seismic velocity model for southern California derived from P and S arrival times from local earthquakes and explosions. To reduce the volume of data and ensure a more uniform source distribution, we compute "composite event" picks for 2597 distributed master events that include pick information for other events within spheres of 2 km radius. The approach reduces random picking error and maximizes the number of S wave picks. To constrain absolute event locations and shallow velocity structure, we also use times from controlled sources, including both refraction shots and quarries. We implement the SIMULPS tomography algorithm to obtain three-dimensional (3-D) V_p and V_p/V_s structure and hypocenter locations of the composite events. Our new velocity model in general agrees with previous studies, resolving low-velocity features at shallow depths in the basins and some high-velocity features in the midcrust. Using our velocity model and 3-D ray tracing, we relocate about 450,000 earthquakes from 1981 to 2005. We observe a weak correlation between seismic velocities and earthquake occurrence, with shallow earthquakes mostly occurring in high P velocity regions and midcrustal earthquakes occurring in low P velocity regions. In addition, most seismicity occurs in regions with relatively low V_p/V_s ratios, although aftershock sequences following large earthquakes are often an exception to this pattern
Triage of women with equivocal or low-grade cervical cytology results: a meta-analysis of the HPV test positivity rate
Introduction Methods Results Discussion Conclusion Abstract Consistent evidence underlines the utility of human papillomavirus (HPV) DNA testing in the management of women with equivocal cervical cytological abnormalities, but not in case of low-grade lesions. We performed a meta-analysis including studies where the high-risk probe of the Hybrid Capture-II is used to triage these two cytological categories. The triage test-positivity rate reflects the colposcopy referral workload.Data were pooled on the HPV test positivity rate in women with atypical squamous cells of undetermined significance (ASCUS/ASC-US) or low-grade squamous intraepithelial lesions (LSIL), derived from different cytological classification systems. The meta-analysis was restricted to studies, published between 1991 and 2007. A random-effect model was applied for meta-analytical pooling and the influence of covariates on the HPV positivity rate was analyzed by meta-regression. The variation by age was assessed within individual studies since age strata were not defined uniformly. On an average, 43% (95% CI: 40-46%) of women with ASCUS/ASC-US were high-risk HPV positive (range 23-74%). In women with LSIL, the pooled positivity rate was 76% (95% CI: 71-81%; range 55-89%). In spite of considerable inter-study heterogeneity, the difference in HPV positivity between the two triage groups was large and highly significant: 32% (95% CI: 27-38%). HPV rates dropped tremendously as age and cutoffs of test positivity increased. Other factors (cytological classification system, country, continent, collection method and year of publication) had no statistically significant impact, except in LSIL triage where HPV positivity was significantly lower in European compared to American studies. Women with LSIL, especially younger women, have high HPV positivity rates suggesting limited utility of reflex HPV triaging these cases. Research is needed to identify more specific methods to triage women with low-grade squamous cervical lesions
A proposal for a coordinated effort for the determination of brainwide neuroanatomical connectivity in model organisms at a mesoscopic scale
In this era of complete genomes, our knowledge of neuroanatomical circuitry
remains surprisingly sparse. Such knowledge is however critical both for basic
and clinical research into brain function. Here we advocate for a concerted
effort to fill this gap, through systematic, experimental mapping of neural
circuits at a mesoscopic scale of resolution suitable for comprehensive,
brain-wide coverage, using injections of tracers or viral vectors. We detail
the scientific and medical rationale and briefly review existing knowledge and
experimental techniques. We define a set of desiderata, including brain-wide
coverage; validated and extensible experimental techniques suitable for
standardization and automation; centralized, open access data repository;
compatibility with existing resources, and tractability with current
informatics technology. We discuss a hypothetical but tractable plan for mouse,
additional efforts for the macaque, and technique development for human. We
estimate that the mouse connectivity project could be completed within five
years with a comparatively modest budget.Comment: 41 page

Combining stereotactic body radiation therapy with immunotherapy: Current data and future directions
Stereotactic body radiation therapy (SBRT) offers excellent local control of early-stage non-small cell lung cancer (NSCLC), but there currently is a need for tolerable systemic therapy to address regional and distant disease progression. One potential option is immunotherapy, which in metastatic NSCLC has shown promise for sustained disease control in a subset of patients. There is also growing evidence for a clinical synergy between radiation and immunotherapy, with several ongoing trials studying the abscopal effect. This review summarizes the current data in the fast-changing field of immuno-radiation therapy, highlighting updates from recent clinical trials
