Contemporary management of atrial fibrillation: what can clinical registries tell us about stroke prevention and current therapeutic approaches?
Relationship between Tibial conformation, cage size and advancement achieved in TTA procedure
Previous studies have suggested that there is a theoretical discrepancy between the cage size and the resultant tibial tuberosity advancement, with the cage size consistently providing less tibial tuberosity advancement than predicted. The purpose of this study was to test and quantify this in clinical cases. The hypothesis was that the advancement of the tibial tuberosity as measured by the widening of the proximal tibia at the tibial tuberosity level after a standard TTA, will be less than the cage sized used, with no particular cage size providing a relative smaller or higher under-advancement, and that the conformation of the proximal tibia will have an influence on the amount of advancement achieved

Academic geriatric medicine in Leicester
.
There has never been a better time to consider joining us. We have recently appointed a
Professor in Geriatric Medicine, alongside Tom Robinson in stroke and Victoria Haunton,
who has just joined as a Senior Lecturer in Geriatric Medicine. We have fantastic
opportunities to support students in their academic pursuits through a well-established
intercalated BSc programme, and routes on through such as ACF posts, and a successful
track-record in delivering higher degrees leading to ACL post. We collaborate strongly
with Health Sciences, including academic primary care. See below for more detail on our
existing academic set-up.
Leicester Academy for the Study of Ageing
We are also collaborating on a grander scale, through a joint academic venture focusing
on ageing, the ‘Leicester Academy for the Study of Ageing’ (LASA), which involves the
local health service providers (acute and community), De Montfort University; University
of Leicester; Leicester City Council; Leicestershire County Council and Leicester Age UK.
Professors Jayne Brown and Simon Conroy jointly Chair LASA and have recently been
joined by two further Chairs, Professors Kay de Vries and Bertha Ochieng. Karen
Harrison Dening has also recently been appointed an Honorary Chair.
LASA aims to improve outcomes for older people and those that care for them that takes
a person-centred, whole system perspective. Our research will take a global perspective,
but will seek to maximise benefits for the people of Leicester, Leicestershire and Rutland,
including building capacity. We are undertaking applied, translational, interdisciplinary
research, focused on older people, which will deliver research outcomes that address
domains from: physical/medical; functional ability, cognitive/psychological; social or
environmental factors. LASA also seeks to support commissioners and providers alike for
advice on how to improve care for older people, whether by research, education or
service delivery. Examples of recent research projects include: ‘Local History Café’
project specifically undertaking an evaluation on loneliness and social isolation; ‘Better
Visits’ project focused on improving visiting for family members of people with dementia
resident in care homes; and a study on health issues for older LGBT people in Leicester.
Clinical Geriatric Medicine in Leicester
We have developed a service which recognises the complexity of managing frail older
people at the interface (acute care, emergency care and links with community services).
There are presently 17 consultant geriatricians supported by existing multidisciplinary
teams, including the largest complement of Advance Nurse Practitioners in the country.
Together we deliver Comprehensive Geriatric Assessment to frail older people with
urgent care needs in acute and community settings.
The acute and emergency frailty units – Leicester Royal Infirmary
This development aims at delivering Comprehensive Geriatric Assessment to frail older
people in the acute setting. Patients are screened for frailty in the Emergency
Department and then undergo a multidisciplinary assessment including a consultant
geriatrician, before being triaged to the most appropriate setting. This might include
admission to in-patient care in the acute or community setting, intermediate care
(residential or home based), or occasionally other specialist care (e.g. cardiorespiratory).
Our new emergency department is the county’s first frail friendly build and includes
fantastic facilities aimed at promoting early recovering and reducing the risk of hospital
associated harms.
There is also a daily liaison service jointly run with the psychogeriatricians (FOPAL); we
have been examining geriatric outreach to oncology and surgery as part of an NIHR
funded study.
We are home to the Acute Frailty Network, and those interested in service developments
at the national scale would be welcome to get involved.
Orthogeriatrics
There are now dedicated hip fracture wards and joint care with anaesthetists,
orthopaedic surgeons and geriatricians. There are also consultants in metabolic bone
disease that run clinics.
Community work
Community work will consist of reviewing patients in clinic who have been triaged to
return to the community setting following an acute assessment described above.
Additionally, primary care colleagues refer to outpatients for sub-acute reviews. You will
work closely with local GPs with support from consultants to deliver post-acute, subacute,
intermediate and rehabilitation care services.

and along with the high standard of vascular surgery locally means one of the best
performances regarding carotid intervention
Influence of Gravity on noncommutative Dirac equation
In this paper, we investigate the influence of gravity and noncommutativity
on Dirac equation. By adopting the tetrad formalism, we show that the modified
Dirac equation keeps the same form. The only modification is in the expression
of the covariant derivative. The new form of this derivative is the product of
its counterpart given in curved space-time with an operator which depends on
the noncommutative -parameter. As an application, we have computed the
density number of the created particles in presence of constant strong electric

Measuring star formation in high-z massive galaxies: A mid-infrared to submillimeter study of the GOODS NICMOS Survey sample
We present measurements of the mean mid-infrared-to-submillimeter flux
densities of massive (M\ast \approx 2 \times 10^11 Msun) galaxies at redshifts
1.7 < z < 2.9, obtained by stacking positions of known objects taken from the
GOODS NICMOS Survey (GNS) catalog on maps: at 24 {\mu}m (Spitzer/MIPS); 70,
100, and 160{\mu}m (Herschel/PACS); 250, 350, 500{\mu}m (BLAST); and 870{\mu}m
(LABOCA). A modified blackbody spectrum fit to the stacked flux densities
indicates a median [interquartile] star-formation rate of SFR = 63 [48, 81]
Msun yr^-1 . We note that not properly accounting for correlations between
bands when fitting stacked data can significantly bias the result. The galaxies
are divided into two groups, disk-like and spheroid-like, according to their
Sersic indices, n. We find evidence that most of the star formation is
occurring in n \leq 2 (disk-like) galaxies, with median [interquartile] SFR =
122 [100,150] Msun yr^-1, while there are indications that the n > 2
(spheroid-like) population may be forming stars at a median [interquartile] SFR
= 14 [9,20] Msun yr^-1, if at all. Finally, we show that star formation is a
plausible mechanism for size evolution in this population as a whole, but find
only marginal evidence that it is what drives the expansion of the

Epidemiology and natural history of central venous access device use and infusion pump function in the NO16966 trial
Background: Central venous access devices in fluoropyrimidine therapy are associated with complications; however, reliable data are lacking regarding their natural history, associated complications and infusion pump performance in patients with metastatic colorectal cancer.<p></p>
Methods: We assessed device placement, use during treatment, associated clinical outcomes and infusion pump perfomance in the NO16966 trial.<p></p>
Results: Device replacement was more common with FOLFOX-4 (5-fluorouracil (5-FU)+oxaliplatin) than XELOX (capecitabine+oxaliplatin) (14.1% vs 5.1%). Baseline device-associated events and post-baseline removal-/placement-related events occurred more frequently with FOLFOX-4 than XELOX (11.5% vs 2.4% and 8.5% vs 2.1%). Pump malfunctions, primarily infusion accelerations in 16% of patients, occurred within 1.6–4.3% of cycles. Fluoropyrimidine-associated grade 3/4 toxicity was increased in FOLFOX-4-treated patients experiencing a malfunction compared with those who did not (97 out of 155 vs 452 out of 825 patients), predominantly with increased grade 3/4 neutropenia (53.5% vs 39.8%). Febrile neutropenia rates were comparable between patient cohorts±malfunction. Efficacy outcomes were similar in patient cohorts±malfunction.<p></p>
Conclusions: Central venous access device removal or replacement was common and more frequent in patients receiving FOLFOX-4. Pump malfunctions were also common and were associated with increased rates of grade 3/4 haematological adverse events. Oral fluoropyrimidine-based regimens may be preferable to infusional 5-FU based on these findings
Combining quantitative and qualitative survey work. Methodological framework, practical issues, and case studies
This document is an output of the DFID funded research project R7033 "Methodological Framework for Integrating Qualitative and Quantitative Approaches for Socio-Economic Survey Work". The project's main objective was to improve survey methodologies used in the natural resources sector, leading in turn to improved quality of information required for decision-making at the various stages of natural resources programmes and projects. The project, which was jointly implemented by the Natural Resources Institute and the Statistical Services Centre, attempted to bridge the gap between informal I qualitative methods on the one hand and formal I quantitative ones on the other. The document is presented in the form of a manual, allowing the user to read each of its parts and sections independently of each other. Nevertheless, cross references indicate links to other parts of the document where appropriate. Part I provides a methodological framework, which will also be available as a Best-Practice Guideline published by the Natural Resources Systems Programme of DFID. Part 11, which contains six theme papers, looks into a number of practical issues encountered when combining quantitative and qualitative survey techniques. And finally, in Part III case studies are used to demonstrate how the two approaches can be integrated in survey work and experiments related to the natural resources sector. The framework document, theme papers and case studies have been produced between 1998 and 2000
Large Scale Structure of the Universe
Galaxies are not uniformly distributed in space. On large scales the Universe
displays coherent structure, with galaxies residing in groups and clusters on
scales of ~1-3 Mpc/h, which lie at the intersections of long filaments of
galaxies that are >10 Mpc/h in length. Vast regions of relatively empty space,
known as voids, contain very few galaxies and span the volume in between these
structures. This observed large scale structure depends both on cosmological
parameters and on the formation and evolution of galaxies. Using the two-point
correlation function, one can trace the dependence of large scale structure on
galaxy properties such as luminosity, color, stellar mass, and track its
evolution with redshift. Comparison of the observed galaxy clustering
signatures with dark matter simulations allows one to model and understand the
clustering of galaxies and their formation and evolution within their parent
dark matter halos. Clustering measurements can determine the parent dark matter
halo mass of a given galaxy population, connect observed galaxy populations at
different epochs, and constrain cosmological parameters and galaxy evolution
models. This chapter describes the methods used to measure the two-point
correlation function in both redshift and real space, presents the current
results of how the clustering amplitude depends on various galaxy properties,
and discusses quantitative measurements of the structures of voids and
filaments. The interpretation of these results with current theoretical models

Kepler eclipsing binary stars. VII. the catalogue of eclipsing binaries found in the entire Kepler data set
The primary Kepler Mission provided nearly continuous monitoring of ~200,000 objects with unprecedented photometric precision. We present the final catalog of eclipsing binary systems within the 105 deg2 Kepler field of view. This release incorporates the full extent of the data from the primary mission (Q0-Q17 Data Release). As a result, new systems have been added, additional false positives have been removed, ephemerides and principal parameters have been recomputed, classifications have been revised to rely on analytical models, and eclipse timing variations have been computed for each system. We identify several classes of systems including those that exhibit tertiary eclipse events, systems that show clear evidence of additional bodies, heartbeat systems, systems with changing eclipse depths, and systems exhibiting only one eclipse event over the duration of the mission. We have updated the period and galactic latitude distribution diagrams and included a catalog completeness evaluation. The total number of identified eclipsing and ellipsoidal binary systems in the Kepler field of view has increased to 2878, 1.3% of all observed Kepler targets
