Profiling the mental health of diabetic patients: a cross-sectional survey of Zimbabwean patients
Objective
The burden of diabetes mellitus has exponentially increased in low resource settings. Patients with diabetes are more likely to exhibit poor mental health which negatively affects treatment outcomes. However, patients with high levels of social support (SS) are likely to report optimal mental health. We sought to determine how SS affects the report of psychiatric morbidity and health-related quality of life (HRQoL) in 108 diabetic patients in Harare, Zimbabwe.
Results
The average age of participants was 54.1 (SD 18.6) years. Most of the participants were; females (69.4%), married (51.9%), and were of low level of income (43.5%). 37.1% of the participants exhibited signs of psychiatric morbidity [mean Shona Symptoms Questionnaire score—6.7 (SD 3.2)]. Further, patients also reported lower HRQoL [mean EQ-5D-VAS score—64.1 (SD 15.3)] and high levels of SS [mean Multidimensional Scale of Perceived Social Support score—43.7 (SD 11.5)]. Patients who received greater amount of SS had optimal mental health. Being female, unmarried, lower education attainment, having more comorbid conditions, being diagnosed with type 2 diabetes and having been diagnosed of diabetes for a longer duration were associated with poorer mental health. It is important to develop context-specific interventions to improve diabetic patients’ mental health

Understanding the influence of a downstream-side bus stop on saturation flows at an isolated junction
This research investigates how saturation flow is affected by bus stops and analyses whether the standard equation used in the UK is adequate for estimating the saturation flow of an approach, especially in the presence of a downstream-side bus stop. As part of the study, we undertook a survey of saturation flows at several junctions in the city of Leeds in England and seek to explain the factors affecting them, taking into account the bus stop located nearby. We develop bootstrapping regression models to explain the difference between the observed and estimated saturation flows and propose an extension to the standard model, accounting for the bus stop located nearby. Finally, this paper illustrates the methods developed and reports on how performance can be improved by reconfiguring a junction
PI Controller untuk Mengatur Kecepatan Motor Induksi 1 Fasa
Induction motors are widely used in the industrial world, home-based businesses as well as in households. Currently in the process of making tofu an induction motor is used as a motor to drive soy blending blades. At this time the use of induction motors is still manually by requiring the operator to regulate the speed of the motor. To reduce operator work, it is necessary to apply PI control as a motor speed controller so that a constant motor rotation is obtained. 1 phase induction motor can be adjusted with variable speed drive (VSD) 0.75KW 1 phase. Blending blade drive uses 0.5HP 1 phase induction motor. In the application of PI control requires some hardware namely Arduino Uno as a minimum system that gives PWM circuit input commands. And the speed sensor as a motor blending speed reader. PI tuning values obtained from the application of the Ziegerl-Nichols I method with the best Kp and Ki tuning values are 1.35 and 0.02673. This research was conducted with 3 speed variables namely 1400 rpm, 1300 and 1200 rpm. From the application of Kp and Ki tuning values, the smallest error value is 4.08% at 1400 rpm with the system response time peak (tp) 5s, rise time (tr) 3s Faster, delay time (td) 3s, and settling time (ts) 9s , and a maximum overshoot of 9.8%
Sustainable financing for vaccination towards advancing universal health coverage in the WHO African region: The strategic role of national health insurance
ABSTRACTThere is a growing political interest in health reforms in Africa, and many countries are choosing national health insurance as their main financing mechanism for universal health coverage. Although vaccination is an essential health service that can influence progress toward universal health coverage, it is not often prioritized by these national health insurance systems. This paper highlights the potential gains of integrating vaccination into the package of health services that is provided through national health insurance and recommends practical policy actions that can enable countries to harness these benefits at population level

Investigating the effect of remote ischaemic preconditioning on biomarkers of stress and injury-related signalling in patients having isolated coronary artery bypass grafting or aortic valve replacement using cardiopulmonary bypass:study protocol for a randomized controlled trial
BACKGROUND: Ischaemia-reperfusion injury occurs during heart surgery that uses cardiopulmonary bypass (CPB) and cardioplegic arrest. It is hypothesised that remote ischaemic preconditioning (RIPC) protects the heart against such injury. Despite the numerous studies investigating the protective effects of RIPC, there is still uncertainty about the interpretation of the findings as well as conflicting results between studies. The objective of this trial is to investigate the cardioprotective effect of RIPC in patients having coronary artery bypass grafting (CABG) or aortic valve replacement surgery. This will be achieved by estimating the effect of the intervention in the two groups of pathologies and by investigating the signalling mechanisms that may underpin the cardioprotective effect. METHODS/DESIGN: A two-centre randomised controlled trial will be used to investigate the effects of RIPC in two pathologies: patients having isolated CABG and those having aortic valve replacement surgery (AVR) with CPB. Participants will be randomised to RIPC or control (sham RIPC), stratified by surgical stratum. The intervention will be delivered by a research nurse. Data will be collected by a research nurse blinded to the intervention. The patient and the theatre staff are also blinded to the allocation. Markers of myocardial injury and inflammation will be measured in myocardial biopsies and in blood samples at different times. DISCUSSION: This trial is designed to investigate whether RIPC will reduce myocardial injury and inflammation following heart surgery and whether there is a difference in effect between participants having CABG or AVR. This trial is a unique opportunity to study the mechanisms associated with RIPC using human myocardial tissue and blood, and to relate these to the extent of myocardial injury/protection.
