    Quality of life in children with psychiatric disorders

    The objective of the study described in this thesis was to examine several aspects of quality of life (QoL) of children with psychiatric disorders. In Chapter 1, the background and the main aims were presented. Because treatment effects for severe and chronic diseases have been improved in the last decades, QoL emerged as an important outcome measure. For the present thesis, a longitudinal study with two assessments across a one-year follow-up period was conducted among 310 children and adolescents (aged 6-18 years) who had been referred to outpatient mental health agencies. Besides, a community sample of 74 children without psychiatric problems was included. The aims of this study were: (1) to assess the usefulness of the Pediatric Quality of Life Inventory (PedsQL) as an instrument measuring QoL in children with psychiatric problems, (2) to compare QoL of children with psychiatric problems to QoL of children from the general population without psychiatric problems, (3) to compare QoL of children with different psychiatric disorders, (4) to identify factors that are associated with QoL, (5) to determine the course of QoL, and (6) to identify predictors of change in QoL across time

    Psychopathology in children: Improvement of quality of life without psychiatric symptom reduction

    Objective: The aim of this study was to assess the association between change in psychopathology and Quality of Life (QoL) across time in children with high levels of psychopathology. Methods: A referred sample of 126 seven- to 19-year-olds was studied across a 1-year follow-up period. Information concerning QoL and psychopathology was obtained from parents. Results: Overall, 38.1% of children showed neither psychiatric symptom reduction nor QoL improvement, 33.3% of children showed both a clinically significant psychiatric symptom reduction and QoL improvement, and 28.6% of children showed either psychiatric symptom reduction or QoL improvement. In 11.1% of all children,QoL improved, while the level of psychopathology remained high. Age, gender, or psychiatric diagnosis did not predict a poor outcome of persistently high psychopathology scores and poor QoL. Conclusion: QoL in children with psychiatric problems may be improved by reducing psychiatric symptoms in a number of children, but it is also possible to improve QoL without psychiatric symptom reduction. This implicates that QoL should become an important aim and treatment outcome measure of psychiatric treatment programs, especially since psychopathology tends to persist. © Steinkopff Verlag 2005

    Correcting for the Effects of Interstellar Extinction

    This paper addresses the issue of how best to correct astronomical data for the wavelength-dependent effects of Galactic interstellar extinction. The main general features of extinction from the IR through the UV are reviewed, along with the nature of observed spatial variations. The enormous range of extinction properties found in the Galaxy, particularly in the UV spectral region, is illustrated. Fortunately, there are some tight constraints on the wavelength dependence of extinction and some general correlations between extinction curve shape and interstellar environment. These relationships provide some guidance for correcting data for the effects of extinction. Several strategies for dereddening are discussed along with estimates of the uncertainties inherent in each method. In the Appendix, a new derivation of the wavelength dependence of an average Galactic extinction curve from the IR through the UV is presented, along with a new estimate of how this extinction law varies with the parameter R = A(V)/E(B-V). These curves represent the true monochromatic wavelength dependence of extinction and, as such, are suitable for dereddening IR--UV spectrophotometric data of any resolution, and can be used to derive extinction relations for any photometry system.Comment: To appear in PASP (January 1999) 14 pages including 4 pages of figures Uses emulateapj style.     Phase diagram of the restricted solid-on-solid model coupled to the Ising model

    We study the phase transitions of a restricted solid-on-solid model coupled to an Ising model, which can be derived from the coupled XY-Ising model. There are two kinds of phase transition lines. One is a Ising transition line and the other is surface roughening transition line. The latter is a KT transition line from the viewpoint of the XY model. Using a microcanonical Monte Carlo technique, we obtain a very accurate two dimensional phase diagram. The two transition lines are separate in all the parameter space we study. This result is strong evidence that the fully frustrated XY model orders by two separate transitions and that roughening and reconstruction transitions of crystal surfaces occur separately.Comment: 17 pages, source RevTeX file and 8 PS figures are tarred and compressed via uufile

    Disordered Flat Phase and Phase Diagram for Restricted Solid on Solid Models of Fcc(110) Surfaces

    We discuss the results of a study of restricted solid-on-solid models for fcc (110) surfaces. These models are simple modifications of the exactly solvable BCSOS model, and are able to describe a (2×1)(2\times 1) missing-row reconstructed surface as well as an unreconstructed surface. They are studied in two different ways. The first is by mapping the problem onto a quantum spin-1/2 one-dimensional hamiltonian of the Heisenberg type, with competing SizSjzS^z_iS^z_j couplings. The second is by standard Monte Carlo simulations. We find phase diagrams with the following features, which we believe to be quite generic: (i) two flat, ordered phases (unreconstructed and missing-row reconstructed); a rough, disordered phase; an intermediate disordered flat (DF) phase, characterized by monoatomic steps, whose physics is shown to be akin to that of a dimer spin state. (ii) a transition line from the (2×1)(2\times 1) reconstructed phase to the DF phase showing exponents which appear to be close, within our numerical accuracy, to the 2D-Ising universality class. (iii) a critical (preroughening) line with variable exponents, separating the unreconstructed phase from the DF phase.     Thermal roughening of an SOS-model with elastic interaction

    We analyze the effects of a long-ranged step-step interaction on thermal roughening within the framework of a solid-on-solid model of a crystal surface by means of Monte Carlo simulation. A repulsive step-step interaction is modeled by elastic dipoles located on sites adjacent to the steps. In order to reduce the computational effort involved in calculating interaction energy based on long-ranged potentials, we employ a multi-grid scheme. As a result of the long-range character of the step interaction, the roughening temperature increases drastically compared to a system with short-range cutoff as a consequence of anti-correlations between surface defects

    Tuning Properties of Iron Oxide Nanoparticles in Aqueous Synthesis without Ligands to Improve MRI Relaxivity and SAR.

    Aqueous synthesis without ligands of iron oxide nanoparticles (IONPs) with exceptional properties still remains an open issue, because of the challenge to control simultaneously numerous properties of the IONPs in these rigorous settings. To solve this, it is necessary to correlate the synthesis process with their properties, but this correlation is until now not well understood. Here, we study and correlate the structure, crystallinity, morphology, as well as magnetic, relaxometric and heating properties of IONPs obtained for different durations of the hydrothermal treatment that correspond to the different growth stages of IONPs upon initial co-precipitation in aqueous environment without ligands. We find that their properties were different for IONPs with comparable diameters. Specifically, by controlling the growth of IONPs from primary to secondary particles firstly by colloidal and then also by magnetic interactions, we control their crystallinity from monocrystalline to polycrystalline IONPs, respectively. Surface energy minimization in the aqueous environment along with low temperature treatment is used to favor nearly defect-free IONPs featuring superior properties, such as high saturation magnetization, magnetic volume, surface crystallinity, the transversal magnetic resonance imaging (MRI) relaxivity (up to r₂ = 1189 mM(-1)·s(-1) and r₂/r₁ = 195) and specific absorption rate, SAR (up to 1225.1 W·gFe(-1))

    Abundances and Physical Conditions in the Warm Neutral Medium Towards mu Columbae

    We present ultraviolet interstellar absorption line measurements for the sightline towards the O9.5 V star mu Columbae obtained with the Goddard High Resolution Spectrograph (GHRS) on board the Hubble Space Telescope. These archival data represent the most complete GHRS interstellar absorption line measurements for any line of sight towards an early-type star. The 3.5 km/s resolution of the instrument allow us to accurately derive the gas-phase column densities of many important ionic species in the diffuse warm neutral medium using a combination of apparent column density and component fitting techniques, and we study in detail the contamination from ionized gas along this sightline. The low-velocity material shows gas-phase abundance patterns similar to the warm cloud (cloud A) towards the disk star zeta Oph, while the component at v = +20.1 km/s shows gas-phase abundances similar to those found in warm halo clouds. We find the velocity-integrated gas-phase abundances of Zn, P, and S relative to H along this sightline are indistinguishable from solar system abundances. We discuss the implications of our gas-phase abundance measurements for the composition of interstellar dust. The relative ionic column density ratios of the intermediate velocity components show the imprint both of elemental incorporation into grains and (photo)ionization. The components at v = -30 and -48 km/s along this sightline likely trace shocked gas with very low hydrogen column densities. Appendices include a new derivation of the GHRS instrumental line spread function, and a new very accurate determination of the total H I column along this sightline.     Determinants of quality of life in children with psychiatric disorders

    Objective: To assess factors that, in addition to childhood psychopathology, are associated with Quality of Life (QoL) in children with psychiatric problems. Methods: In a referred sample of 252 8 to 18-year-olds, information concerning QoL, psychopathology and a broad range of child, parent, and family/ social network factors was obtained from children, parents, teachers and clinicians. Results: Poor child, parent, and clinician reported QoL was associated with child psychopathology, but given the presence of psychopathology, also with child factors, such as low self-esteem, and poor social skills, and family/social network factors, such as poor family functioning, and poor social support. In multiple linear regression analyses the importance of parent factors, such as parenting stress, was almost negligible. Conclusion: To increase QoL of children with psychiatric problems, treatment of symptoms is important, but outcome might improve if treatment is also focussed on other factors that may affect QoL. Results are discussed in relation to current treatment programs. © Springer 2005

    Lack of high BMI-related features in adipocytes and inflammatory cells in the infrapatellar fat pad (IFP)

    BACKGROUND: Obesity is associated with the development and progression of osteoarthritis (OA). Although the infrapatellar fat pad (IFP) could be involved in this association, due to its intracapsular localization in the knee joint, there is currently little known about the effect of obesity on the IFP. Therefore, we investigated cellular and molecular body mass index (BMI)-related features in the IFP of OA patients. METHODS: Patients with knee OA (N = 155, 68% women, mean age 65 years, mean (SD) BMI 29.9 kg/m2 (5.7)) were recruited: IFP volume was determined by magnetic resonance imaging in 79 patients with knee OA, while IFPs and subcutaneous adipose tissue (SCAT) were obtained from 106 patients undergoing arthroplasty. Crown-like structures (CLS) were determined using immunohistochemical analysis. Adipocyte size was determined by light microscopy and histological analysis. Stromal vascular fraction (SVF) cells were characterized by flow cytometry. RESULTS: IFP volume (mean (SD) 23.6 (5.4) mm(3)) was associated with height, but not with BMI or other obesity-related features. Likewise, volume and size of IFP adipocytes (mean 271 pl, mean 1933 μm) was not correlated with BMI. Few CLS were observed in the IFP, with no differences between overweight/obese and lean individuals. Moreover, high BMI was not associated with higher SVF immune cell numbers in the IFP, nor with changes in their phenotype. No BMI-associated molecular differences were observed, besides an increase in TNFα expression with high BMI. Macrophages in the IFP were mostly pro-inflammatory, producing IL-6 and TNFα, but little IL-10. Interestingly, however, CD206 and CD163 were associated with an anti-inflammatory phenotype, were the most abundantly expressed surface markers on macrophages (81% and 41%, respectively) and CD163(+) macrophages had a more activated and pro-inflammatory phenotype than their CD163(-) counterparts. CONCLUSIONS: BMI-related features usually observed in SCAT and visceral adipose tissue could not be detected in the IFP of OA patients, a fat depot implicated in OA pathogenesis
