    Potential effects of warmer worms and vectors on onchocerciasis transmission in West Africa

    Development times of eggs, larvae and pupae of vectors of onchocerciasis (Simulium spp.) and of Onchocerca volvulus larvae within the adult females of the vectors decrease with increasing temperature. At and above 25C,the parasite could reach its infective stage in less than 7 days when vectors could transmit after only two gonotrophic cycles. After incorporating exponential functions for vector development into a novel blackfly population model, it was predicted that fly numbers in Liberia and Ghana would peak at air temperatures of 29C and 34C, about 3C and 7C above current monthly averages, respectively; parous rates of forest flies (Liberia) would peak at 298C and of savannah flies (Ghana) at 308C. Small temperature increases (less than 28C) might lead to changes in geographical distributions of different vector taxa. When the new model was linked to an existing framework for the population dynamics of onchocerciasis in humans and vectors, transmission rates and worm loads were projected to increase with temperature to at least 338C. By contrast, analyses of field data on forest flies in Liberia and savannah flies in Ghana, in relation to regional climate change predictions, suggested, on the basis of simple regressions, that 13–41% decreases in fly numbers would be expected between the present and before 2040. Further research is needed to reconcile these conflicting conclusions

    “We now have a patient and not a criminal” : An exploratory study of judges and lawyers’ views on suicide attempters and the law in Ghana

    This study explored the views of judges and lawyers of the superior courts of Ghana on the law criminalizing attempted suicide. Qualitative data were collected from 12 experienced legal practitioners of the superior courts (five judges and seven lawyers) using a semi-structured interview schedule. Thematic analysis of the data yielded three main perspectives: In defence of the Law, Advocating a Repeal, and Pro-Health Orientation. Although exploratory, the findings of this study offer cues for stepping up suicide literacy and advocacy programmes toward either a repeal of the law or a reform

    A prospective, multicenter, phase I matched-comparison group trial of safety, pharmacokinetics, and preliminary efficacy of riluzole in patients with traumatic spinal cord injury.

    A prospective, multicenter phase I trial was undertaken by the North American Clinical Trials Network (NACTN) to investigate the pharmacokinetics and safety of, as well as obtain pilot data on, the effects of riluzole on neurological outcome in acute spinal cord injury (SCI). Thirty-six patients, with ASIA impairment grades A-C (28 cervical and 8 thoracic) were enrolled at 6 NACTN sites between April 2010 and June 2011. Patients received 50 mg of riluzole PO/NG twice-daily, within 12 h of SCI, for 14 days. Peak and trough plasma concentrations were quantified on days 3 and 14. Peak plasma concentration (Cmax) and systemic exposure to riluzole varied significantly between patients. On the same dose basis, Cmax did not reach levels comparable to those in patients with amyotrophic lateral sclerosis. Riluzole plasma levels were significantly higher on day 3 than on day 14, resulting from a lower clearance and a smaller volume of distribution on day 3. Rates of medical complications, adverse events, and progression of neurological status were evaluated by comparison with matched patients in the NACTN SCI Registry. Medical complications in riluzole-treated patients occurred with incidences similar to those in patients in the comparison group. Mild-to-moderate increase in liver enzyme and bilirubin levels were found in 14-70% of patients for different enzymes. Three patients had borderline severe elevations of enzymes. No patient had elevated bilirubin on day 14 of administration of riluzole. There were no serious adverse events related to riluzole and no deaths. The mean motor score of 24 cervical injury riluzole-treated patients gained 31.2 points from admission to 90 days, compared to 15.7 points for 26 registry patients, a 15.5-point difference (p=0.021). Patients with cervical injuries treated with riluzole had more-robust conversions of impairment grades to higher grades than the comparison group

    Studies of Anopheles gambiae s.l (Diptera: Culicidae) exhibiting different vectorial capacities in lymphatic filariasis transmission in the Gomoa district, Ghana

    <p>Abstract</p> <p>Background</p> <p>Two lymphatic filariasis endemic communities Mampong and Hwida in Ghana have been regularly monitored for impact on transmission after annual mass drug administration (MDA) with albendazole and ivermectin. After six MDAs even though the ABR for Mampong was 55883/person/year and that of Hwida was 2494/person/year, they both had ATPs of 15.21 infective larvae/person/year. Interestingly the human microfilaraemia levels had reduced significantly from 14% to 0% at Mampong and 12% to 3% at Hwida. In an attempt to understand this anomaly, we collected mosquitoes over a 5-month period using human landing catches to determine the species composition, the number of cibarial teeth, the lengths and widths of the cibarium and the cibarial dome of the vector populations.</p> <p>Results</p> <p>Out of 2553 mosquitoes caught at Mampong, 42.6% were <it>An. gambiae </it>s.l. All 280 identified further by PCR were <it>An. gambiae </it>s.s (275 M and 5 S molecular forms). At Hwida, 112 mosquitoes were obtained; 67 (59.8%) were <it>An. gambiae </it>s.l, comprised of 40 (59.7%) <it>An. melas</it>, 24 (35.8%) <it>An. gambiae </it>s.s (17 and 5 M and S molecular forms respectively) and 3 (4.5%) unidentified. The mean number of teeth for <it>An. melas </it>was 14.1 (median = 14, range = 12-15), <it>An. gambiae </it>s.s., 15.7 (median = 15, range = 13-19) M form 15.5 (median = 15 range = 13-19) and S form 16 (median = 16, range 15-17). The observed differences in teeth numbers were significantly different between <it>An. melas </it>and <it>An. gambiae </it>s.s (p = 0.004), and the M form (p = 0.032) and the S form (p = 0.002).</p> <p>Conclusions</p> <p>In this study, <it>An. gambiae </it>s.s was the main vector at Mampong and was found to possess significantly more cibarial teeth than <it>An. melas</it>, the principal vector at Hwida. We postulate that the different impact observed after 6 MDAs may be due to <it>An. gambiae </it>s.s exhibiting 'facilitation' at Mampong and at Hwida <it>An. melas </it>the main vector exhibits 'limitation'. Thus it may be necessary to compliment MDA with vector control to achieve interruption of transmission in areas where <it>An. melas </it>may exhibit limitation.</p

    Does increasing treatment frequency address sub-optimal responses to ivermectin for the control and elimination of river blindness?

    Background Several African countries have adopted a biannual ivermectin distribution strategy in some foci to control and eliminate onchocerciasis. In 2010, the Ghana Health Service started biannual distribution to combat transmission hotspots and sub-optimal responses to treatment. We assessed the epidemiological impact of the first 3 years of this strategy and quantified responses to ivermectin over two consecutive rounds of treatment in 10 sentinel communities. Methods We evaluated Onchocerca volvulus microfilarial intensity and prevalence in those aged +/-20 years before the first, second and fifth (or sixth) biannual treatment rounds using skin snip data from 956 participants. We used longitudinal regression modelling to estimate rates of microfilarial repopulation of the skin in a cohort of 217 participants who were followed-up over the first two rounds of biannual treatment. Results Biannual treatment has had a positive impact, with substantial reductions in infection intensity after 4 or 5 rounds in most communities. We identified three communities—all having been previously recognised as responding sub-optimally to ivermectin—with statistically significantly high microfilarial repopulation rates. We did not find any clear association between microfilarial repopulation rate and the number of years of prior intervention, coverage, or the community level of infection. Conclusions The strategy of biannual ivermectin treatment in Ghana has reduced O. volvulus microfilarial intensity and prevalence, but sub-optimal responses to treatment remain evident in a number of previously and consistently implicated communities. Whether increasing the frequency of treatment will be sufficient to meet the World Health Organization’s 2020 elimination goals remains uncertain

    Environmental Impact of Poultry Manure Gasification Technology for Energy and Ash Valorization

    Thermochemical technologies offer potential solutions for energy recovery and mitigating the environmental impacts of biomass waste. Poultry manure (PM), a nutrient-rich biomass but also a potentially problematic biomass waste, presents an opportunity for recovery and recycling. This study compares the environmental performance of a real-scale novel gasification technology called Chimera (designed and developed through an EU LIFE program) in locally treating PM with anaerobic digestion (AD) and incineration. Using life cycle assessment (LCA), the potential environmental impacts of the technologies were assessed using the Environmental Footprint (EF) 3.0 midpoint life cycle impact assessment method. We performed an attributional LCA with substitution. The selected functional unit (FU) is the treatment of one tonne (1000 kg) PM at 40% dry matter in the Netherlands in 2021 for 20 years. The LCA results of the three technologies compared showed that no single technology outperformed the other across all the impact categories. Climate change scores for the various technologies were −383 (incineration), −206 (Chimera), and −161 (anaerobic digestion) kg CO2 eq./FU. The results were influenced mainly by the potential utilization of the substituted heat and electricity. This study expands the existing literature on environmental sustainability assessments of PM treatment technologies. It underscores the prospects for these technologies to promote circularity while also indicating the bottlenecks for the potential environmental impacts and highlighting the most sensitive aspects that can influence the environmental performance of these technologies

    The Airbus bribery scandal: A collective myopia perspective

Drawing on collective myopia as a lens, we explore the infamous Airbus bribery scandal to show how the executives of the global aircraft manufacturer, through their actions and behaviours, institutionalised the payment of bribes to secure contracts. Data for the inquiry consist of publicly available court-approved documents, company website and internal emails, and newspaper articles on the scandal. Unpacking the bribery scheme operated by Airbus, we found that bribing of foreign government officials and airline executives to secure contracts was part and parcel of the firm's organising strategy. In this regard, the organising practices of Airbus actively encouraged employees to break its own bribery compliance policies which they employed as smokescreens to cover their illegal activities. Building on our findings, we developed a collective myopic-bribery framework outlining how the collective myopia in organising drove the bribery activities at Airbus.     Environmental performance of chocolate produced in ghana using life cycle assessment

    Ghana is an important cocoa producer and exporter and this production is of high economic importance. Increasing interest in the sustainable productions of cocoa/chocolate necessitated the need to assess the environmental impacts associated with the production of different chocolate variants (extra dark (EDC), dark (DC), milk (MC) and flavoured milk (FMC) in Ghana, including the identification of environmental hotspots for improvement. The life cycle assessment tool was used following the CML_IA and CED impact assessment methods. EDC had the lowest scores for most of the impact categories while FMC was most impactful. For Global Warming Potential (GWP), EDC and FMC were estimated to be 1.61 kg CO2 eq. and 4.21 kg CO2 eq., respectively. CED ranged from 1.44 × 102 to 1.50 × 102 MJ-eq. Chocolate manufacturing phase was generally more impactful than cocoa cultivation due to high emissions from milk and sugar production. The impact scores for 100 g packaged chocolate bar were the lowest in comparison to 300 g chocolate pouches and 12.5 g packaged chocolate strips. GWP for 100 g and 12.5 g were 0.20 kg CO2 eq. and 0.39 kg CO2 eq., respectively. Comparing different destination points for the manufactured chocolate, impact scores for the international destination were similar to those recorded for local destinations.
