Common features between neoplastic and preneoplastic lesions of the biliary tract and the pancreas
The bile duct system and pancreas show many similarities due to their anatomical proximity and common embryological origin. Consequently, preneoplastic and neoplastic lesions of the bile duct and pancreas share analogies in terms of
molecular, histological and pathophysiological features. Intraepithelial neoplasms are reported in biliary tract, as biliary intraepithelial neoplasm (BilIN), and in pancreas, as pancreatic intraepithelial neoplasm (PanIN). Both can evolve
to invasive carcinomas, respectively cholangiocarcinoma (CCA) and pancreatic ductal adenocarcinoma (PDAC). Intraductal papillary neoplasms arise in biliary tract and pancreas. Intraductal papillary neoplasm of the biliary tract (IPNB)
share common histologic and phenotypic features such as pancreatobiliary, gastric, intestinal and oncocytic types, and biological behavior with the pancreatic counterpart, the intraductal papillary mucinous neoplasm of the pancreas (IPMN). All these neoplastic lesions exhibit similar immunohistochemical phenotypes, suggesting a common carcinogenic process.
Indeed, CCA and PDAC display similar clinic-pathological features as growth pattern, poor response to conventional chemotherapy and radiotherapy and, as a consequence, an unfavorable prognosis. Meta-analysis of genome-wide association studies of asthma in ethnically diverse North American populations.
Asthma is a common disease with a complex risk architecture including both genetic and environmental factors. We performed a meta-analysis of North American genome-wide association studies of asthma in 5,416 individuals with asthma (cases) including individuals of European American, African American or African Caribbean, and Latino ancestry, with replication in an additional 12,649 individuals from the same ethnic groups. We identified five susceptibility loci. Four were at previously reported loci on 17q21, near IL1RL1, TSLP and IL33, but we report for the first time, to our knowledge, that these loci are associated with asthma risk in three ethnic groups. In addition, we identified a new asthma susceptibility locus at PYHIN1, with the association being specific to individuals of African descent (P = 3.9 × 10(-9)). These results suggest that some asthma susceptibility loci are robust to differences in ancestry when sufficiently large samples sizes are investigated, and that ancestry-specific associations also contribute to the complex genetic architecture of asthma
Optimal hydraulic energy harvesting strategy for PaT installation in Water Distribution Networks
Water Distribution Networks (WDNs) represent a noteworthy field for possible implementation of Small Hydropower (SHP), by replacing Pressure Reduction Valves (PRV) with turbomachines, in particular Pump as Turbines (PaTs), to control and regulate the pressure, while harvesting energy otherwise wasted. Different models were developed to predict the performance and select the positioning of the PaTs for the maximum energy recovery but most of them neglect practical aspect such as: power grid limitations and optimal harvesting strategy. In this framework, we intend to propose a new method to select a PaT, defining its optimal working point, by introducing an energy exploitation coefficient. The proposed methodology is based on the experimental results of a real PaT tested in the high capacity hydraulic laboratory at Polytechnic University of Bari. Firstly, the selected commercial centrifugal pump was tested in both pump and turbine modes. Then, three different approaches, for the Best Efficiency Point (BEP) selection, are described and compared in terms of energy exploitation and capacity factor for a WDN. The first consists of selecting the BEP at the average flow rate, the second one considers the probability distribution of the flow rate and the corresponding available hydraulic energy, whereas the latter is based on the highest energy harvesting. By applying energy production, economic and environmental analyses, the new proposed methodology, based on the third approach, shows a remarkable advantage in terms of exploited energy. Indeed a remarkable 60% energy recovery is achieved with 334 ton CO2/year avoided. Furthermore, the impact of the electrical motor on the maximum power generation (cut-off) is considered. Eventually, useful insights for the future PaT selection and installation are discussed
Clinical application of mindfulness-oriented meditation: A preliminary study in children with ADHD
Mindfulness-oriented meditation (MOM) is a self-regulatory training used for attentional and behavioral problems. With its focus on attention, MOM is a promising form of training that is gaining empirical support as a complementary or alternative intervention for attention deficit/hyperactivity disorder (ADHD). In this study, we tested the preliminary efficacy of MOM training in children with ADHD, by comparing its efficacy with an active control condition (Emotion Education Program, EEP). Twenty-five children with ADHD aged 7\u201311 years participated in MOM training (n = 15) or EEP (n = 10) 3 times per week for 8 weeks. Neuropsychological and academic measures and behavioral, emotional, and mindfulness ratings were collected before and after the two programs. On average, MOM training had positive effects on neuropsychological measures, as evidenced by a significant mean improvement in all outcome measures after training. Moreover, positive effects on ADHD symptoms were found only in the MOM group. Although they are preliminary, our results documented that MOM training promotes changes in neuropsychological measures and in certain behavioral symptoms, suggesting it as a promising tool for ameliorating cognitive and clinical manifestations of ADHD
Study of the Failure Mechanism of a High-Density Polyethylene Liner in a Type IV High-Pressure Storage Tank
The use of Type IV cylinders for gas storage is becoming more widespread in various sectors, especially in transportation, owing to the lightweight nature of this type of cylinder, which is composed of a polymeric liner that exerts a barrier effect and an outer composite material shell that primarily imparts mechanical strength. In this work, the failure analysis of an HDPE liner in a Type IV cylinder for high-pressure storage was carried out. The breakdown occurred during a cyclic pressure test at room temperature and manifested in the hemispherical head area, as cracks perpendicular to the liner pinch-off line. The failed sample was thoroughly investigated and its characteristics were compared with those of other liners at different stages of production of a Type IV cylinder (blow molding, curing of the composite material). An examination of the liner showed that no significant chemical and morphological changes occurred during the production cycle of a Type IV cylinder that could justify the liner rupture, and that the most likely cause of failure was a design-related fatigue phenomenon
Effect of early and current Helicobacter pylori infection on the risk of anaemia in 6.5-year-old Ethiopian children
Background: Epidemiological and clinical studies in high income countries have suggested that Helicobacter pylori
(H. pylori) may cause anaemia, but evidence is lacking from low income countries.We examined associations between H. pylori infection in early childhood and anaemia at the age of 6.5 years in an Ethiopian birth cohort.
Methods: In 2011/12, 856 children (85.1 % of the 1006 original singletons in a population-based birth cohort) were followed up at age six and half. An interviewer-led questionnaire administered to mothers provided information on demographic and lifestyle variables. Haemoglobin level and red cell indices were examined using an automated haematological analyzer (Cell Dyn 1800, Abbott, USA), and stool samples analyzed for H. pylori antigen. The independent effects of H. pylori infection (measured at age 3.5 and 6.5 years) on anaemia, haemoglobin level, and red cell indices (measured at age 6.5 years) were determined using multiple logistic and linear regression.
Results: The prevalence of anemia was 34.8 % (257/739), and the mean (SD) haemoglobin concentration was 11.8 (1.1) gm/dl. Current H. pylori infection at age 6.5 years was positively, though not significantly related to prevalence of anaemia (adjusted OR, 95 % CI, 1.15; 0.69, 1.93, p = 0.59). Any H. pylori infection up to age 6.5 years was significantly associated with an increased risk of anaemia at age 6.5 (adjusted OR, 95 % CI, 1.68; 1.22, 2.32, p = 0.01). A significant reduction in haemoglobin concentration and red cell indices was also observed among children who had any H. pylori infection up to age 6.5 (Hb adjusted β = −0.19, 95 % CI, −0.35 to −0.03, p = 0.01; MCV adjusted β = −2.22, 95 % CI, −3.43 to −1.01, p = 0.01; MCH adjusted β = −0.63, 95 % CI, −1.15 to - 0.12, p = 0.01; and MCHC adjusted β = −0.67, 95 % CI, −1.21 to −0.14, p = 0.01), respectively.
Conclusion: This study provides further evidence from a low income country that any H. pylori infection up to age 6.5 is associated with higher prevalence of anaemia, and reduction of haemoglobin level and red cell indices at age 6.5
Global trends in opioid use for pain management in acute pancreatitis: A multicentre prospective observational study

United European Gastroenterology Journal published by Wiley Periodicals LLC on behalf of United European Gastroenterology.Background: Since there is no current international consensus on the optimal approach for pain management in acute pancreatitis (AP), analgesic practices may vary across different healthcare settings. Objective: This study explored global disparities in analgesic use, in particular opioids, during admission and at discharge in hospitalised AP patients. Methods: This was a post hoc analysis of the prospective PAINAP database, which included all admissions for AP between April and June 2022 with a 1-month follow-up. Demographic details, analgesic use, and clinical outcomes were recorded during admission and at discharge. Odds ratios (ORs) for opioid use during admission and at discharge were identified using multivariable regression analyses. Results: Amongst the 1864 patients (52% males, median age 56 (interquartile range, 41–71)) across three different continents, simple analgesics were predominantly used as the primary analgesic (70%). Opioid use during admission was lowest in European centres (67%). Admission in Asian (OR, 2.53 (95% confidence interval (CI), 1.59–4.04), p < 0.001), and Australian (OR, 5.81 (95% CI, 3.19–10.56), p < 0.001) centres was associated with opioid administration during admission compared with European centres. Increased pain severity, longer pre-admission pain duration, organ failure, and longer length of admission increased opioid use during admission. At discharge, Asian (OR, 2.01 (95% CI, 1.40–2.88), p < 0.001) and Australian (OR, 1.91 (95% CI, 1.28–2.85), p = 0.002) centres were associated with opioid prescription compared with European centres. Increased pain severity, longer pre-admission pain duration, acute necrotic collections, and walled-off necrosis also increased the likelihood of opioid prescription at discharge. Conclusion: There are substantial intercontinental differences in opioid use for AP pain. Accordingly, there is a need for international guidelines on pain management in AP
The effects of new 2030 scenario: Reduction of short-circuit power and widening of voltage dips
In Italy, 80% of PV installations are at MV and LV levels, which makes it particularly challenging to control them from the national dispatch centre; this leads to an increase of the reverse power flow in the primary and secondary substations, increasing pressure on the existing measuring and protection systems and on voltage control. The National Strategic Plan, approved on November 10 th , 2017 by the Ministry of Economic Development and the Ministry of the Environment, has launched an ambitious challenge, e.g., phasing-out of coal and the increase of electricity from renewable sources: more than 55% by 2030 of gross final consumption. The power system must be able to withstand unplanned events and manage their impacts on the network, thus reducing the risk of cascading effects and maintaining suitable quality of supply. During operation with a high percentage of RES connected to the network through inverters, the system is weaker (lower short circuit power) and, with currently adopted controls, less able to react to emerging faults. With the increase of PV installations, also due to reduction of rotating synchronous machines connected to transmission grid, there is lower Short-circuit-Power available and therefore voltage dips generated at transmission level have larger impact (c.g. area impacting supply quality widens hundreds of kilometers away from the event). This paper summarizes the main challenges in terms of impacting supply quality for the Italian Power system in a new scenario with more than 50% RES by 2030 of gross final consumption and suggests some computation procedures to investigate the phenomenon
