    Projecting prevalence by stage of care for prostate cancer and estimating future health service needs: protocol for a modelling study

    Introduction Current strategies for the management of prostate cancer are inadequate in Australia. We will, in this study, estimate current service needs and project the future needs for prostate cancer patients in Australia. Methods and analysis First, we will project the future prevalence of prostate cancer for 2010-2018 using data for 1972-2008 from the New South Wales (NSW) Central Cancer Registry. These projections, based on modelled incidence and survival estimates, will be estimated using PIAMOD (Prevalence, Incidence, Analysis MODel) software. Then the total prevalence will be decomposed into five stages of care: initial care, continued monitoring, recurrence, last year of life and long-term survivor. Finally, data from the NSW Prostate Cancer Care and Outcomes Study, including data on patterns of treatment and associated quality of life, will be used to estimate the type and amount of services that will be needed by prostate cancer patients in each stage of care. In addition, Central Cancer Registry episode data will be used to estimate transition rates from localised or locally advanced prostate cancer to metastatic disease. Medicare and Pharmaceutical Benefits data, linked with Prostate Cancer Care and Outcomes Study data, will be used to complement the Cancer Registry episode data. The methods developed will be applied Australia-wide to obtain national estimates of the future prevalence of prostate cancer for different stages of clinical care. Ethics and dissemination This study was approved by the NSW Population and Health Services Research Ethics Committee.     Effects of scalding and dehairing of pig carcasses at abattoirs on the visibility of welfare-related lesions

    There is increasing interest in developing abattoir-based welfare measures for pigs.The primary aim of this study was to determine the most appropriate place on theslaughter line to conduct assessments of welfare-related lesions, namely apparentaggression-related skin lesions (hereafter referred to as 'skin lesions'), loin bruising andapparent tail biting damage. The study also lent itself to an assessment of theprevalence of these lesions, and the extent to which they were linked with productionparameters. Finishing pigs processed at two abattoirs on the Island of Ireland (n =1950 in abattoir A, and n = 1939 in abattoir B) were used. Data were collected over 6days in each abattoir in July 2014. Lesion scoring took place at two points on theslaughter line: (1) at exsanguination (Slaughter Stage 1 [SS1]), and (2) followingscalding and dehairing of carcasses (Slaughter Stage 2 [SS2]). At both points, eachcarcass was assigned a skin and tail lesion score ranging from 0 (lesion absent) to 3 or4 (severe lesions), respectively. Loin bruising was recorded as present or absent.Differences in the percentage of pigs with observable lesions of each type werecompared between SS1 and SS2 using McNemar/McNemar-Bowker tests. Theassociations between each lesion type, and both cold carcass weight andcondemnations, were examined at batch level using Pearson's correlations. Batch wasdefined as the group of animals with a particular farm identification code on a givenday. The overall percentage of pigs with a visible skin lesion (i.e. score > 0) decreasedbetween SS1 and SS2 (P<0.001). However, the percentage of pigs with a severe skinlesion increased numerically from SS1 to SS2. The percentage of pigs with a visible taillesion and with loin bruising also increased between SS1 and SS2 (P<0.001). Therewas a positive correlation between the percentage of carcasses that were partiallycondemned, and the percentage of pigs with skin lesions, tail lesions and loin bruising(P<0.05). Additionally, as the batch-level frequency of each lesion type increased,average cold carcass weight decreased (P<0.001). These findings suggest that severeskin lesions, tail lesions and loin bruising are more visible on pig carcasses after theyhave been scalded and dehaired, and that this is when abattoir-based lesion scoringshould take place. The high prevalence of all three lesion types, and the links witheconomically important production parameters, suggests more research into identifying key risk factors is warranted

    Photodisintegration of Three-Body Nuclei with Realistic 2N and 3N Forces

    Total photonuclear absorption cross sections of 3^3H and 3^3He are studied using realistic NN and NNN forces. Final state interactions are fully included. Two NN potential models, the AV14 and the r-space Bonn-A potentials, are considered. For the NNN forces the Urbana-VIII and Tucson-Melbourne models are employed. We find the cross section to be sensitive to nuclear dynamics. Of particular interest in this work is the effect which NNN forces have on the cross section. The addition of NNN forces not only lowers the peak height but increases the cross section beyond 70 MeV by roughly 15%. Cross sections are computed using the Lorentz integral transform method.Comment: Results for Bonn potential with model Bonn rA instead of model rB. The Bonn rB results contained a small inexactness. After the correction it turned out that Bonn rA is more suited for our purpose because it leads to a binding energy of 8.15 MeV (about 0.25 MeV more than Bonn rB). In addition the results for the other realistic potentials models are improved at low energies (HH expansion was not completely convergent for the low-energy results). LaTeX, 8 pages, 4 ps figure

    Comparison of human uterine cervical electrical impedance measurements derived using two tetrapolar probes of different sizes

    BACKGROUND We sought to compare uterine cervical electrical impedance spectroscopy measurements employing two probes of different sizes, and to employ a finite element model to predict and compare the fraction of electrical current derived from subepithelial stromal tissue. METHODS Cervical impedance was measured in 12 subjects during early pregnancy using 2 different sizes of the probes on each subject. RESULTS Mean cervical resistivity was significantly higher (5.4 vs. 2.8 Ωm; p < 0.001) with the smaller probe in the frequency rage of 4–819 kHz. There was no difference in the short-term intra-observer variability between the two probes. The cervical impedance measurements derived in vivo followed the pattern predicted by the finite element model. CONCLUSION Inter-electrode distance on the probes for measuring cervical impedance influences the tissue resistivity values obtained. Determining the appropriate probe size is necessary when conducting clinical studies of resistivity of the cervix and other human tissues

    Sustained expression of microRNA-155 in hematopoietic stem cells causes a myeloproliferative disorder

    Mammalian microRNAs are emerging as key regulators of the development and function of the immune system. Here, we report a strong but transient induction of miR-155 in mouse bone marrow after injection of bacterial lipopolysaccharide (LPS) correlated with granulocyte/monocyte (GM) expansion. Demonstrating the sufficiency of miR-155 to drive GM expansion, enforced expression in mouse bone marrow cells caused GM proliferation in a manner reminiscent of LPS treatment. However, the miR-155–induced GM populations displayed pathological features characteristic of myeloid neoplasia. Of possible relevance to human disease, miR-155 was found to be overexpressed in the bone marrow of patients with certain subtypes of acute myeloid leukemia (AML). Furthermore, miR-155 repressed a subset of genes implicated in hematopoietic development and disease. These data implicate miR-155 as a contributor to physiological GM expansion during inflammation and to certain pathological features associated with AML, emphasizing the importance of proper miR-155 regulation in developing myeloid cells during times of inflammatory stress

    Enhanced local-type inflationary trispectrum from a non-vacuum initial state

    We compute the primordial trispectrum for curvature perturbations produced during cosmic inflation in models with standard kinetic terms, when the initial quantum state is not necessarily the vacuum state. The presence of initial perturbations enhances the trispectrum amplitude for configuration in which one of the momenta, say k3k_3, is much smaller than the others, k3k1,2,4k_3 \ll k_{1,2,4}. For those squeezed configurations the trispectrum acquires the so-called local form, with a scale dependent amplitude that can get values of order ϵ(k1/k3)2 \epsilon ({k_1}/{k_3})^2. This amplitude can be larger than the prediction of the so-called Maldacena consistency relation by a factor 10610^6, and can reach the sensitivity of forthcoming observations, even for single-field inflationary models.Comment: 11 pages, 1 figure.     Initial correlations effects on decoherence at zero temperature

    We consider a free charged particle interacting with an electromagnetic bath at zero temperature. The dipole approximation is used to treat the bath wavelengths larger than the width of the particle wave packet. The effect of these wavelengths is described then by a linear Hamiltonian whose form is analogous to phenomenological Hamiltonians previously adopted to describe the free particle-bath interaction. We study how the time dependence of decoherence evolution is related with initial particle-bath correlations. We show that decoherence is related to the time dependent dressing of the particle. Moreover because decoherence induced by the T=0 bath is very rapid, we make some considerations on the conditions under which interference may be experimentally observed.Comment: 16 pages, 1 figur
