State-of-the-art in product service-systems
A Product Service-System (PSS) is an integrated combination of products and services.
This western concept embraces a service-led competitive strategy, environmental sustainability,
and the basis to differentiate from competitors who simply offer lower priced products. This
paper aims to report the state-of-the-art of PSS research by presenting a clinical review of
literature currently available on this topic. The literature is classified and the major outcomes
of each study are addressed and analysed. On this basis, this paper defines the PSS concept,
reports on its origin and features, gives examples of applications along with potential benefits
and barriers to adoption, summarises available tools and methodologies, and identifies future
research challenges
Effects of short-term exposure to particulate matter air pollution on cognitive performance
This paper assesses the effect of short-term exposure to particulate matter (PM) air pollution on human cognitive performance via a double cross over experimental design. Two distinct experiments were performed, both of which exposed subjects to low and high concentrations of PM. Firstly, subjects completed a series of cognitive tests after being exposed to low ambient indoor PM concentrations and elevated PM concentrations generated via candle burning, which is a well-known source of PM. Secondly, a different cohort underwent cognitive tests after being exposed to low ambient indoor PM concentrations and elevated ambient outdoor PM concentrations via commuting on or next to roads. Three tests were used to assess cognitive performance: Mini-Mental State Examination (MMSE), the Stroop Color and Word test, and Ruff 2 & 7 test. The results from the MMSE test showed a statistically robust decline in cognitive function after exposure to both the candle burning and outdoor commuting compared to ambient indoor conditions. The similarity in the results between the two experiments suggests that PM exposure is the cause of the short-term cognitive decline observed in both. The outdoor commuting experiment also showed a statistically significant short-term cognitive decline in automatic detection speed from the Ruff 2 and 7 selective attention test. The other cognitive tests, for both the candle and commuting experiments, showed no statistically significant difference between the high and low PM exposure conditions. The findings from this study are potentially far reaching; they suggest that elevated PM pollution levels significantly affect short term cognition. This implies average human cognitive ability will vary from city to city and country to country as a function of PM air pollution exposure.</p
Heart rate variability and target organ damage in hypertensive patients
Background:
We evaluated the association between linear standard Heart Rate Variability (HRV) measures and vascular, renal and cardiac target organ damage (TOD).
Methods:
A retrospective analysis was performed including 200 patients registered in the Regione Campania network (aged 62.4 ± 12, male 64%). HRV analysis was performed by 24-h holter ECG. Renal damage was assessed by estimated glomerular filtration rate (eGFR), vascular damage by carotid intima-media thickness (IMT), and cardiac damage by left ventricular mass index.
Results:
Significantly lower values of the ratio of low to high frequency power (LF/HF) were found in the patients with moderate or severe eGFR (p-value < 0.001). Similarly, depressed values of indexes of the overall autonomic modulation on heart were found in patients with plaque compared to those with a normal IMT (p-value <0.05). These associations remained significant after adjustment for other factors known to contribute to the development of target organ damage, such as age. Moreover, depressed LF/HF was found also in patients with left ventricular hypertrophy but this association was not significant after adjustment for other factors.
Conclusions:
Depressed HRV appeared to be associated with vascular and renal TOD, suggesting the involvement of autonomic imbalance in the TOD. However, as the mechanisms by which abnormal autonomic balance may lead to TOD, and, particularly, to renal organ damage are not clearly known, further prospective studies with longitudinal design are needed to determine the association between HRV and the development of TOD

The contribution of cooking appliances and residential traffic proximity to aerosol personal exposure
Purpose Indoor and outdoor factors affect personal exposure to air pollutants. Type of cooking appliance (i.e. gas, electricity),
and residential location related to traffic are such factors. This research aims to investigate the effect of cooking with gas and
electric appliances, as an indoor source of aerosols, and residential traffic as outdoor sources, on personal exposures to particulate
matter with an aerodynamic diameter lower than 2.5 μm (PM2.5), black carbon (BC), and ultrafine particles (UFP).
Methods Forty subjects were sampled for four consecutive days measuring personal exposures to three aerosol pollutants, namely
PM2.5, BC, and UFP, which were measured using personal sensors. Subjects were equally distributed into four categories according
to the use of gas or electric stoves for cooking, and to residential traffic (i.e. houses located near or away from busy roads).
Results/conclusion Cooking was identified as an indoor activity affecting exposure to aerosols, with mean concentrations during
cooking ranging 24.7–50.0 μg/m3 (PM2.5), 1.8–4.9 μg/m3 (BC), and 1.4 × 104
–4.1 × 104 particles/cm3 (UFP). This study also
suggest that traffic is a dominant source of exposure to BC, since people living near busy roads are exposed to higher BC
concentrations than those living further away from traffic. In contrast, the contribution of indoor sources to personal exposure to
PM2.5 and UFP seems to be greater than from outdoor traffic sources. This is probably related to a combination of the type of
building construction and a varying range of activities conducted indoors. It is recommended to ensure a good ventilation during
cooking to minimize exposure to cooking aerosols
Neuronal circuitry for pain processing in the dorsal horn
Neurons in the spinal dorsal horn process sensory information, which is then transmitted to several brain regions, including those responsible for pain perception. The dorsal horn provides numerous potential targets for the development of novel analgesics and is thought to undergo changes that contribute to the exaggerated pain felt after nerve injury and inflammation. Despite its obvious importance, we still know little about the neuronal circuits that process sensory information, mainly because of the heterogeneity of the various neuronal components that make up these circuits. Hybrid SPECT/CT for the assessment of a painful hip after uncemented total hip arthroplasty
Background The diagnosis of hip pain after total hip replacement (THR)
represents a highly challenging question that is of increasing concern to
orthopedic surgeons. This retrospective study assesses bone scintigraphy with
Hybrid SPECT/CT for the diagnosis of painful THR in a selected cohort of
patients. Methods Bone SPECT/CT datasets of 23 patients (mean age 68.9 years)
with a painful hip after THR were evaluated. Selection of the patients
required an inconclusive radiograph, normal serum levels of inflammatory
parameters (CRP and ESR) or a negative aspiration of the hip joint prior to
the examination. The standard of reference was established by an
interdisciplinary adjudication-panel using all imaging data and clinical
follow-up data (>12 month). Pathological and physiological uptake patterns
were defined and applied. Results The cause of pain in this study group could
be determined in 18 out of 23 cases. Reasons were aseptic loosening (n = 5),
spine-related (n = 5), heterotopic ossification (n = 5), neuronal (n = 1),
septic loosening (n = 1) and periprosthetic stress fracture (n = 1). In (n =
5) cases the cause of hip pain could not be identified. SPECT/CT imaging
correctly identified the cause of pain in (n = 13) cases, in which the
integrated CT-information led to the correct diagnosis in (n = 4) cases,
mainly through superior anatomic correlation. Loosening was correctly assessed
in all cases with a definite diagnosis. Conclusions SPECT/CT of THA reliably
detects or rules out loosening and provides valuable information about
heterotopic ossifications. Furthermore differential diagnoses may be detected
with a whole-body scan and mechanical or osseous failure is covered by CT-
imaging. SPECT/CT holds great potential for imaging-based assessment of
painful prostheses
