    Bilevel positive airway pressure in two moments after bariatric surgery

    Objective: To investigate the use of Bilevel Positive Airway Pressure (BiPAP) in morbidly obese individuals in two moments following bariatric surgery (Roux-en-Y gastric bypass): post-anesthetic recovery (PAR) and first postoperative day (1PO). DESIGN: Randomized and blinded clinical trial. Methods: We studied 40 morbidly obese individuals aged between 25 and 55 years who underwent pulmonary function test and chest X-ray preoperatively, and on the day of discharge (2nd day after surgery). They were randomly allocated into two groups: PAR-G (BiPAP in PAR for one hour), and 1PO-G (BIPAP for one hour on the 1PO). Results: In the PAR-G and 1PO-G, respectively there were significant reductions in slow vital capacity (SVC) (p=0.0007 vs. p<0.0001), inspiratory reserve volume (IRV) (p=0.0016 vs. p=0.0026), and forced vital capacity (FVC) (p=0.0013 vs. p<0.0001) and expiratory reserve volume (ERV) was maintained only for the PAR-G (p=0.4446 vs. p=0.0191). Comparing the groups, the SVC (p=0.0027) and FVC (p=0.0028) showed a significant difference between the treatments, while the PAR-G showed smaller declines in these capacities. The prevalence of atelectasis was 10% for the PAR-G and 30% for the 1PO-G (p=0.0027).     New Pharmacological Agents to Aid Smoking Cessation and Tobacco Harm Reduction: What has been Investigated and What is in the Pipeline?

    A wide range of support is available to help smokers to quit and aid attempts at harm reduction, including three first-line smoking cessation medications: nicotine replacement therapy, varenicline and bupropion. Despite the efficacy of these, there is a continual need to diversify the range of medications so that the needs of tobacco users are met. This paper compares the first-line smoking cessation medications to: 1) two variants of these existing products: new galenic formulations of varenicline and novel nicotine delivery devices; and 2) twenty-four alternative products: cytisine (novel outside of central and eastern Europe), nortriptyline, other tricyclic antidepressants, electronic cigarettes, clonidine (an anxiolytic), other anxiolytics (e.g. buspirone), selective 5-hydroxytryptamine (5-HT) reuptake inhibitors, supplements (e.g. St John’s wort), silver acetate, nicobrevin, modafinil, venlafaxine, monoamine oxidase inhibitors (MAOI), opioid antagonist, nicotinic acetylcholine receptors (nAChR) antagonists, glucose tablets, selective cannabinoid type 1 receptor antagonists, nicotine vaccines, drugs that affect gamma-aminobutyric acid (GABA) transmission, drugs that affect N-methyl-D-aspartate receptors (NMDA), dopamine agonists (e.g. levodopa), pioglitazone (Actos; OMS405), noradrenaline reuptake inhibitors, and the weight management drug lorcaserin. Six criteria are used: relative efficacy, relative safety, relative cost, relative use (overall impact of effective medication use), relative scope (ability to serve new groups of patients), and relative ease of use (ESCUSE). Many of these products are in the early stages of clinical trials, however, cytisine looks most promising in having established efficacy and safety and being of low cost. Electronic cigarettes have become very popular, appear to be efficacious and are safer than smoking, but issues of continued dependence and possible harms need to be considered

    Diagnóstico de hiperquilomicronemia familiar: um relato de caso raro

    A síndrome de hiperquilomicronemia familiar (SQF) é uma doença autossômica recessiva rara, resultante de mutações na via da lipoproteína lipase, que levam à hipertrigliceridemia e acúmulo de quilomícrons no plasma. As manifestações incluem dor abdominal recorrente e pancreatite aguda potencialmente fatal. Este estudo objetiva relatar um caso de SQF e revisar brevemente a literatura. Trata-se de um estudo descritivo do tipo relato de caso, aprovado pelo Comitê de Ética em Pesquisa do Centro Universitário CESMAC (CAAE: 78345824.2.0000.0039, Parecer: 6.858.992). O caso é de uma paciente do sexo feminino, 15 anos, assintomática, apresentou triglicerídeos e colesterol elevados em exames de rotina, sendo encaminhada ao endocrinologista. Iniciou tratamento com Sinvastatina e Fenofibrato, além de modificações no estilo de vida. Aos 33 anos, desenvolveu pancreatite aguda, necessitando internação. Em 2019, aos 35 anos, teve três episódios de pancreatite, sendo encaminhada ao geneticista, que confirmou o diagnóstico de SQF. Atualmente, com 40 anos, é acompanhada no Centro Ambulatorial do CESMAC, em uso de Fenofibrato, Ômega 3, Orlistat, Vitamina D, Cloridrato de Metformina XR e Rosuvastatina cálcica. Mantém restrição alimentar e apresenta dor leve ao ingerir gorduras, sem novas crises pancreáticas. Conclui-se que o acompanhamento clínico e laboratorial contínuo possibilitou uma abordagem terapêutica eficaz e menos invasiva, preservando a qualidade de vida da paciente

    Mental health problems among medical students in Brazil: a systematic review and meta-analysis

    Objective: To provide a comprehensive picture of mental health problems (MHPs) in Brazilian medical students by documenting their prevalence and association with co-factors. Methods: We systematically searched the MEDLINE/PubMed, SciELO, LILACS, and PsycINFO databases for cross-sectional studies on the prevalence of MHPs among medical students in Brazil published before September 29, 2016. We pooled prevalences using a random-effects meta-analysis, and summarized factors associated with MHP. Results: We included 59 studies in the analysis. For meta-analyses, we identified the summary prevalence of different MHPs, including depression (25 studies, prevalence 30.6%), common mental disorders (13 studies, prevalence 31.5%), burnout (three studies, prevalence 13.1%), problematic alcohol use (three studies, prevalence 32.9%), stress (six studies, prevalence 49.9%), low sleep quality (four studies, prevalence 51.5%), excessive daytime sleepiness (four studies, prevalence 46.1%), and anxiety (six studies, prevalence 32.9%). Signs of lack of motivation, emotional support, and academic overload correlated with MHPs. Conclusion: Several MHPs are highly prevalent among future physicians in Brazil. Evidence-based interventions and psychosocial support are needed to promote mental health among Brazilian medical students

    Fatores associados ao desenvolvimento neuropsicomotor em crianças de 6-18 meses de vida inseridas em creches públicas do Município de João Pessoa, Paraíba, Brasil

    O estudo objetivou verificar a prevalência de alteração no desenvolvimento neuropsicomotor em lactentes inseridos nas creches públicas de João Pessoa, Paraíba, Brasil, e analisar fatores associados ao desenvolvimento infantil. Realizou-se um estudo seccional com o censo de crianças entre 6 e 18 meses de vida inseridas nas creches. O desenvolvimento infantil foi avaliado pelo Teste de Triagem de Denver II. Variáveis de natureza biológica, materna, social, demográfica e relativas à creche foram investigadas através de questionário e formulário. As associações foram ajustadas por regressão logística. A prevalência de alteração no desenvolvimento foi 52,7%. Associou-se à alteração no desenvolvimento a criança ter mais que 12 meses de idade (RC = 4,3), o parto vaginal (RC = 4,4), a fototerapia (RC = 7,9) e a creche não ter apoio da Estratégia Saúde da Família (RC = 2,9). Os achados sugerem que o desenvolvimento infantil é o reflexo das condições familiares, bem como da assistência recebida pelos serviços de educação e saúde
