    Vape away: Why a Minimalist Regulatory Structure Is the Best Option for FDA E-Cigarette Regulation

    This comment argues that the FDA should regulate ecigarettes, but in doing so, it should take a minimalist approach to regulation. Instead of continuing its attempts to regulate tobacco products under the drug and device provisions of the Federal Food, Drug, and Cosmetic Act ( FDCA ), an incredibly stringent statute, the FDA should focus its efforts under the Tobacco Control Act ( TCA ), which is tailored specifically to regulate tobacco products. While the FDA has extensive regulatory options under the TCA, much of this authority should not be applied to ecigarettes. Part II will provide background information on ecigarettes generally as well as steps the FDA has taken to regulate e-cigarettes and other tobacco products. Part III addresses the FDA\u27s limited regulatory ability under the TCA and describes why it has no regulatory authority over e-cigarettes under the FDCA. Part IV presents alternatives to aggressive regulation of e-cigarettes as well as the benefits of a minimalist federal regulatory scheme that allows ecigarette and other tobacco product users to fully benefit from the potentially reduced health risks of e-cigarettes

    The regulatory mechanisms underlying the response to pro-inflammatory cytokines in cardiac diseases are poorly understood. Here, we use iPSC-derived cardiovascular progenitor cells (CVPCs) to model the response to interferon gamma (IFNγ) in human cardiac tissue. We generate RNA-seq and ATAC-seq for four CVPCs that were treated with IFNγ and compare them with paired untreated controls. Transcriptional differences after treatment show that IFNγ initiates an innate immune cell-like response, shifts the CVPC transcriptome toward coronary artery and aorta profiles, and stimulates expression of endothelial cell-specific genes. Analysis of the accessible chromatin shows that IFNγ is a potent chromatin remodeler and establishes an IRF-STAT immune-cell like regulatory network. Finally, we show that 11 GWAS risk variants for 8 common cardiac diseases overlap IFNγ-upregulated ATAC-seq peaks. Our findings reveal insights into IFNγ-induced activation of an immune-like regulatory network in human cardiac tissue and the potential role that regulatory elements in this pathway play in common cardiac diseases

    The relationship between the development of technologies and the history of the cultural and agricultural landscape is linked to the concepts of "cultural landscape", understood as a space in continuous construction that changes with the change of individual, collective, social and cultural relationships of the inhabitants of the territory, or of the "cultural inhabitants", citizens who are producers of culture, rather than users. A vision of the "future as an open place" emerges, understood as a place of usability and sharing of all human, material and immaterial productions.Technologies, within a similar perspective, are presented as the historical evolution of téchne, whose degree of development today allows an extension of the level of human action.This study, in agreement with the scientific literature based on the use of recently developed digital models, demonstrates that the mainly agricultural territory of Basilicata, historically the site of complex social relations, has created a traditional rural society in which the concept of neighborhood and the spatial connotation also had the symbolic value of sharing knowledge and practices, relationships based on inclusiveness and sustainability. The diffusion of 5G technology is generating important cultural transformations. What used to be the neighborhood community in Matera (IT) - also following the activities launched with the CTEMT project and the social consequences of the Covid-19 pandemic - is now becoming a virtual community for sharing knowledge and practices , beliefs and values, including the use and management of cultural heritage, which takes place through the network, and therefore using applications that promote a transformative intervention of the landscape, such as to make it functional to human needs, and, at the same time, sustainable with respect to the perpetuation of ecosystem relationships.The diffusion of 5G technology, is generating important cultural transformations. What in the past was, in Matera (IT), the neighbourhood community - also as a result of the activities launched with the CTEMT project and the social consequences of the Covid-19 pandemic - now becomes a virtual community, sharing knowledge and practices, beliefs and values, including the use and management of cultural heritage, occurs through the network with the use of applications that promote accessibility and sustainability in both the urban and agricultural landscape. As argued by the International Union for Conservation of Nature (IUCN), the attention to the dynamic conservation of the landscape should not be placed so much to the "culture itself" or to the "nature itself" but rather to the relationship between these two dynamic components has been established, but also from the holistic mentioned many times, attentive to the values of identity and comforted by the knowledge and decoding of the intangible heritage, from which we deduce the active role, shared social behaviours, the mechanisms of transmission of knowledge and transgenerational awareness also thanks to the complex and fascinating universe of uses, traditions, rituals and rites that are an important tool of conscious management of the landscape and its culture. The conscious use of artificial intelligence is the concretion of the virtuous relationship between Humanism and technologies. For the biodiversity it is a support to the recognition of the species, in particular of the native ones, and it allows people to recognize themselves culturally and find into the biodiversity a collective and cultural belonging to the community and to the landscape. Therefore, thanks to the use of new technologies biodiversity becomes an historical-anthropological archive of knowledge and practices of a territory, and new technologies a powerful tool for the conservation of the cultural heritage

    Cemented Polyethylene Cups in Patients Younger Than 40 Years

    Although uncemented cup implants frequently are used in young patients, we believe long-term survival rates of cups in these patients are somewhat disappointing, and therefore we have continued to use cemented cups in primary THA, even in young patients. However, in cases of acetabular bone stock defects, we also use bone impaction grafting. We prospectively followed 130 patients with 175 cemented cups; no patients were lost to followup. The mean age of the patients at surgery was 31 years (range, 16–39 years). An acetabular reconstruction with bone impaction grafting was performed in 84 hips (48%). The minimum followup was 2 years (average, 8.1 years; range, 2.0–18.5 years). Twenty-one of the 175 cups (12%) were revised at an average of 8.1 years (range, 2.0–18.5 years). Reasons for revision were infection (one early, seven late), recurrent dislocations (two), traumatic loosening (one), and aseptic loosening (10). The 10-year survival rate of all cemented cups with end point of revision for any cause was 85%. Survival with end point of aseptic loosening of all cups was 92%. Survival with end point of revision for aseptic loosening was 90% for the cups without impaction grafting and 95% for the cups with impaction grafting. We believe cemented acetabular cups in young patients have acceptable midterm survival; however, in the case of acetabular bone defects, we recommend reconstruction with impaction grafting

    Country-specific birth weight and length in type 1 diabetes high-risk HLA genotypes in combination with prenatal characteristics

    Objective:To examine the relationship between high-risk human leukocyte antigen (HLA) genotypes for type 1 diabetes and birth size in combination with prenatal characteristics in different countries.Study Design:Four high-risk HLA genotypes were enrolled in the Environmental determinants of Diabetes in the Young study newborn babies from the general population in Finland, Germany, Sweden and the United States. Stepwise regression analyses were used to adjust for country, parental physical characteristics and environmental factors during pregnancy.Result:Regression analyses did not reveal differences in birth size between the four type 1 diabetes high-risk HLA genotypes. Compared with DQ 4/8 in each country, (1) DQ 2/2 children were heavier in the United States (P=0.028) mostly explained however, by parental weight; (2) DQ 2/8 (P=0.023) and DQ 8/8 (P=0.046) children were longer in Sweden independent of parents height and as well as (3) in the United States for DQ 2/8 (P=0.023), but again dependent on parental height.Conclusion:Children born with type 1 diabetes high-risk HLA genotypes have comparable birth size. Longitudinal follow-up of these children should reveal whether birth size differences between countries contribute to the risk for islet autoimmunity and type 1 diabetes.Journal of Perinatology advance online publication, 28 April 2011; doi:10.1038/jp.2011.26
