Lived Experience of Caregivers of Family-Centered Care in the Neonatal Intensive Care Unit: “Evocation of Being at Home
Background: In recent decades, family-centered care (FCC) has come to be known, accepted, and reported as the best care strategy
for admitted children and their families. However, in spite of the increasing application of this approach, the experiences of the
caregivers have not yet been studied.
Objectives: The present study aimed at the description and interpretation of the FCC experience in two neonatal intensive care
units (NICU) at Shiraz University of Medical Sciences.
Methods: This study was conducted through the hermeneutic phenomenological approach. Semi-structured interviews were conducted
with 17 professional and familial caregivers, and their interactions were observed in three work shifts. The interviews were
audiotaped and transcribed verbatim. After observations, field notes were also written. Finally, the data were analyzed through van
Manen’s methodology.
Results: One of the essential themes that emerged in this study was the “evocation of being at home” among familial and even professional
caregivers. This theme had three subthemes: i.e., “meta-family interaction,” “comprehensive support,” and “reconstruction
of a normal family.” Accordingly, FCC eliminated borders between professional and non-professional caregivers and built close
relationships among them in the NICU. It also provided for the needs of neonates, their families, and even professional caregivers
through perceived and received support.
Conclusions: Parents of the neonates admitted to the NICU experience hard moments. They not only play the role of primary caregivers,
but they also receive the care. Focusing on the different meanings of this care from the caregivers’ points of view and having
managers provide certain requirements can guarantee the establishment of comprehensive care for clients and proper support for
the staff in this uni
Who is dominant? Occupational Health and Safety management in Chinese shipping
This paper investigates the implementation of the International Safety Management
(ISM) Code in the Chinese chemical shipping industry. In particular, it examines the
tension between management focus on speedy production and seafarers’ participation
in safety related decision making and analyses how this tension is managed. It shows
that while on paper companies have policies stating safety commitment in compliance
with the ISM Code, in practice shore management tends to prioritise efficient
production. When Occupational Health and Safety (OHS) and ship’s sailing schedules
are in conflict, managers implicitly request shipmasters to prioritise the ‘core interest’
of the company. Although the ISM Code endows shipmasters with overriding
authorities in relation to shipboard safety management, they tend to read between the
lines and tacitly follow managers’ intentions. The study suggests that if the ISM
implementation makes a difference, it is the practice that managers become more
subtle in giving orders to exert their dominance. The study further reveals that the
management’s practice is not only irresponsive to seafarers’ safety concerns but also
makes rather limited contributions to promote OHS Management
Impact of Dreissena fouling on the physiological condition of native and invasive bivalves : interspecific and temporal variations
The impact of Dreissena fouling on unionids
has hardly been studied in Europe, despite the fact
that in some ecosystems (e.g. Lake Balaton, Hungary)
infestations of several hundreds to a thousand individuals
per unionid have been observed. At present,
the zebra mussel Dreissena polymorpha is a dominant
species in Lake Balaton and in the last decade three
other invasive bivalves were introduced, potentially
increasing the pressure on native unionid survival. We
examined whether the fouling of dreissenids (zebra
and quagga (D. rostriformis bugensis) mussels) has a
negative impact on native (Anodonta anatina, Unio
pictorum and U. tumidus) and invasive (Corbicula
fluminea and Sinanodonta woodiana) bivalves and
whether there are any interspecific and temporal
variations in fouling intensity and physiological
condition measured by standard condition index and
glycogen content. A significant negative impact was detected on native unionids only in July and September
(no impact was detected in May), when the fouling
rate was high. For invasive species, a significant
negative impact was detected on S. woodiana with a
high level of dressenid infestation; whereas no significant
impact was detected on C. fluminea. Overall, this
study confirms that Dreissena may threaten unionid
species including the invasive S. woodiana, although
high interspecific and temporal variations were
observed. This situation should be taken into account
in future ecological and conservational assessments
because species respond differently to Dreissena
fouling and effects seem to be more pronounced in
late summer/early autumn. In addition, this study
provides the first evidence that the invasive C.
fluminea appear to be less vulnerable to dressenid

Multi-messenger observations of a binary neutron star merger
On 2017 August 17 a binary neutron star coalescence candidate (later designated GW170817) with merger time 12:41:04 UTC was observed through gravitational waves by the Advanced LIGO and Advanced Virgo detectors. The Fermi Gamma-ray Burst Monitor independently detected a gamma-ray burst (GRB 170817A) with a time delay of ~1.7 s with respect to the merger time. From the gravitational-wave signal, the source was initially localized to a sky region of 31 deg2 at a luminosity distance of 40+8-8 Mpc and with component masses consistent with neutron stars. The component masses were later measured to be in the range 0.86 to 2.26 Mo. An extensive observing campaign was launched across the electromagnetic spectrum leading to the discovery of a bright optical transient (SSS17a, now with the IAU identification of AT 2017gfo) in NGC 4993 (at ~40 Mpc) less than 11 hours after the merger by the One- Meter, Two Hemisphere (1M2H) team using the 1 m Swope Telescope. The optical transient was independently detected by multiple teams within an hour. Subsequent observations targeted the object and its environment. Early ultraviolet observations revealed a blue transient that faded within 48 hours. Optical and infrared observations showed a redward evolution over ~10 days. Following early non-detections, X-ray and radio emission were discovered at the transient’s position ~9 and ~16 days, respectively, after the merger. Both the X-ray and radio emission likely arise from a physical process that is distinct from the one that generates the UV/optical/near-infrared emission. No ultra-high-energy gamma-rays and no neutrino candidates consistent with the source were found in follow-up searches. These observations support the hypothesis that GW170817 was produced by the merger of two neutron stars in NGC4993 followed by a short gamma-ray burst (GRB 170817A) and a kilonova/macronova powered by the radioactive decay of r-process nuclei synthesized in the ejecta
Frontal lobe changes occur early in the course of affective disorders in young people
<p>Abstract</p> <p>Background</p> <p>More severe and persistent forms of affective disorders are accompanied by grey matter loss in key frontal and temporal structures. It is unclear whether such changes precede the onset of illness, occur early in the course or develop gradually with persistence or recurrence of illness. A total of 47 young people presenting with admixtures of depressive and psychotic symptoms were recruited from specialist early intervention services along with 33 age matched healthy control subjects. All participants underwent magnetic resonance imaging and patients were rated clinically as to current stage of illness. Twenty-three patients were identified as being at an early 'attenuated syndrome' stage, while the remaining were rated as having already reached the 'discrete disorder' or 'persistent or recurrent illness' stage. Contrasts were carried out between controls subjects and patients cohorts with attenuated syndromes and discrete disorders, separately.</p> <p>Results</p> <p>The patients that were identified as having a discrete or persisting disorder demonstrated decreased grey matter volumes within distributed frontal brain regions when contrasted to both the control subjects as well as those patients in the attenuated syndrome stage. Overall, patients who were diagnosed as more advanced in terms of the clinical stage of their illness, exhibited the greatest grey matter volume loss of all groups.</p> <p>Conclusions</p> <p>This study suggests that, in terms of frontal grey matter changes, a major transition point may occur in the course of affective illness between early attenuated syndromes and later discrete illness stages.</p
Efficacy of Combined Therapy with Amantadine, Oseltamivir, and Ribavirin In Vivo against Susceptible and Amantadine-Resistant Influenza A Viruses
The limited efficacy of existing antiviral therapies for influenza – coupled with widespread baseline antiviral resistance – highlights the urgent need for more effective therapy. We describe a triple combination antiviral drug (TCAD) regimen composed of amantadine, oseltamivir, and ribavirin that is highly efficacious at reducing mortality and weight loss in mouse models of influenza infection. TCAD therapy was superior to dual and single drug regimens in mice infected with drug-susceptible, low pathogenic A/H5N1 (A/Duck/MN/1525/81) and amantadine-resistant 2009 A/H1N1 influenza (A/California/04/09). Treatment with TCAD afforded >90% survival in mice infected with both viruses, whereas treatment with dual and single drug regimens resulted in 0% to 60% survival. Importantly, amantadine had no activity as monotherapy against the amantadine-resistant virus, but demonstrated dose-dependent protection in combination with oseltamivir and ribavirin, indicative that amantadine's activity had been restored in the context of TCAD therapy. Furthermore, TCAD therapy provided survival benefit when treatment was delayed until 72 hours post-infection, whereas oseltamivir monotherapy was not protective after 24 hours post-infection. These findings demonstrate in vivo efficacy of TCAD therapy and confirm previous reports of the synergy and broad spectrum activity of TCAD therapy against susceptible and resistant influenza strains in vitro

using encounter data from marked individuals have
made it possible to include estimates of the probability of
recruitment in population projection models. However, the
projected growth rate of the population, and the sensitivity
of projected growth to changes in recruitment, can vary
significantly depending upon both the structural form of the
model and how recruitment is parameterized. We show that
the common practices of (1) collapsing some age classes
into a single, terminal ‘aggregated’ age-class, and (2) parameterizing
recruitment using the proportion of recruited
individuals (breeders) in a given age-class may confound
analysis of age-based (Leslie) matrix projection models in
some instances, relative to state-based projection models
where recruited and pre-recruited individuals are treated as
separate states. Failing to account for these differences can
lead to misinterpretation of the relative role of recruitment in
the dynamics of an age-structured population.We show that
such problems can be avoided, either by structural changes
to the terminal aggregated age-class in age-based models,
or by using using a state-based model instead. Since all
the metrics of general interest from a classical age-based
matrix models are readily derived from a state-based model
equivalent, this suggests there may be little reason to use the
classical age-based approach in situations where recruitment
is a parameter of interest
How do ADHD children perceive their cognitive, affective, and behavioral aspects of anger expression in school setting?
<p>Abstract</p> <p>Background</p> <p>Anger is an ignored research area in children and young adolescents with Attention deficit hyperactivity disorder (ADHD) in the school setting. This study compares school anger dimensions in children and young adolescents with ADHD and a control group.</p> <p>Methods</p> <p>The subjects were a clinical sample of 67 children and young adolescents with ADHD and their parents, with a sample of 91 children from the community of similar age and gender as control group. Anger was measured by the Farsi version of the Multidimensional School Anger Inventory (MSAI).</p> <p>Results</p> <p>The scores of the two components of "Hostile Outlook" and "Positive Coping" were different between the groups. The mean scores for the Anger components did not statistically differ between the children with ADHD and ODD and ADHD without ODD, boys and girls, or different types of ADHD.</p> <p>Conclusion</p> <p>Children with ADHD do not report higher rates of experience of anger and they do not apply destructive strategies more than the control group. However, children with ADHD appear to have a more hostile outlook toward school and their coping strategy is weaker than that of the control group.</p
Swarming populations of Salmonella represent a unique physiological state coupled to multiple mechanisms of antibiotic resistance
Salmonella enterica serovar Typhimurium is capable of swarming over semi-solid surfaces. Although its swarming behavior shares many readily observable similarities with other swarming bacteria, the phenomenon remains somewhat of an enigma in this bacterium since some attributes skew away from the better characterized systems. Swarming is quite distinct from the classic swimming motility, as there is a prerequisite for cells to first undergo a morphological transformation into swarmer cells. In some organisms, swarming is controlled by quorum sensing, and in others, swarming has been shown to be coupled to increased expression of important virulence factors. Swarming in serovar Typhimurium is coupled to elevated resistance to a wide variety of structurally and functionally distinct classes of antimicrobial compounds. As serovar Typhimurium differentiates into swarm cells, the pmrHFIJKLM operon is up-regulated, resulting in a more positively charged LPS core. Furthermore, as swarm cells begin to de-differentiate, the pmr operon expression is down-regulated, rapidly reaching the levels observed in swim cells. This is one potential mechanism which confers swarm cells increased resistance to antibiotics such as the cationic antimicrobial peptides. However, additional mechanisms are likely associated with the cells in the swarm state that confer elevated resistance to such a broad spectrum of antimicrobial agents
Fluid-structure interaction in abdominal aortic aneurysms: effects of asymmetry and wall thickness
BACKGROUND: Abdominal aortic aneurysm (AAA) is a prevalent disease which is of significant concern because of the morbidity associated with the continuing expansion of the abdominal aorta and its ultimate rupture. The transient interaction between blood flow and the wall contributes to wall stress which, if it exceeds the failure strength of the dilated arterial wall, will lead to aneurysm rupture. Utilizing a computational approach, the biomechanical environment of virtual AAAs can be evaluated to study the affects of asymmetry and wall thickness on this stress, two parameters that contribute to increased risk of aneurysm rupture. METHODS: Ten virtual aneurysm models were created with five different asymmetry parameters ranging from β = 0.2 to 1.0 and either a uniform or variable wall thickness to study the flow and wall dynamics by means of fully coupled fluid-structure interaction (FSI) analyses. The AAA wall was designed to have a (i) uniform 1.5 mm thickness or (ii) variable thickness ranging from 0.5 – 1.5 mm extruded normally from the boundary surface of the lumen. These models were meshed with linear hexahedral elements, imported into a commercial finite element code and analyzed under transient flow conditions. The method proposed was then compared with traditional computational solid stress techniques on the basis of peak wall stress predictions and cost of computational effort. RESULTS: The results provide quantitative predictions of flow patterns and wall mechanics as well as the effects of aneurysm asymmetry and wall thickness heterogeneity on the estimation of peak wall stress. These parameters affect the magnitude and distribution of Von Mises stresses; varying wall thickness increases the maximum Von Mises stress by 4 times its uniform thickness counterpart. A pre-peak systole retrograde flow was observed in the AAA sac for all models, which is due to the elastic energy stored in the compliant arterial wall and the expansion force of the artery during systole. CONCLUSION: Both wall thickness and geometry asymmetry affect the stress exhibited by a virtual AAA. Our results suggest that an asymmetric AAA with regional variations in wall thickness would be exposed to higher mechanical stresses and an increased risk of rupture than a more fusiform AAA with uniform wall thickness. Therefore, it is important to accurately reproduce vessel geometry and wall thickness in computational predictions of AAA biomechanics
